- CONCLUSIONS: In patients with failing Fontan circulation and junctional rhythm, restoring AV synchrony by atrial pacing may reverse Fontan hypertension and improve end-organ congestion.Junctional rhythm should be considered a reversible contributor to elevated Fontan pressures before attributing failure to anatomy. Atrial pacing, when AV conduction is intact, can improve Fontan hemodynamics and hepatic congestion while avoiding ventricular pacing.

- CONCLUSIONS: Nonrheumatic GAS-associated perimyocarditis is a rare inflammatory cardiac complication of streptococcal pharyngitis that may present with chest pain, myocardial injury, and left ventricular dysfunction.GAS-associated perimyocarditis should be considered in patients presenting with acute chest pain and left ventricular dysfunction shortly after streptococcal pharyngitis. Recognition of this syndrome is important to guide appropriate diagnostic evaluation, including exclusion of obstructive coronary disease and utilization of multimodality imaging.

Emergent angiography plays a critical role in distinguishing KS subtypes.KS should be suspected in any perioperative patient developing ST elevation after drug administration; emergent angiography is essential to differentiate vasospasm from obstructive disease and guide management.

- CONCLUSIONS: Native T1 can provide pragmatic diagnostic supportive thresholds, whereas ECV-calibrated native T1 strata reproduced a graded 3-year mortality risk. Native T1 offers a practical contrast-free approach to support diagnosis and risk stratification, although ECV remains the preferred quantitative marker when contrast CMR is available. Thresholds and z-score based approaches still require local recalibration and external validation before adoption at other centers.

PD-1 expression is blunted in infiltrating inflammatory cells in LAM compared with ACR, suggesting a disruption of the PD-L1/PD-1 axis in LAM.
