- An Empirical Analysis of Litigation Involving Locum Tenens Neurosurgery and Spine Surgery in the United States in the Past 40 Years. [Journal Article]Neurosurgery. 2026 Nov 02. [Online ahead of print]N
- CONCLUSIONS: This is the first study to provide empirical data on the legal liability involving the practice of neurosurgery as locum tenens in the United States from 1980 to 2025. As locum tenens utilization continues to grow within neurosurgery, neurosurgeons must carefully review contracts, confirm malpractice coverage, and understand institutional liability frameworks before entering such a work model.

- Hypoxia Paradox in Middle Meningeal Artery Embolization for Chronic Subdural Hematoma: A Mechanistic Framework for Treatment Resistance. [Journal Article]Neurosurgery. 2026 Nov 02. [Online ahead of print]N
- CONCLUSIONS: The hypoxia paradox represents a testable mechanistic hypothesis, offering a coherent explanation for heterogeneous MMAE outcomes and generates specific, testable predictions regarding post-MMAE molecular signatures in the outer hematoma membrane. Prospective paired tissue sampling integrated with multimodal molecular analyses is essential to validate this model and advance CSDH management toward individualized, mechanism-guided therapy.

- Long-Term Outcomes of Cement Augmented Percutaneous Pedicle Screw Fixation in Cancer-Related Vertebral Fractures. [Journal Article]Neurosurgery. 2026 Oct 30. [Online ahead of print]N
- CONCLUSIONS: Cement-augmented PPSF demonstrated long-term durability for stabilization of oncological VCFs, with a low rate of revision surgery. Mechanical stability was accompanied by improvements in pain and functional status. Increasing age demonstrated an association with increased revision rates, although the low number of events limits definitive identification of predictors of failure.

- Association Between Craniotomy and Outcomes in Patients With Acute Subdural Hematoma: Analysis of a Nationwide Trauma Registry in Japan. [Journal Article]Neurosurgery. 2026 Oct 30. [Online ahead of print]N
- CONCLUSIONS: In patients with large ASDHs, craniotomy was significantly associated with improved survival, particularly among patients age 65 years and older and those with lower GCS scores (6-12). A uniform approach based solely on hematoma thickness may be suboptimal. Instead, surgical decision-making needs to adopt a more individualized approach that incorporates age and neurological severity.

- What Is the Most Appropriate Method for Determining Minimally Clinical Important Difference in Grade 2 Spondylolisthesis? A QOD Spine CORe® Study. [Journal Article]Neurosurgery. 2026 Oct 30. [Online ahead of print]N
- CONCLUSIONS: Using satisfaction as an anchor, this study identified the optimal percentage cutoff as the most appropriate method for determining MCID in ODI and NRS pain scores, while the optimal numeric cutoff was most appropriate for EQ-5D in grade 2 spondylolisthesis. These findings provide reference definitions for evaluating postoperative improvement in this patient population.

- Outcomes of Microvascular Decompression Versus Stereotactic Radiosurgery in the Treatment of Trigeminal Neuralgia: A Decades-Long, Single-Center Retrospective Cohort Analysis. [Journal Article]Neurosurgery. 2026 Oct 30. [Online ahead of print]N
- CONCLUSIONS: Both MVD and SRS were effective in treating TN-associated pain, but MVD offers superior durability and medication reduction at the cost of higher complication rates but still an acceptable risk profile. MVD may be preferred for low surgical-risk patients, while SRS remains a valuable noninvasive option. MRI reliably predicts neurovascular conflict, and neurolysis may be considered when no vascular conflict is present.

- This practice guideline from the American Epilepsy Society and the Congress of Neurological Surgeons provides evidence-based recommendations for surgical treatment of epilepsy for adults and children. It is an update to previous practice parameters published by the Academy of Neurology in 2003. The multidisciplinary panel addressed resective procedures for mesial temporal epilepsy, neocortical ep…

- Is There a Cervical Alignment Threshold Warranting Cervical Inclusion in the Construct? [Journal Article]Neurosurgery. 2026 Oct 26. [Online ahead of print]N
- CONCLUSIONS: Two years after TL ASD surgery, 13.9% had CS decompensation, with 14% requiring CS extension. Baseline CS alignment independently predicted CS decompensation. Patients with preoperative C2-PA >26.2° may benefit from CS inclusion in the TL construct after external validation of this threshold by future research.

- Machine Learning Prediction of Treatment Failure After Middle Meningeal Artery Embolization for Subdural Hematoma. [Journal Article]Neurosurgery. 2026 Oct 23. [Online ahead of print]N
- CONCLUSIONS: This ML model stratified MMAE patients into risk groups with a 37.2-fold difference in treatment failure. High-risk patients had 59.6% failure, whereas low-risk patients had 1.6% failure. Risk stratification may improve patient selection and monitoring.

- CONCLUSIONS: Supplemental NLP-modified radiology reports were associated with improved objective readability and higher patient-reported perceived ease of understanding in a spine surgery initial consultation setting. These findings support further investigation of NLP-generated supplemental explanations within supervised clinical workflows.

- White Matter Functional Disconnection Predicts Long-Term Disability and Early Mortality in Spontaneous Intracerebral Hemorrhage. [Journal Article]Neurosurgery. 2026 Oct 16. [Online ahead of print]N
- CONCLUSIONS: Assessment of white matter functional disconnection severity seems to be a promising approach for predicting long-term disability after ICH, although prospective validation in larger multicenter cohorts is required before clinical implementation. The findings suggest that different brain networks exhibit variable vulnerability to injury and differential potential for functional recovery.
