- Barriers and Opportunities for Implementing Precision Oncology in Geriatric Cancer Care. [Review]
- Precision oncology can guide treatment selection and treatment omission in older adults, but molecular actionability alone does not establish an acceptable individual benefit-burden balance. This focused narrative review examines tumor biology, host vulnerability, and implementation barriers, prioritizing randomized trials, prospective cohorts, and guidance published during 2019-2025. Clonal hema…

- Comparative Impact of Anticholinergic Exposure, Potentially Inappropriate Medications, and Polypharmacy on Clinical Outcomes: A Nationwide Claims Database Study in Japan. [Journal Article]
- CONCLUSIONS: These findings suggest that comprehensive exposure assessments that account for disease characteristics and temporal factors are crucial for appropriately evaluating medication-related risks.

- Planned Antihypertensive Deprescribing in Long-Term Care Residents: A Systematic Review of Randomized Evidence. [Systematic Review]
- CONCLUSIONS: Randomized evidence for antihypertensive deprescribing in long-term care remains sparse and clinically heterogeneous. Within individual trials, supervised deprescribing reduced antihypertensive exposure and raised blood pressure, but the magnitude and clinical consequences varied by strategy and follow-up. Current evidence does not establish a common effect on mortality, hospitalization, or other resident-important outcomes, making trial-specific estimates and deprescribing protocols more informative than a single pooled treatment effect.

- Opioid Use in Older Adults with Cancer Pain Admitted to an Acute Palliative Care Unit: Does Age Matter? A Secondary Analysis. [Journal Article]
- CONCLUSIONS: Age was associated with clinically significant differences in opioid prescribing within the acute palliative care unit. Compared with patients aged ≥65 years, adults aged <65 years received higher opioid doses and more complex opioid regimens, whereas prescribing patterns were similar in the two geriatric age groups. Despite age-related differences in opioid prescribing, comparable pain control was achieved across all age groups (see Table 1 and Results), supporting individualized age-tailored opioid titration in specialist palliative care.

- Benzodiazepine Prescription and Use in the Older Adults in Catalonia, Spain from 2018 to 2022: A Drug Utilization Study. [Journal Article]
- CONCLUSIONS: Benzodiazepine prescribing among Catalan older adults remains high, dominated by rapid-onset and short-acting to intermediate-acting agents, with widespread long-term use. These findings highlight the need for structured medication reviews, deprescribing programs, and prescriber education to reduce inappropriate BZD exposure in this population.

- Intravenous Iron Therapy in Very Old Adults with Heart Failure: A Frailty-Oriented Therapeutic Perspective. [Review]
- Iron deficiency is common in heart failure (HF) and particularly frequent in older adults, in whom it may contribute to fatigue, exercise intolerance, impaired quality of life and functional decline, even without overt anaemia. In HF, intravenous iron has shown consistent benefits on symptoms, functional capacity and health-related quality of life in selected populations, mainly with reduced ejec…

- Long-Term Proton Pump Inhibitor Therapy Is Common Among Older Adults: An 8-Year Finnish Nationwide Register Study. [Journal Article]
- CONCLUSIONS: Long-term PPI use is highly prevalent among older adults in Finland. These findings highlight the need for regular medication reviews and targeted deprescribing efforts to improve pharmacotherapy quality and safety in aging populations.

- Patterns of Evidence-Based Medication Use in Older Adults with Chronic Heart Failure in Slovakia: A Population-Based Study Using the National Health Information Center Database. [Journal Article]Drugs Aging. 2026 Sep 30. [Online ahead of print]DA
- CONCLUSIONS: Among older adults with HF, the prescription of evidence-based medications was strongly associated with physician specialty, reflecting the importance of specialised expertise in the management of HF.

- Medication Appropriateness and Potentially Inappropriate Medications in Older Critical Care Patients: A Scoping Review. [Review]
- CONCLUSIONS: Medication appropriateness assessment tools have been used to a limited degree to assess medications in older critical care people. Further research is needed to establish their validity, clinical utility and impact on prescribing decisions and to evaluate interventions aimed at improving medication appropriateness and outcomes for older people in critical care.

- Risk of Dementia With Renin-Angiotensin System Inhibitors Versus Calcium Channel Blockers in Hypertensive Adults: A Nationwide Multicenter Study. [Journal Article]
- CONCLUSIONS: In adults with hypertension, initiation of RASIs was associated with a significantly lower risk of dementia compared with CCBs. No significant difference was observed between ACEIs and ARBs. Further validation is warranted.

- Differences in Anticholinergic Exposure Assessment Across Burden Scales and Exposure Measures in a Population-Based Study: Implications for Targeted Deprescribing in Older Adults. [Journal Article]Drugs Aging. 2026 Sep 15. [Online ahead of print]DA
- CONCLUSIONS: Scale or measure selection substantially influences who is classified as exposed, medication profiles and exposure patterns across subgroups. Anticholinergic burden scales and exposure measures are therefore not interchangeable and selection should reflect whether the intended purpose is population surveillance, comprehensive medication review or targeted deprescribing.

- Creating Awareness of preScribing Cascades and Managing Adverse Drug Effects (CASC-ADE) Tool: Appraisal by Future End Users. [Journal Article]
- CONCLUSIONS: The initial CASC-ADE tool was positively appraised across the assessed domains by the HCPs and students. Further research is needed to assess whether its use will improve recognition, management and prevention of prescribing cascades.

- Influence of Ageing on the Pharmacology, Efficacy and Safety of Oral Targeted Therapies for Inflammatory Bowel Disease: Focus on Janus Kinase (JAK) Inhibitors and Sphingosine-1-Phosphate (S1P) Receptor Modulators. [Review]
- Older adults represent a rapidly expanding subgroup of patients with inflammatory bowel disease, yet they remain markedly under-represented in pivotal clinical trials, limiting age-specific estimates of drug benefit and harm. This review synthesises the available evidence on the influence of ageing on the pharmacology, efficacy and safety of orally administered targeted inflammatory bowel disease…
