- Prognosis and predictors of fatigue after stroke. An individual patient data meta-analysis from three large multicentre trials of fluoxetine for stroke recovery. [Journal Article]Int J Stroke. 2026 Oct 02; :17474930261496637. [Online ahead of print]IJ
- BackgroundFatigue affects about half of stroke survivors, but its natural history and associations are uncertain. We aimed to determine the trajectory of fatigue between 6 and 12 months after stroke, and factors associated with changes in fatigue between 6 and 12 months after stroke, in patients recruited to three large clinical trials.MethodsWe performed three large randomised, placebo-controlle…

- International Modified Delphi Consensus on Assessment and Management of Covert Brain Infarction. [Journal Article]Int J Stroke. 2026 Oct 02; :17474930261496691. [Online ahead of print]IJ
- BackgroundCovert brain infarction (CBI) is a frequent incidental neuroradiological finding increasingly recognized as clinically relevant due to its association with stroke, cognitive decline, and other adverse outcomes. However, substantial uncertainty remains regarding its optimal assessment and management, and existing guidelines provide limited and heterogeneous recommendations. We aimed to d…

- Increased Risk of Cerebrovascular Events in Pseudoxanthoma Elasticum. [Journal Article]Int J Stroke. 2026 Sep 28; :17474930261495889. [Online ahead of print]IJ
- CONCLUSIONS: Patients with PXE have a 4.8-fold higher risk of cerebrovascular events compared to the general population. Various ischemic cerebrovascular event subtypes occur in patients with PXE, with SVD being the most common subtype.

- A roadmap for somatosensory rehabilitation and research after stroke: Consensus-based core recommendations from the fourth Stroke Recovery and Rehabilitation Roundtable. [Review]Int J Stroke. 2026 Sep 22; :17474930261494430. [Online ahead of print]IJ
- CONCLUSIONS: A framework for understanding somatosensation, and consensus-based recommendations for somatosensory interventions and for research priorities are provided to advance the science and practice of evidence-based rehabilitation of somatosensation after stroke.

- Cardiovascular risk management through a community-based digital health stroke education platform (Love Your Brain): a multi-arm randomized controlled trial. [Journal Article]Int J Stroke. 2026 Sep 22; :17474930261494297. [Online ahead of print]IJ
- CONCLUSIONS: The text message intervention increased medical practitioner visits for cardiovascular risk assessment/management among community-dwelling adults without prior cardiovascular events. The online course was ineffectual, potentially due to low completion. The text message intervention warrants scale-up to support primary stroke prevention.

- STRIVE-2 MRI-Defined Brain Frailty and Functional Outcomes After Thrombectomy: A Multicenter Study. [Journal Article]Int J Stroke. 2026 Sep 11; :17474930261490798. [Online ahead of print]IJ
- CONCLUSIONS: Preexisting lacunes, atrophy, and WMH are independent determinants of worse functional recovery after MT. Importantly, cumulative total CSVD burden acts as a strong, dose-dependent predictor of functional outcomes and symptomatic hemorrhage. These findings suggest that while CSVD assessment improves post-thrombectomy prognostication, it should not be used as a criterion to preclude patients from MT eligibility.

- Adherence to anticoagulants in people with stroke and atrial fibrillation: Implementation of standardized analytic methods in Australia and Canada. [Journal Article]Int J Stroke. 2026 Sep 09; :17474930261489902. [Online ahead of print]IJ
- CONCLUSIONS: Implementation of standardized methods across two countries revealed high levels of adherence to direct oral anticoagulants in patients with ischemic stroke and atrial fibrillation. Scale-up of these methods to other countries and medications is warranted to reliably measure and compare trends in post-stroke medication adherence.

- Remote ischemic conditioning in the acute treatment of ischemic and hemorrhagic stroke: A systematic review and meta-analysis of randomized controlled trials. [Review]Int J Stroke. 2026 Sep 02; :17474930261487798. [Online ahead of print]IJ
- CONCLUSIONS: Remote ischemic conditioning appears to improve functional and neurological outcomes in acute ischemic stroke patients. However, the quality of evidence is limited by regional generalizability (i.e. most studies came from China), small sample sizes, and methodological heterogeneity. We encourage future trials to include patients presenting beyond 4.5 h and to incorporate bilateral remote ischemic conditioning application and to extend remote ischemic conditioning use beyond 7 days. We also encourage studies to focus on patients who are not eligible for reperfusion therapy.

- Clinical Outcomes among Patients with Active Cancer and Acute Ischemic Stroke Treated with Antiplatelet versus Anticoagulant Therapy. [Journal Article]Int J Stroke. 2026 Aug 30; :17474930261487275. [Online ahead of print]IJ
- CONCLUSIONS: In a registry spanning two decades, most patients with AC were discharged on antiplatelet therapy after AIS. Antiplatelet and anticoagulant groups showed similar long-term risks of ischemic cerebrovascular recurrence. Antiplatelet therapy was associated with reduced medium-term ischemic cerebrovascular recurrences and mortality, although between-group clinical differences may have contributed to these findings.Data access statement:Anonymized data are available from the corresponding author upon reasonable request and subject to a signed data transfer and use agreement.

- MRI-derived oxygen extraction fraction is associated with cognitive impairment in cerebral small vessel disease. [Journal Article]Int J Stroke. 2026 Aug 28; :17474930261485653. [Online ahead of print]IJ
- CONCLUSIONS: MRI-derived OEF was associated with cognitive impairment in CSVD independently of conventional structural MRI burden. These findings suggest that OEF may provide complementary physiological information for characterising cognitive vulnerability in CSVD. Larger multicentre longitudinal studies incorporating quantitative perfusion imaging and validated amyloid and tau biomarkers are warranted.

- The future of high-dose, high-intensity neurorehabilitation: Consensus-based core recommendations from the fourth Stroke Recovery and Rehabilitation Roundtable. [Journal Article]Int J Stroke. 2026 Aug 22; :17474930261483951. [Online ahead of print]IJ
- CONCLUSIONS: The primary limitation to HDHI is implementation capacity, not biological plausibility, or lack of evidence. Implementation research should now proceed in parallel with ongoing efficacy refinement. Priorities include building mechanistic and evidentiary literacy, clarifying professional roles, enabling home-based delivery, aligning reimbursement with long-term economic evaluation, and developing the infrastructure and protocols for scalable longitudinal care.

- Telestroke care in resource-limited settings: A World Stroke Organization scientific statement. [Review]Int J Stroke. 2026 Aug 22; :17474930261480603. [Online ahead of print]IJ
- CONCLUSIONS: This statement provides practical guidance to support the safe and feasible implementation of telestroke services across diverse resource settings. Future research should focus on defining minimum effective models, evaluating individual components of telestroke systems, and addressing economic, regulatory, and cross-border implementation challenges. This scientific statement has been reviewed and approved by the WSO Executive.

- The 2017 Stroke Recovery and Rehabilitation Roundtable (SRRR) consensus statement on biomarkers identified genetic and blood biomarkers as a developmental priority for stroke recovery and rehabilitation. While progress has been made, this nascent field still lags behind other medical disciplines. Further development is critical because molecular biomarkers hold great promise to provide key insigh…
