- Language Impairment in Alzheimer's Disease: An Updated Narrative Review of Digital Assessment, Social Interventions, and Medical Treatment, 2016-2026. [Review]Clin Interv Aging. 2026; 21:633855.CI
- Language impairment is a core clinical manifestation of Alzheimer's disease (AD) that profoundly affects autonomy, decision-making, and social participation. Building upon the 2015 review by Kuca et al., this narrative review synthesizes empirical evidence from 2016 through 2026. A structured Web of Science Core Collection search retrieved 154 records. Following screening, 66 records were exclude…

- Rapidly Progressive Alzheimer's Disease: Candidate Mechanisms and Diagnostic Challenges. [Review]
- Alzheimer's disease (AD) usually follows a slowly progressive course, but some patients experience unusually rapid cognitive and functional decline. This clinical phenotype, commonly termed rapidly progressive Alzheimer's disease (rpAD), has no consensus definition; its status as a distinct biological entity remains unresolved, and its reported frequency depends strongly on operational criteria a…

- Clinical Characteristics and Comprehensive Management of Multimorbidity in Older Adults with Diabetes: A Narrative Review. [Review]
- With the acceleration of population aging and the progressive decline in physiological function, older adults with diabetes frequently present with multiple chronic conditions. This not only increases the complexity of clinical management but also adversely affects disease prognosis, quality of life, and the efficient allocation of healthcare resources. Traditional single-disease-centered managem…

- Incremental Predictive Value of Preoperative Systemic Immune-Inflammation Index and Prognostic Nutritional Index for Postoperative Delirium in Older Patients with Intertrochanteric Fractures. [Journal Article]
- CONCLUSIONS: Higher SII and lower PNI were associated with POD in older patients undergoing PFNA for intertrochanteric fractures. Although adding these biomarkers produced statistically significant improvements in discrimination, the absolute improvements were modest, and the combined model retained only moderate discriminative performance. The model should therefore be considered exploratory and requires external validation before clinical implementation.

- Neuromuscular Remodeling in Knee Osteoarthritis Across Aging: An Evidence-Bounded Joint-Afferent-Motor Unit Framework. [Review]
- Quadriceps weakness is a major determinant of disability in knee osteoarthritis (KOA), particularly in later life, yet weakness often exceeds the loss of muscle size. This discrepancy is clinically important because joint-derived activation failure occurs against a background of declining neuromuscular and regenerative reserve. We critically examine an evidence-bounded joint-afferent-motor unit f…

- Life's Essential 8 and Life's Crucial 9 as Prognostic Indicators for Hip Fracture Outcomes: A Multicenter Prospective Cohort Study. [Multicenter Study]
- CONCLUSIONS: In this multicenter prospective cohort study, LC9 was independently associated with mortality and functional recovery and demonstrated greater discriminative performance than LE8, with psychological health assessment providing critical incremental value. Integration of LC9 into clinical practice may enhance risk stratification and inform targeted perioperative interventions for this vulnerable population.

- Analysis of Risk Factors and Development of a Predictive Model for 1-Year Mortality After Acute Hip Fracture Surgery in Patients Aged ≥80 Years: The Prognostic Nutritional Index as a Potential Predictor. [Journal Article]
- CONCLUSIONS: This internally validated nomogram integrating PNI and other risk factors predicts 1-year postoperative mortality in patients aged ≥80 years with hip fracture and may support clinical risk-stratification decisions.

- The Effect of Mindfulness-Based Cognitive Therapy on Kinesiophobia in Older Adults with Chronic Knee Osteoarthritis: A Randomized Controlled Trial. [Randomized Controlled Trial]
- CONCLUSIONS: Four weeks of MBCT may influence the trajectory of kinesiophobia over time in older adults with knee osteoarthritis.

- HbA1c and LDL-C Time in Therapeutic Range: A Study on All-Cause Mortality in Older Adults with Type 2 Diabetes Mellitus and Chronic Kidney Disease. [Multicenter Study]
- CONCLUSIONS: In older adults with T2DM and CKD, higher LDL-TTR was independently associated with lower mortality, and a significant HbA1c-TTR × LDL-TTR interaction suggested that the inverse association of one TTR measure with mortality was attenuated at higher levels of the other. This interaction was absent in the younger cohort and under an alternative HbA1c target (6.5-8.0%), and was inconsistent across sensitivity analyses (significant with time-varying covariates, HR 1.013, P=0.019; non-significant in a landmark analysis, P=0.968). These exploratory findings do not support changing current management targets and require prospective validation.

- Preoperative Frailty and Risk of Post-ERCP Pancreatitis in Older Adults with Choledocholithiasis. [Journal Article]
- CONCLUSIONS: Preoperative frailty assessed using EHR-CFI was independently associated with PEP and may provide additional value for risk stratification in elderly patients with choledocholithiasis undergoing ERCP.

- Associations of Inflammatory Biomarkers with Disease Severity in Elderly Patients with Sepsis-Associated ARDS: A Cross-Sectional Study. [Journal Article]
- CONCLUSIONS: ESM-1, PCT, and HBP are independently associated with disease severity in elderly patients with sepsis-associated ARDS. ESM-1 demonstrated discriminative ability comparable to conventional severity scores and may serve as an adjunctive marker for severity assessment at enrollment, pending prospective validation. Limitations include the single-center, cross-sectional design and the lack of external validation.

- Supporting Social Engagement in Long-Term Care Through Paired Socially Assistive Robot-Virtual Reality Activities for Older Adults with Cognitive Impairment and Apathy. [Journal Article]
- Older adults living in long-term care settings, particularly those with cognitive impairment and apathy, frequently experience reduced social engagement. Socially assistive robots and virtual reality systems may help support engagement, but little is known about whether they can sustain engagement during real-world deployment while also facilitating interaction between older adults. This study ex…

- Clinical Characteristics and Predictors of Mortality in Elderly Patients with Bloodstream Infections: A 12-Year Retrospective Cohort Study. [Journal Article]
- CONCLUSIONS: Prolonged invasive medical interventions, including mechanical ventilation, central venous catheterization, and urinary catheterization, as well as chemotherapy or radiotherapy, were independently associated with higher mortality in elderly BSI patients. These findings highlight the need for risk-stratified management strategies to optimize outcomes in these elderly patients.

- Lactate-Informed Exercise Prescription in Mild Cognitive Impairment and Alzheimer's Disease: A Translational Framework for Preserving Cognition, Mobility, and Independence. [Review]
- CONCLUSIONS: The novelty of the framework is its integration of disease stage, geriatric phenotype, peripheral metabolic response, and recovery into clinically actionable dose decisions rather than a universal lactate threshold. Prospective multicenter trials should standardize sampling, validate response thresholds, and determine whether this approach improves cognition, mobility, independence, safety, and caregiver-relevant outcomes beyond usual advice or fixed-dose exercise.

- Screening for Geriatric Problems in Older Adults Receiving Hemodialysis Using a Self-Administered Battery. [Journal Article]
- CONCLUSIONS: The GOLD demonstrated agreement with geriatrician evaluation for many geriatric problems and was feasible to implement in dialysis clinics. These findings suggest potential utility for scalable geriatric screening in dialysis care, although further work is needed to confirm these findings in larger cohorts and to refine screening for cognitive impairment and other domains.
