- Impact of HIV on Perioperative Organ Injury and Surgical Outcomes in 41 Million Surgical Patients: A Retrospective Cohort Study. [Journal Article]Anesth Analg. 2026 Oct 14. [Online ahead of print]A&A
- CONCLUSIONS: People living with HIV incur a significantly higher risk of perioperative organ injury and worse surgical outcomes despite advances in antiretroviral therapy. HIV status was independently associated with adverse perioperative outcomes in adjusted models, supporting its inclusion as a risk stratification variable in surgical planning. Future research should incorporate immunologic markers, such as CD4 count and viral load, to further refine perioperative risk prediction and optimize outcomes for this population.

- Effect of Sevoflurane, Propofol, and Remifentanil on Hemodynamic Response to Pneumoperitoneum and Laparoscopic Surgery: A Randomized Controlled Trial. [Journal Article]Anesth Analg. 2026 Oct 14. [Online ahead of print]A&A
- CONCLUSIONS: The relative potency of sevoflurane to propofol in reducing the MAP response to pneumoperitoneum is similar to that for tolerance of laryngoscopy but differs from that for hypnotic endpoints. The effect of remifentanil on reducing the MAP response is similar whether it is combined with sevoflurane or propofol. Because titration to a dynamic clinical end point decouples the traditional relationship between drug input and clinical outcome, titration data cannot be used to validate pharmacodynamic models.

- Characterization of the Bispectral Index in Healthy Volunteers Receiving Inhalation Anesthetics in Combination With Opioids in a Randomized Trial. [Journal Article]Anesth Analg. 2026 Oct 14. [Online ahead of print]A&A
- CONCLUSIONS: These results describe the relationship between BIS levels and responsiveness for commonly used volatile anesthetics, with and without opioids, across a range of anesthetic concentrations. The results of this study should be interpreted in light of the study design which included young, healthy adult volunteers with no sustained, noxious stimulation. These findings may not be generalizable to other populations or clinical situations.

- CONCLUSIONS: Both groups improved similarly in pain and function, though new analgesic usage was more prevalent in the sham group. Neuroimaging revealed differential changes in cerebellar-sensory-visual networks and baseline patterns distinguishing responders from nonresponders, supporting feasibility and design of larger, adequately powered CES trials in fibromyalgia.

- External Validation of the Risk Assessment of Morbidity in Pediatric Surgery Score. [Journal Article]Anesth Analg. 2026 Oct 12. [Online ahead of print]A&A
- CONCLUSIONS: The RAMPS score performed well in this external validation with overall less patient complexity and less morbidity than in the original cohort included for development and internal validation. The score moderately overestimated risk in the most complex patients, which does not diminish its utility in guiding resource allocation to manage or prevent perioperative adverse events. In summary, the RAMPs score is a simple, highly discriminant, well-calibrated, and externally validated tool clinicians and health systems can use to predict morbidity among pediatric surgery patients.

- Determining Cerebrovascular Sensitivity to pH Using Continuous Arterial Blood Gas Analysis-A Prospective Observational Study. [Journal Article]Anesth Analg. 2026 Oct 12. [Online ahead of print]A&A
- CONCLUSIONS: This exploratory study provides a quantitative estimate of the relationship between CBFV and pH, and suggests that changes in CBFV are more closely related to arterial pH than to CO2. This estimate of the cerebrovascular sensitivity to pH was used to describe a feasible pH-guided approach in the management of CBF during the treatment of partially compensated acid-base disorders.

- CONCLUSIONS: TeaM-L&DSim is a scalable, interdisciplinary simulation program that improved team-based clinical competencies in L&D units, better effective in Level 3A facilities. More data are needed to understand adaptation to lower resource settings. This model holds promise for broader dissemination in global maternal safety initiatives.

- Regional anesthesia provides significant perioperative benefits, including improved pain control and reduced opioid consumption. Clinical trial evidence underpins anesthetic practice, yet inadequate demographic representation and reporting may limit the ability to likely evaluate findings and perpetuate inequities. This systematic review assessed the reporting of race, ethnicity, and sex in rando…
