Tuberculosis (TB) disease exposure within maternity units is a significant concern due to increased risk of infection among pregnant and post-partum women and their newborns. During April-August 2025, personnel from several military health authorities were deployed to assist a military treatment facility in Alaska after a labor and delivery nurse was diagnosed with pulmonary TB disease. A total of 282 individuals were identified through contact tracing. Staff contacted the affected families, answered questions, and facilitated evaluation and treatment. 263 (92.9%) of the 283 contacts were tested. Window prophylaxis was recommended for 26 children under five years of age. Of these, 21 (80.8%) completed the full therapeutic course. Four families declined the intervention, and one child was lost to follow-up. No additional cases of TB disease were detected; however, four cases of latent TB infection were diagnosed. Standardized guidance regarding TB screening and treatment procedures including routine risk assessments, screening for healthcare workers, and window prophylaxis timelines for infants and children is imperative for prevention.

Crusted scabies is a rare parasitic hyperinfestation, commonly associated with impaired host immunity. Dysregulation of T cell homeostasis has been reported to drive disease progression. We characterized a profoundly imbalanced T cell phenotype in an immunocompetent male with crusted scabies, potentially induced by prior tralokinumab and corticosteroid treatment for presumed atopic dermatitis.
