Etiology
Hypothalamic-pituitary causes (low-normal FSH)
- Idiopathic delayed puberty
- Pituitary tumor
- Hypothalamic amenorrhea, eg, stress, weight change, exercise
- Anorexia nervosa

- Cushings syndrome

- Androgenic steroids
Ovarian causes (high FSH)

Pseudohermaphroditism (high LH)
