- Can nailfold videodermatoscopy detect microvascular alterations in patients with migraine? A prospective case-control study. [Journal Article]Neurol Sci. 2026 Oct 23; 47(11).NS
- CONCLUSIONS: Patients with migraine exhibited significantly more nailfold microvascular abnormalities than healthy controls. These findings suggest potential systemic microvascular involvement in migraine and indicate that nailfold videodermatoscopy may represent a practical, noninvasive tool for assessing peripheral microvascular alterations. Further multicenter prospective studies are warranted.

- Transient focal neurological episodes in cerebral amyloid angiopathy-related convexity subarachnoid hemorrhage. [Case Reports]Neurol Sci. 2026 Oct 23; 47(11).NS
- CONCLUSIONS: This case demonstrates that CAA-related cSAH may present with recurrent transient monoparesis that closely mimics TIA. Careful evaluation of susceptibility weighted imaging is important for recognizing the underlying hemorrhagic lesion in patients with recurrent stereotyped TFNEs.

- Neuronal intermediate filament neurologic autoimmunity in a patient with Merkel cell carcinoma receiving pembrolizumab. [Case Reports]Neurol Sci. 2026 Oct 23; 47(11).NS
- CONCLUSIONS: This case expands the growing spectrum of ICI-associated NIF paraneoplastic neurologic autoimmunity complicating Merkel cell carcinoma. To our knowledge, persistent hemifacial and cervical spasm has rarely been reported in this clinical setting. Early recognition of this syndrome, prompt antibody testing, discontinuation of ICI therapy, and initiation of therapy may improve neurological outcomes while informing further oncologic management.

- STN-SNr co-stimulation in Parkinson's disease: a systematic review and meta-analysis of motor and freezing-of-gait outcomes. [Systematic Review]Neurol Sci. 2026 Oct 23; 47(11).NS
- CONCLUSIONS: Low-certainty evidence suggests no clinically important difference in global motor outcomes, while very-low-certainty evidence does not demonstrate a consistent additional FoG benefit. Improvements versus stimulation-off or baseline cannot establish an incremental SNr effect. Larger phenotype-stratified trials with objective gait endpoints and systematic adverse-event reporting are needed.

- APOE ε4 accelerates plasma p-tau217 trajectory: a Bayesian joint longitudinal analysis of ADNI. [Journal Article]Neurol Sci. 2026 Oct 23; 47(11).NS
- CONCLUSIONS: APOE ε4 carriage predicts a steeper plasma p-tau217 trajectory over up to ten years, an effect that is predominantly direct rather than amyloid-mediated. Individual-level p-tau217 trajectory tracks cognitive decline; GFAP does not. Plasma p-tau217 trajectory is informative of cognitive trajectory, although whether it predicts or parallels decline cannot be determined from these data.

- Social vulnerability as a key dimension of frailty in Alzheimer's disease and frontotemporal dementia. [Journal Article]
- CONCLUSIONS: Our findings point to an urgent need for person-centered care strategies that extend beyond biomedical models. Deficits in social connections and support represent a primary and common underpinning of vulnerability in dementia populations. A timely periodic evaluation of non-physical determinants of vulnerability may contribute to a better and earlier detection of maladaptive trajectories with a consequent beneficial effect on the well-being and burden of the patient-caregiver dyad.

- Clinical characteristics and prognosis of central nervous system Klebsiella pneumoniae infection: a focus on hypervirulent and classical strains. [Journal Article]
- CONCLUSIONS: hvKP and cKP CNS infections differ significantly. hvKP is associated with community-onset severe disease (multiple brain abscesses, pyoventricle, focal deficits) and high mortality, while cKP links to nosocomial factors and better outcomes. mNGS is key for diagnosis, offering high CSF positivity and concurrent virulence/resistance profiling. Potential prognostic factors identified include high CSF mNGS reads, resistance genes, early liver abscess drainage, timely intravitreal injection, and combination therapy.

- Diet and physical activity in multiple sclerosis: associations with quality of life and disability by disease duration. [Journal Article]
- CONCLUSIONS: Physical activity is associated with higher QoL and lower disability irrespective of MS duration and should be integrated across the disease course. MS duration may modify associations between other lifestyle factors and QoL, supporting consideration of disease stage when tailoring lifestyle recommendations.

- Computerized fine motor assessment in Parkinson's Disease: Task-specific measures provide complementary clinical information. [Journal Article]
- CONCLUSIONS: These findings suggest that MLS-derived task-specific measures provide objective quantitative information that complements conventional clinical motor assessment in Parkinson's disease. Analysis of individual task-specific parameters may provide complementary clinically relevant information beyond a single global composite score. However, larger prospective studies are required to determine the clinical utility of task-specific computerized fine motor assessment for monitoring disease progression and treatment response.

- Cerebrospinal fluid positive for SARS-CoV-2 associated with neurological manifestations and proteome alterations in a multiple myeloma patient. [Case Reports]
- CONCLUSIONS: This case supports the hypothesis that SARS-CoV-2 can invade the central nervous system and cause severe neurological impairment, even in the absence of typical respiratory symptoms. The findings underscore the importance of considering SARS-CoV-2 in the differential diagnosis of rapidly progressive neurological syndromes and highlight the need for further investigation into the mechanisms of neuroinvasion and the potential neurotropism of emerging viral variants.
