- Fulminant non-autoimmune diabetes presenting as fatal diabetic ketoacidosis in a 2-month-old infant with genetically confirmed Wolcott-Rallison syndrome. [Journal Article]Diabet Med. 2026 Oct 27. [Online ahead of print]DM
- CONCLUSIONS: WRS can present with fulminant DKA and progress to catastrophic multiorgan collapse early in infancy. This case highlights the crucial need to consider monogenic non-autoimmune diabetes in early-infantile DKA and underscores the clinical challenge of distinguishing primary metabolic organ failure from secondary complications of severe DKA and shock. Early molecular diagnosis remains vital for family counselling.

- The transition from pregnancy to motherhood in women with type 2 diabetes: A longitudinal qualitative study. [Journal Article]Diabet Med. 2026 Oct 27. [Online ahead of print]DM
- CONCLUSIONS: In this study, the transition to motherhood among women with type 2 diabetes was experienced as a dynamic and phase-dependent process, in which needs and challenges varied before, during and after pregnancy. The findings underline the importance of preparing women for diabetes-related changes in self-care and treatment both pregestationally and antenatally, and of providing access to structured and continuous postpartum care.

- Examining the facilitators and barriers to health behaviour change for type 2 diabetes remission in a young population with early diabetes: A qualitative study of participants in the DIADEM-I randomised controlled trial. [Journal Article]Diabet Med. 2026 Oct 27. [Online ahead of print]DM
- CONCLUSIONS: The findings highlight the complexity of health behaviour change initiation and maintenance in the context of T2DM diagnosis and aiming for T2DM remission. The study provides insights for clinicians aiming to motivate individuals toward sustained health behaviour change for T2DM remission and implement service delivery.

- National clinical audits as distributed diabetes surveillance infrastructure: Consensus lessons from Ireland. [Review]Diabet Med. 2026 Oct 27. [Online ahead of print]DM
- CONCLUSIONS: National clinical audit infrastructures may function as distributed chronic disease intelligence systems even in the absence of mature integrated diabetes registries.

- Early sodium-glucose co-transporter 2 inhibitor initiation is associated with improved cardiovascular outcomes in type 2 diabetes and acute coronary syndrome. [Journal Article]Diabet Med. 2026 Oct 27. [Online ahead of print]DM
- CONCLUSIONS: Sodium-glucose co-transporter 2 inhibitor therapy at hospital discharge was associated with a significantly lower risk of major adverse cardiovascular events in patients with type 2 diabetes and acute coronary syndrome.

- The art of public engagement in type 1 diabetes research. [Review]Diabet Med. 2026 Oct 27. [Online ahead of print]DM
- CONCLUSIONS: These examples illustrate how artistic approaches can make basic science more accessible and create opportunities for dialogue between researchers and people affected by T1D.

- Characterising differential maturation of stem cell-derived islets in mice: Graft function and transcriptomic analysis. [Journal Article]Diabet Med. 2026 Oct 25. [Online ahead of print]DM
- CONCLUSIONS: Longitudinal plasma C-peptide and glucose tolerance can be used to characterise in vivo SC-islet maturation, which is associated with transcriptomic changes not observed in vitro. Additionally, elevated blood glucose does not impair long-term functional maturation. These findings support the importance of in vivo cues for SC-islet maturation to inform future preclinical studies.

- The national implementation of 'Healthy Living', a web-based type 2 diabetes self-management education and support service: Narrative synthesis of evaluation findings and implications for service delivery and research. [Review]Diabet Med. 2026 Oct 25. [Online ahead of print]DM
- CONCLUSIONS: We present the first logic model of a UK diabetes self-management and education programme. The implications for UK diabetes care include a need for clarity on eligibility for different services, the importance of expanding access to people from minority ethnicities and areas of highest deprivation, and the need to improve retention. A future research agenda includes achieving consensus on a roadmap through diabetes services, how and why people choose (or not) to participate in self-management services, and which components of these services are most strongly associated with positive health benefits.

- Mesenchymal stromal cell-derived extracellular vesicles improve islet β-cell metabolic function via miRNA-mediated regulation of Ankyrin-B expression. [Journal Article]Diabet Med. 2026 Oct 25. [Online ahead of print]DM
- CONCLUSIONS: These findings identify a novel miRNA-mediated mechanism underlying the beneficial effects of MSC-EVs on islet function and support the development of MSC-EV-based cell-free strategies to enhance islet function for diabetes therapies.

- Real-world observational study evaluating metabolic and kidney outcomes in patients living with chronic kidney disease and type 2 diabetes mellitus with obesity prescribed tirzepatide. [Journal Article]Diabet Med. 2026 Oct 19; :e70500. [Online ahead of print]DM
- CONCLUSIONS: In this real-world, multi-ethnic cohort, including patients with advanced CKD, tirzepatide was associated with clinically meaningful improvements in weight, glycaemic control, albuminuria, lipid profile and insulin requirements without adversely affecting kidney function. The introduction of tirzepatide to patients with CKD should be guided by a multidisciplinary team.

- Predictive performance of the Saint Elian Wound Score System adjusted for age for major amputation in diabetic foot: A competing risks analysis. [Journal Article]Diabet Med. 2026 Oct 19; :e70503. [Online ahead of print]DM
- CONCLUSIONS: The SEWSS, when adjusted for age and analysed using a competing risks framework, shows good prognostic performance for predicting MA in patients with DF. The model may support risk stratification and clinical decision-making, although external validation in independent populations is required.

- Islet-exocrine crosstalk in diabetes: Distinguishing whole-pancreas pathology from causal communication. [Review]Diabet Med. 2026 Oct 19; :e70505. [Online ahead of print]DM
- CONCLUSIONS: Diabetes should be studied as a multicellular pancreatic disorder without interpreting associated pathology as evidence of causation. Longitudinal human studies and multicellular models that allow selective perturbation are required to identify clinically actionable interactions.

- Type 1 diabetes results from an autoimmune-mediated destruction of pancreatic β-cells, yet considerable heterogeneity exists regarding disease onset, progression and severity. Emerging evidence suggests that this variability reflects not only differences in immune responses, but also differences in the architecture and immune landscape of the endocrine pancreas. - What do administrative diabetes indicators measure? Treatment-side and screening-side evidence from nationwide NDB Open Data in Japan. [Journal Article]Diabet Med. 2026 Oct 19; :e70501. [Online ahead of print]DM
- CONCLUSIONS: Neither indicator family provided a direct estimate of disease burden. Prescription quantities reflected demographic and income structure, whereas HbA1c indicators showed a selective ecological association with downstream service use. Both are better interpreted as ecological surveillance indicators than direct measures of disease burden or prevalence.
