- Long-term cost-effectiveness of CKD screening in middle-aged adults. [Journal Article]Nephrol Dial Transplant. 2026 Oct 24. [Online ahead of print]ND
- CONCLUSIONS: In Finns aged 55 years, one-time CKD screening with eGFR/UACR followed by SGLT2i and RAASi treatment improves long-term outcomes and is likely cost-effective.

- Circular RNAs at the Crossroads of Inflammation, Fibrosis and Tissue Repair. [Journal Article]Nephrol Dial Transplant. 2026 Oct 23. [Online ahead of print]ND
- Circular RNAs (circRNAs) are covalently closed RNA molecules generated through back-splicing of precursor mRNA (pre-mRNA) transcripts. Although circRNAs lack the 5' cap and the 3' poly(A) tail characteristic of linear mRNAs, their circular structure confers enhanced stability and resistance to exonucleolytic degradation, establishing circRNAs as important regulators of gene expression that contri…

- The Veterans Affairs Pentoxifylline in Diabetic Kidney Disease (VA PTXRx) Study: Baseline Characteristics. [Journal Article]Nephrol Dial Transplant. 2026 Oct 22. [Online ahead of print]ND
- CONCLUSIONS: The results, anticipated in 2027, are expected to determine whether PTX is a treatment option for patients with high-risk DKD. Because PTX is a generic medication, it is inexpensive in many health systems, which would facilitate access if it proves effective. Trial Registration: This study is registered with clinicaltrials.gov (Identifier: NCT03625648).

- Pentoxifylline for chronic kidney disease in people with diabetes: a meta-analysis of randomized trials. [Journal Article]Nephrol Dial Transplant. 2026 Oct 22. [Online ahead of print]ND
- CONCLUSIONS: Pentoxifylline reduces albuminuria, but the effect is heterogeneous, depends on which trials are counted, and may not reappear in a new trial. Kidney failure and death have scarcely been tested. The priority is an adequately powered, long-term trial reporting hard endpoints; gastrointestinal intolerance should be anticipated.

- C5 Inhibition for Anti-Cancer Therapy-Induced Renal Thrombotic Microangiopathy. [Journal Article]Nephrol Dial Transplant. 2026 Oct 22. [Online ahead of print]ND
- CONCLUSIONS: In this rigorously phenotyped cohort of patients with biopsy-proven, anti-cancer therapy-associated renal TMA, we observed a signal of benefit from C5 inhibition, most consistently for renal recovery and possibly extending to hematologic outcomes, particularly among patients with gemcitabine-associated TMA.

- Aldosterone and Residual Cardiorenal Risk: Pathophysiology and Emerging Therapeutic Strategies. [Journal Article]Nephrol Dial Transplant. 2026 Oct 16. [Online ahead of print]ND
- Aldosterone, the terminal effector of the renin-angiotensin-aldosterone system, regulates sodium retention, potassium excretion, and blood pressure. Beyond these classical effects, aldosterone-associated and mineralocorticoid receptor (MR)-mediated signaling is implicated in inflammation, oxidative stress, fibrosis, albuminuria, and progressive cardiorenal injury, while its synthesis is regulated…

- High sensitivity C-reactive protein and the risk of cardio-kidney outcomes in adults with moderate to severe chronic kidney disease: a nationwide study. [Journal Article]Nephrol Dial Transplant. 2026 Oct 16. [Online ahead of print]ND
- CONCLUSIONS: In this nationwide cohort of adults with moderate to severe CKD receiving nephrology care, inflammation was common, and higher hsCRP levels were associated with adverse cardiovascular and kidney outcomes, as well as mortality. Although associations were less consistent for KFRT, these findings collectively support the utility of hsCRP monitoring in this population.

- Long forms of cardiac troponin T are independent of kidney function and improve the diagnosis of myocardial infarction. [Journal Article]Nephrol Dial Transplant. 2026 Oct 16. [Online ahead of print]ND
- CONCLUSIONS: In contrast to hs-cTnT, long cTnT is unaffected by kidney function and outperforms hs-cTnT in the detection of MI, with the greatest diagnostic advantage observed in patients with lower eGFR.

- Anti-CD38 monoclonal antibodies in kidney disease: mechanistic rationale and emerging clinical evidence. [Journal Article]Nephrol Dial Transplant. 2026 Oct 15. [Online ahead of print]ND
- Over the past decades, the therapeutic landscape of nephrology has been transformed by the expanding use of immunosuppressive therapies, reflecting major advances in our understanding of the roles of the innate and adaptive immune systems in the pathogenesis of kidney diseases. While B-cell depletion via rituximab or obinutuzumab has improved immune-mediated kidney disease management, these agent…

- Microangiopathic lesions associated with poor immunosuppressive therapy response in IgA nephropathy. [Journal Article]Nephrol Dial Transplant. 2026 Oct 15. [Online ahead of print]ND
- CONCLUSIONS: Renal arteriolar microangiopathic lesions are relatively common in IgAN and are associated with worse clinical outcomes and poorer response to immunosuppressive therapy. These findings suggest that conventional immunosuppression may be less effective for this high-risk subgroup, necessitating further research into targeted therapeutic strategies.
