  • CONCLUSIONS: Patients with HFpEF and lower-than-expected RMR have greater prevalence of obesity, and those with higher-than-expected RMR display circulatory adaptations to heightened metabolic demand at rest, which are coupled with blunted reserve capacity with exercise, and increased risk for adverse clinical outcomes. Further studies are warranted to elucidate the underlying mechanisms by which RMR contributes to the pathophysiology of HFpEF and to clarify its potential utility in therapeutic strategies.
  • Molecular Heterogeneity and Prognostic Implications of Low-Grade Acute Cellular Rejection (1R/1A) in Heart Transplantation. [Journal Article]

  • CONCLUSIONS: Histologic 1R/1A represents a biologically heterogeneous state rather than a uniformly low-risk finding. Compared with 0R biopsies, 1R/1A was associated with higher dd-cfDNA levels, higher GEP scores, and greater transcriptomic alloimmune activation. Although histologic 1R/1A was not associated with graft dysfunction, retransplantation, or death, it was associated with an increased risk of subsequent rejection and dd-cfDNA elevation. Importantly, MMDx identified a biologically active subset of 1R/1A biopsies at increased risk of future rejection and provided incremental prognostic information beyond conventional histology. These findings support the integration of molecular diagnostics into post-transplant risk stratification and surveillance.

  • CONCLUSIONS: Deep learning analysis of echocardiograms accurately discriminated future HF risk, with favorable performance over current clinical HF scores. Future work should assess whether broader use of artificial intelligence-enabled echocardiographic risk stratification may improve HF prevention and clinical outcomes, including among individuals who do not have a clinical indication for echocardiography.
