- Impact of dementia and its progression on mortality in heart failure with preserved ejection fraction -insight from the KUNIUMI registry chronic cohort. [Journal Article]J Cardiol. 2026 Oct 07. [Online ahead of print]JC
- CONCLUSIONS: Dementia, especially its progression, was associated with worse prognosis in HFpEF. These findings highlight the importance of longitudinal cognitive assessment to improve risk stratification and guide management strategies.

- Nationwide Survey of Prehospital 12‑lead Electrocardiogram Recording and Transmission in Japan: Equipment Availability Is No Longer the Major Barrier. [Journal Article]J Cardiol. 2026 Oct 07. [Online ahead of print]JC
- CONCLUSIONS: Prehospital 12‑lead ECG recording equipment is widely available in Japan, but 12‑lead ECG transmission systems remain insufficiently implemented. The principal challenge is no longer equipment availability but establishing effective EMS-hospital communication pathways within regional STEMI networks.

- Palliative morphine use in patients aged 90 years or older with advanced heart failure receiving best supportive care. [Journal Article]J Cardiol. 2026 Sep 29. [Online ahead of print]JC
- CONCLUSIONS: We found no clear evidence that age ≥ 90 years was associated with a lower early response rate or a higher frequency of observed sedation/respiratory events after low-dose morphine initiation. However, the limited sample size does not allow equivalence between the age groups to be established.

- Safety profile of finerenone: Pooled analysis of different types of arrhythmic events. [Letter]J Cardiol. 2026 Sep 28. [Online ahead of print]JC
- CONCLUSIONS: Finerenone shows no clear increase in clinically reported arrhythmic events in the pooled analysis; however, the included studies were not designed to assess the rate of arrhythmic outcomes and might have underestimated the true rates.

- Contemporary lipid management and achievement of LDL-C targets after acute myocardial infarction in Japan: Insights from the K-ACTIVE registry. [Journal Article]J Cardiol. 2026 Sep 21. [Online ahead of print]JC
- CONCLUSIONS: Although approximately 60% of patients achieved the current LDL-C target, this proportion may be insufficient in light of the anticipated transition toward a lower LDL-C threshold (<55 mg/dL) proposed in recent Japanese scientific statements and aligned with contemporary international guidelines. Early intensification of lipid-lowering therapy, including high-intensity statins and ezetimibe, may be warranted in selected patients to optimize secondary prevention.

- Association of in-hospital mineralocorticoid receptor antagonist initiation with outcomes according to guideline-defined eGFR and potassium eligibility in patients with heart failure. [Journal Article]J Cardiol. 2026 Sep 21. [Online ahead of print]JC
- CONCLUSIONS: In this nationwide real-world cohort, in-hospital MRA initiation was associated with a lower 2-year risk of the primary endpoint, and no statistically significant interaction was detected according to eGFR- and potassium-based eligibility. These hypothesis-generating findings warrant prospective evaluation with systematic safety monitoring.

- Discharge use of mineralocorticoid receptor antagonists and long-term outcomes in HFpEF. [Journal Article]J Cardiol. 2026 Sep 11. [Online ahead of print]JC
- CONCLUSIONS: In patients with preserved ejection fraction discharged after ADHF, MRA use at discharge was associated with a lower hazard of the composite of all-cause death or HF rehospitalization.

- Mitral valve prolapse: New clinical and scientific insights in 2026. [Review]J Cardiol. 2026 Sep 11. [Online ahead of print]JC
- Mitral valve prolapse (MVP) is a prevalent valvular abnormality affecting 2-3% of the general population. MVP has two distinct anatomical subtypes: Barlow's disease and fibroelastic deficiency; each reflects different underlying structural and pathological processes. While most individuals generally remain asymptomatic with subclinical regurgitation, some develop significant mitral regurgitation …

- Prognostic effect of liver fibrosis marker in systolic heart failure with cardiac nerve denervation and renal dysfunction. [Letter]J Cardiol. 2026 Sep 07. [Online ahead of print]JC
- CONCLUSIONS: Liver fibrosis parameters are also independent prognostic factors along with cardiac sympathetic nervous function and renal dysfunction in patients with HF, potentially enabling additive risk stratification of these patients.

- Paradigm shift in hydrogen medicine: Target molecules and catalytic mechanisms elucidated at the atomic level. [Review]J Cardiol. 2026 Aug 29. [Online ahead of print]JC
- Molecular hydrogen (H2) has been shown to exert antioxidant and anti-inflammatory effects across a broad spectrum of disease models and clinical trials; however, its direct reaction rate with hydroxyl radicals (•OH) in aqueous solution is exceedingly slow, and the underlying mechanism of action has remained unresolved for many years as the "hydrogen paradox". The present review delineates a parad…
