  • Responding to Miyatani et al., this commentary, focused on the United States. We contend that handheld intestinal ultrasound reframes the point-of-care debate regarding timing and cost, not diagnostic superiority. For many pediatric inflammatory bowel disease programs, bridging with radiology while building training and governance remains the wisest near-term approach.

  • CONCLUSIONS: Acute and chronic pouchitis are common complications following pediatric RPC-IPAA. While pouch failure is rare, a significant percentage will require postoperative advanced IBD therapy. Families should be counseled that proctocolectomy can achieve disease control but does not eliminate the need for IBD-directed medical therapy.

  • CONCLUSIONS: These observations support an alternative view of CD in which intestinal lymphatic dysfunction plays an under-recognized role in both promoting and sustaining inflammation. This framework may explain disease features (patchy distribution, transmural inflammation, mesenteric hypertrophy) and positions the lymphatic system as a therapeutic target to restore drainage and modulate immune-lymphatic crosstalk.
