- Achieving opioid-free anaesthesia. [Review]Ir J Med Sci. 2026 Oct 29. [Online ahead of print]IJ
- CONCLUSIONS: Rigid opioid avoidance risks undertreating acute pain, so OFA is best applied selectively, guided by procedure type, patient comorbidities, and institutional resources. This review evaluates the rationale, implementation, and limitations of OFA, providing clinicians with a balanced appraisal of its role in modern perioperative care.

- A case of esophageal adenocarcinoma presenting as a submucosal tumor. [Review]Ir J Med Sci. 2026 Oct 28. [Online ahead of print]IJ
- CONCLUSIONS: Clinicians should keep in mind that esophageal submucosal tumors covered by normal epithelium are not invariably benign. Endoscopic surveillance for such lesions carries potential risks, and adequate communication with patients is therefore mandatory before management.

- Pumpkin seed and seed oil affect triglyceride levels in adults with hyperlipidemia: a randomized controlled trial. [Journal Article]Ir J Med Sci. 2026 Oct 28. [Online ahead of print]IJ
- CONCLUSIONS: TG decreased significantly within the pumpkin seed and pumpkin seed oil groups, but between-group differences were not significant. Further studies are needed to determine whether pumpkin seed products provide benefits beyond those of dietary intervention alone.

- Burden, management and outcomes of emergency haematuria admissions: a 9-month retrospective cohort study. [Journal Article]Ir J Med Sci. 2026 Oct 28. [Online ahead of print]IJ
- CONCLUSIONS: Emergency haematuria admissions represent a high-acuity workload predominantly affecting elderly, antithrombotic-treated patients. Malignancy predicts readmission and radiation cystitis drives transfusion burden. These findings support risk-stratified admission pathways incorporating early malignancy triage and haemoglobin-guided decision-making.

- Early selective deep capillary plexus impairment at diagnosis in children with type 1 diabetes: an OCT angiography study. [Journal Article]
- CONCLUSIONS: DCP impairment is detectable at T1DM diagnosis before clinically visible diabetic retinopathy. OCTA may provide a sensitive, noninvasive biomarker of early retinal microvascular involvement in pediatric T1DM.

- Intracuff alkalinized lidocaine attenuates airway irritation and hemodynamic responses during extubation: a retrospective observational study. [Journal Article]
- CONCLUSIONS: Intracuff alkalinized lidocaine was associated with reduced airway irritation and improved periextubation hemodynamic stability compared with intravenous lidocaine, suggesting it may be a simple and clinically effective strategy to improve extubation quality.

- The medical and surgery outcomes of pulmonary hydatid cyst disease: A 12-year multidisciplinary experience. [Journal Article]
- CONCLUSIONS: Positive IFA results, multiple pulmonary cysts, and bilateral lung involvement are associated with concomitant hepatic hydatid disease. Cyst size and perforation status significantly influence postoperative recovery, particularly hospital stay and drainage duration. Surgical treatment provides excellent outcomes with low reoperation rates and no disease-related mortality in experienced centers.

- Performance psychology workshops for surgical trainees: A prospective pilot study on anxiety and mental toughness. [Journal Article]
- CONCLUSIONS: This brief high-performance psychology intervention was feasible and acceptable and was associated with short-term reductions in performance anxiety and sustained improvements in mental toughness in surgical trainees. These findings support the feasibility of integrating time-efficient psychological training in surgical education. Larger, controlled studies are required to confirm efficacy and mechanisms.

- Diagnostic yield of reflex multiplex PCR after a negative rapid antigen test in children hospitalized with presumed viral acute respiratory tract infection: a prospective cohort study. [Journal Article]
- CONCLUSIONS: A negative antigen test did not exclude a respiratory virus; reflex PCR improved detection, varying by virus and age. With selective testing, the yield is a lower bound. Antibiotic exposure was high regardless of viral detection; its appropriateness could not be judged without systematic bacterial testing.

- Making vascular outcomes visible: an implementation framework for an Irish national vascular registry. [Review]
- CONCLUSIONS: Ireland does not require further proof that national vascular audit is feasible; it requires a deliberately limited, testable, and sustainably commissioned implementation model.

- Combatting gambling in Ireland: adopting demand and supply strategies from the fight against big tobacco and alcohol. [Letter]
- This paper outlines the threat posed by gambling in Ireland and proposes a range of policy responses. Drawing on established tobacco and alcohol control measures, it argues for restrictions on advertising and point-of-sale (PoS) marketing, mandatory warnings, plain packaging, improved enforcement, raising the minimum gambling age to 21, increasing minimum spend requirements, introducing temporal …

- Molecular identification and genotype distribution of Acanthamoeba in environmental water. [Journal Article]
- CONCLUSIONS: The high detection rate, predominance of the clinically relevant T4 genotype, and identification of potentially virulent strains highlight a substantial environmental reservoir with epidemiological significance. These findings underscore the need for routine molecular surveillance of environmental waters within a One Health framework to mitigate the risk of severe infections such as Acanthamoeba keratitis and granulomatous amoebic encephalitis.

- Computer tomography imaging for suspected acute aortic syndrome. [Journal Article]
- CONCLUSIONS: This study highlights the low positivity rate of CTA aorta scans in an Irish ED but high mortality of patients diagnosed with acute aortic syndrome. Increasing CTA utilisation, alongside the overall low incidence of AAS, suggests opportunities to improve diagnostic efficiency.

- Prostatic artery embolization for symptomatic benign prostatic hyperplasia: an Irish experience. [Journal Article]
- CONCLUSIONS: Our experience demonstrates PAE as a feasible and promising treatment option for BPH-related symptoms and catheter-dependence in the Irish setting. Notably, the strength of conclusions is limited by the small, single-centre, retrospective nature of this study and future research is required to establish safety and efficacy.
