Low intraocular culture yield and imperfect concordance with blood cultures complicate pathogen-directed therapy. Presenting VA is the dominant prognostic factor, but gram-negative infections-particularly Serratia-carry markedly higher risk of globe loss. These findings support organism-informed clinical decision-making and highlight the need for improved intraocular diagnostics and targeted interventions.

- CONCLUSIONS: Among the procedures with adequate data, vitamin A deficiency is procedure-specific and common during pregnancy after RYGB. Ocular disease may present years to decades after surgery, but often improves when deficiency is recognized and treated. Certainty was limited by heterogeneity, the small number of cohorts for some procedure subgroups, and selective reporting of ocular outcomes. Nyctalopia, ocular surface disease, atypical outer retinal findings, or unexplained visual decline in a post-bariatric patient warrant prompt serum retinol testing and coordinated nutritional management regardless of the interval since surgery.

- CONCLUSIONS: The incidence and prevalence of pediatric uveitis increased over the past decade. Anterior uveitis predominated, whereas ocular morbidity and treatment intensity concentrated in intermediate uveitis and vision loss in posterior uveitis. Juvenile idiopathic arthritis and tubulointerstitial nephritis and uveitis were frequent associations. Prospective studies should determine whether rising rates reflect a true increase in disease or improved detection.

Delayed referral and limited use of acute-phase therapies may contribute to poor outcomes, emphasizing the need for national guidelines for early referral, standardized care, and coordinated strategies to prevent avoidable vision loss.
