- Clinical Impact of Physiology-Guided PCI: 1-year Results of the Post-Revascularization Optimization and Physiological Evaluation of Intermediate Lesions using Fractional Flow Reserve (PROPHET-FFR) Study. [Journal Article]Circ Cardiovasc Interv. 2026 Oct 15. [Online ahead of print]CC
- Background: The clinical value of physiological assessment after percutaneous coronary intervention (PCI) remains uncertain. We evaluated clinical outcomes associated with post-PCI physiology and temporal trends in its adoption. Methods: PROPHET-FFR (NCT05056662) is an ambispective observational study of patients undergoing invasive physiological assessment of intermediate coronary stenoses. Pati…

- Classification and Outcomes of Coronary Microvascular Disease in Myocardial Infarction and Angina With Nonobstructive Coronary Arteries. [Journal Article]Circ Cardiovasc Interv. 2026 Oct 15; :e016552. [Online ahead of print]CC
- CONCLUSIONS: CMD is common in both MINOCA and ANOCA. The higher prevalence of endothelium-independent CMD in MINOCA suggests a potential mechanistic role in infarction. Patients with MINOCA have a worse prognosis than ANOCA, particularly when CMD is present.

- Use of Myocardial Perfusion Imaging to Guide Prognosis and Management in Older Adults: Is There an Age Limit? [Journal Article]Circ Cardiovasc Imaging. 2026 Oct 14; :e019932. [Online ahead of print]CC
- CONCLUSIONS: Ischemia remains prognostic across all ages and may help identify candidates who have improved outcomes with ER. In an exploratory analysis, this association appeared attenuated in patients aged ≥85 years, a hypothesis-generating observation. Positron emission tomography-derived myocardial flow reserve may provide complementary physiological information for treatment selection in older adults.

- Multidisciplinary Approach to Antiplatelet Management in Coronary Syndromes: A Scientific Statement From the American Heart Association. [Review]Circulation. 2026 Oct 13. [Online ahead of print]Circ
- Antiplatelet therapy is a cornerstone of care for patients with acute coronary syndrome and chronic coronary disease, yet its optimal use requires careful balancing of thrombotic risk reduction with bleeding complications. Since the mid-2000s, advances in stent technology, antiplatelet pharmacotherapy, procedural technique, and risk stratification tools have led to increasingly individualized app…

- Dual Control of Low-Density Lipoprotein Cholesterol Levels by ANGPTL3 and ANGPTL8. [Journal Article]Circulation. 2026 Oct 13. [Online ahead of print]Circ
- CONCLUSIONS: A8 acts in concert with A3 to differentially modulate lipoprotein lipase- and endothelial lipase-mediated lipolysis, which antagonizes hepatic clearance of newly secreted atherogenic ApoB-containing lipoproteins. This mechanistic framework refines our understanding of A3-targeted lipid lowering and highlights the therapeutic potential of dual A3- plus A8-directed strategies to treat dyslipidemia and prevent atherosclerotic cardiovascular disease.

- Bleeding With Asundexian Versus Placebo in Patients With Noncardioembolic Ischemic Stroke or High-Risk TIA: OCEANIC-STROKE Randomized Trial. [Journal Article]Circulation. 2026 Oct 13. [Online ahead of print]Circ
- CONCLUSIONS: Asundexian added to antiplatelet therapy reduced ischemic stroke without significantly increasing major, major or CRNM, or any bleeding. Lower gastrointestinal bleeding was more frequent with asundexian, whereas treatment effects on major and major or CRNM bleeding were consistent across prespecified subgroups.

- Impact of [18]FDG-Avidity and Immunosuppression on Idiopathic and Genetic Cardiomyopathies. [Journal Article]
- CONCLUSIONS: Myocardial-only [18]FDG-avidity is not significantly associated with HF or arrhythmia risk in patients who do not clearly have cardiac sarcoidosis (including those with genetic cardiomyopathy). Immunosuppression therapy does not seem to modify the disease course, suggesting that not all myocardial [18]FDG uptake reflects clinically significant inflammation.

- Definition and Prognosis of Mitral Regurgitation in Transthyretin Amyloid Cardiomyopathy: A Multicenter International Cohort Study. [Journal Article]Circulation. 2026 Oct 13. [Online ahead of print]Circ
- CONCLUSIONS: Quantitative MR thresholds in transthyretin amyloid cardiomyopathy are substantially lower than current guideline cutoffs. A validated, risk-based composite MR grading scheme improves prediction of both mortality and HFH, better reflecting the restrictive low-flow pathophysiology of transthyretin amyloid cardiomyopathy and supporting disease-specific MR grading.
