- CONCLUSIONS: The rCPT identified delayed recovery of vigilance following ketamine and dexmedetomidine. Dexmedetomidine showed prolonged impairment across behavioral measures, including visual processing speed (hit latency) and reward retrieval, suggesting sustained effects on psychomotor function, motor coordination, and behavioral engagement that persisted for at least 4 hours after RORR. These findings support the utility of the rCPT as a translational paradigm for characterizing agent-specific and domain-specific trajectories of cognitive recovery following anesthesia.

Filling a new ERO prescription was associated with small but consistent increases in adverse clinical outcomes balanced against decreased 1 year health system costs. Combined with concerns regarding opioid-related harm and the opioid epidemic, these findings reinforce a cautious approach in postoperative ERO prescribing.

- Remimazolam versus sevoflurane for maintenance of anesthesia and postoperative nausea and vomiting in laparoscopic gynecologic oncology surgery: a randomized controlled trial. [Randomized Controlled Trial]
- CONCLUSIONS: Remimazolam-based anesthesia was associated with a lower incidence of PONV compared with sevoflurane in patients undergoing laparoscopic gynecologic oncology surgery. Although statistically significant, the observed reduction was slightly smaller than the effect size predefined during sample size estimation, and its clinical relevance requires further evaluation.

- CONCLUSIONS: Protocolized intraoperative arterial pressure-targeting strategies did not demonstrate a consistent incremental benefit over contemporary usual care in major noncardiac surgery. Mortality and MINS remained compatible with no treatment effect. Although AKI showed the most favorable estimates, the evidence remained uncertain, was sensitive to individual-trial exclusion, and should be interpreted cautiously.

- CONCLUSIONS: Patients receiving lower intraoperative DO2i during cardiopulmonary bypass had a higher risk of postoperative mortality, AKI, and prolonged recovery. These findings support the clinical relevance of oxygen delivery during bypass and provide a rationale for future mechanistic and interventional studies.
