- Practice Variation in the Pharmacologic Management of Severe Alcohol Withdrawal: a Survey of Medical Toxicology and Critical Care Physicians. [Journal Article]
- CONCLUSIONS: This survey demonstrates that phenobarbital and benzodiazepines are the reported preferred treatments for severe alcohol withdrawal among medical toxicologists and critical care medicine physicians, with a slight overall preference for phenobarbital. Respondents also favored symptom-triggered dosing guided by a withdrawal-specific assessment scale, such as the CIWA-Ar. Importantly, these findings underscore substantial variability in clinical practice and clinician interest in randomized trials to identify optimal treatment strategies for severe alcohol withdrawal.

- Examining Trends in Early Childhood Lead Exposure Using the National Poison Data System: 2007-2024. [Journal Article]
- CONCLUSIONS: Lead exposure cases reported to NPDS in early childhood declined overall from 2007 to 2020 but have been increasing since 2020. In most cases, no specific substance of concern was identified. Additionally, few cases reported to poison centers experienced severe outcomes. Children aged 1-2 years accounted for the majority of cases in the National Poison Data System. Reasons may include increased mobility and exploration, hand-to-mouth behaviors, and efficient lead absorption in this age range. Other factors such as mandated lead testing at ages 12 and 24 months for children enrolled in Medicaid might also contribute to the observed age distribution.

- Acrodynia and CASPR2 Autoimmunity in Elemental Mercury Toxicity. [Case Reports]
- Elemental mercury exposure is associated with neurologic manifestations, which can improve after source removal with time. Autoimmune complications from mercury intoxication are rarely reported and may lead to worsening or persistent neurologic symptoms. A 2-year-old girl, in addition to her parents and sibling, developed cough and congestion initially attributed to a respiratory infection. For t…

- Adult U.S. Poison Center Exposure Case Trends Involving Nonprescription Dextromethorphan Amid the COVID-19 Pandemic: a Quasi-experimental Interrupted Time Series Study. [Journal Article]
- CONCLUSIONS: Nonprescription DXM exposures declined sharply at the pandemic's onset, with unintentional-therapeutic errors subsequently rebounding - especially among females - surpassing pre-pandemic levels toward the end of the pandemic's public health emergency declaration, while intentional exposures showed modest changes following initial declines. These findings underscore the need for targeted prevention strategies regarding inappropriate nonprescription DXM use and enhanced medication safety messaging during prolonged public health crises.

- Therapeutic Medication Errors in the Non-Healthcare Facility Setting Associated with Hospital Admission, 2000-2024. [Journal Article]
- CONCLUSIONS: These study findings highlight opportunities to strengthen prevention efforts targeting therapeutic medication errors, particularly among older adults and individuals using cardiovascular medications, which could meaningfully decrease associated morbidity, mortality, and healthcare costs.

- Octreotide Administration Methods in Sulfonylurea Poisoning: A Retrospective Chart Review. [Multicenter Study]
- CONCLUSIONS: The infusion group had a higher incidence of hypoglycemia and received more post-octreotide dextrose than intravenous bolus or subcutaneous groups, despite a larger total octreotide dose. These findings suggest octreotide infusions may be inferior, but further studies are needed to determine the optimal octreotide administration route for sulfonylurea induced hypoglycemia.

- Kratom-Associated Pulmonary Toxicity: A Systematic Review of Case Reports and Forensic Series with a Proposed Clinical Phenotype Classification. [Systematic Review]
- CONCLUSIONS: Reported kratom-associated pulmonary presentations cluster into five provisional, hypothesis-generating categories spanning primary lung injury and secondary systemic effects. Clinicians should consider kratom and 7-hydroxymitragynine in unexplained acute lung injury, alveolar hemorrhage, eosinophilic pneumonitis, or opioid-type respiratory depression; targeted toxicology and standardized reporting are needed to refine this framework.

- Systematic Review and Meta-Analysis of Lipid Emulsion for Non-Local Anesthetic Poisoning. [Systematic Review]
- CONCLUSIONS: Intravenous lipid emulsion is associated with reduced intensive care unit admission and mortality for aluminum phosphide and organophosphate poisonings and a reduced length of admission for aluminum phosphide, organophosphate, and antipsychotic poisonings. The evidence is limited by study heterogeneity and study quality. Larger, high-quality trials are needed to confirm benefit.

- Targeting Mitochondrial Dysfunction in Acute Organophosphate Toxicity: A Preliminary Evaluation of Cyclosporine A. [Journal Article]
- CONCLUSIONS: These preliminary findings support the feasibility of integrating mitochondrial respiratory phenotyping into translational models of acute OP toxicity and further support investigation of mitochondrial-targeted strategies in OP-associated mitochondrial dysfunction. The observed respiratory and physiologic trends associated with CsA exposure, particularly in the pretreatment paradigm, provide an exploratory foundation for future preclinical studies evaluating mitochondrial-directed countermeasures in OP poisoning.

- Metal Chelation Following Inappropriate Testing: Approach to Non-Medically Indicated Therapy Requests : Ethics in Practice. [Journal Article]
- Requests for chelation therapy following inappropriate or scientifically unsound testing in medical toxicology practice raise important ethical tensions between patient autonomy, professional integrity, and the therapeutic relationship. The central tension is how medical toxicologists should respond when patients strongly request non-medically indicated therapies that lack evidence of benefit and…
