- Frailty as a Predictor of Outcomes After Fibula Free Flap for Head and Neck Reconstruction: A Multi-Institutional Analysis Using the mFI-5. [Multicenter Study]Microsurgery. 2026 Oct; 46(7):e70301.M
- CONCLUSIONS: Frailty is a stronger predictor of postoperative morbidity than chronological age. Incorporating frailty assessment may enhance risk stratification and perioperative decision-making.

- Evaluating the Impact of Therapeutic Anticoagulation Management in Head and Neck Free Tissue Transfer: A Retrospective Study of 126 Cases. [Journal Article]Microsurgery. 2026 Oct; 46(7):e70298.M
- CONCLUSIONS: This study highlights the challenges of managing therapeutic anticoagulation in head and neck free flap surgery, where thromboembolic risk during interruption and hemorrhagic complications upon resumption point to the need for clearer guidelines. Evidence-based protocols are needed to replace discretionary practices and improve outcomes.

- Prophylactic Immediate Lymphatic Reconstruction for the Prevention of Secondary Lymphedema: A Systematic Review and Meta-Analysis of Comparative Studies. [Systematic Review]Microsurgery. 2026 Oct; 46(7):e70295.M
- CONCLUSIONS: Prophylactic LVS was associated with a lower incidence of lymphedema following axillary and inguinal lymph node dissection. However, confidence in the estimated treatment effect is limited by the predominance of observational evidence, methodological heterogeneity, and possible publication bias, highlighting the need for adequately powered multicenter randomized trials.

- Expanding Criteria for Totally Extraperitoneal Robotic DIEP Flap Harvest: A Case Report and Review of the Literature. [Case Reports]Microsurgery. 2026 Sep; 46(6):e70288.M
- Robotic-assisted deep inferior epigastric perforator (DIEP) flap harvest minimizes donor-site morbidity by reducing fascial disruption and preserving rectus muscle integrity. Traditional criteria for robotic DIEP have limited use to short intramuscular perforators, but this case demonstrates the feasibility of a totally extraperitoneal (TEP) approach in a patient with a long (91.6 mm) intramuscul…

- A Supercharged Pedicled PTAP-Saphenous Flap for Distal Lower-Limb Reconstruction. [Journal Article]Microsurgery. 2026 Sep; 46(6):e70299.M
- CONCLUSIONS: A "tripedicled," routinely supercharged PTA perforator plus saphenous flap, combined with a skin pedicle, provided reliable coverage of distal-leg, ankle, and foot defects with a low rate of flap loss. Based on our data and experience, this configuration is feasible and safe for crush injuries, elderly/comorbid patients, and longer flaps.

- Nasal Septum Reconstruction With Medial Femoral Condyle Free Flap: A Case Report and Literature Review. [Case Reports]Microsurgery. 2026 Sep; 46(6):e70297.M
- Nasal septum defects cause significant functional and aesthetic morbidity. In large septal defects requiring reconstruction, the goal is to achieve appropriate tissue matching, obtain well-vascularized tissue, and minimize donor site morbidity. Repair alternatives are quite limited in large defects. The medial femoral condyle (MFC) free flap is a preferred option in craniofacial reconstructions d…

- Evaluation of the Feasibility, Safety, and Effectiveness of a Tailored Enhanced Recovery After Surgery Protocol for Gastroepiploic Vascularized Lymph Node Transfer in Breast Cancer-Related Upper Extremity Lymphedema: A Preliminary Retrospective Comparative Study. [Journal Article]
- CONCLUSIONS: An ERAS-based protocol tailored to VLNT is feasible and safe, and is associated with improved perioperative recovery and reduced healthcare costs without evidence of compromised surgical or functional outcomes. These findings support the integration of ERAS principles into lymphatic microsurgery and warrant further prospective validation.

- Triceps Motor Branch Transfers to the Infraspinatus and Teres Minor Motor Branches for Restoration of Shoulder External Rotation: A Cadaver Study. [Journal Article]Microsurgery. 2026 Sep; 46(6):e70289.M
- CONCLUSIONS: Selective transfers from triceps branches to bIS and bTm are anatomically feasible. Medial head branches combine favorable length, diameter, and axon load, supporting their use as preferred donors to restore external rotation.

- Pre-Incision Time Intervals and Outcomes of Free Flap re-Exploration: A Retrospective Single-Center Study. [Journal Article]
- CONCLUSIONS: In this cohort, pre-incision delays (arrival and OR preparation (prep)) were not associated with failure. Signals instead pointed toward patient and anesthetic complexity and technically challenging salvage procedures. Thus, quality improvement attempts should prioritize rapid recognition/decision pathways and anesthesia-surgery coordination rather than focusing solely on logistics between ward and incision.

- Bilateral Lumbar Artery Perforator (LAP) Flaps for Extensive Lumbosacral Defects: A Report of Two Cases. [Case Reports]
- Extensive lumbosacral defects remain a major reconstructive challenge because of limited regional tissue mobility, contamination risk, proximity to neural structures, and frequent association with prior surgery or spinal instrumentation. Although lumbar artery perforator (LAP) flaps are recognized as muscle-sparing options for posterior trunk reconstruction, their capacity to safely reconstruct v…

- Post-Traumatic Lymphatic Vessel Thrombosis Treated With Thrombus Removal and Lymphaticovenous Anastomosis: A Case Report. [Case Reports]
- Lymphatic vessel thrombosis is a rare finding, and its role in the development of lymphedema remains unclear. Secondary lymphedema is usually related to oncologic lymph node dissection, radiotherapy, infection, or major tissue trauma. In contrast, focal lymphatic obstruction after minor limb trauma has rarely been described. We report the case of a 71-year-old woman who developed progressive Stag…
