- IgA nephropathy: gut microbiome insights and unmet needs in pediatrics. [Review]Pediatr Nephrol. 2026 Oct 26. [Online ahead of print]PN
- Immunoglobulin A nephropathy (IgAN) is the most common primary glomerulopathy worldwide and a leading cause of chronic kidney disease and kidney failure in children and young adults. Well-established pathogenic pathways include genetic susceptibility, intrinsic B-cell glycosylation abnormalities, complement activation, and immune dysregulation, and accumulating evidence further places the gut mic…

- Predicting the causative pathogen in pediatric hemodialysis catheter infections before culture results: the role of inflammatory indices. [Journal Article]
- CONCLUSIONS: CBC-derived inflammatory indices, particularly NLR and SII, showed moderate discrimination between Gram-negative and Gram-positive catheter-related infections in pediatric HD patients. Their immediate availability may provide useful complementary information on pathogen likelihood and infection severity while awaiting culture confirmation, supporting early risk assessment and antimicrobial stewardship without replacing standard empirical therapy.

- External validation of the renal angina index for early prediction of severe acute kidney injury in critically ill children: a prospective cohort study from Vietnam. [Journal Article]
- CONCLUSIONS: RAI showed acceptable performance for early prediction of severe AKI in critically ill children and may serve as a practical bedside risk stratification tool in resource-limited PICU settings.

- When to stop kidney replacement therapy in critically ill children: current evidence and practical considerations from a pediatric critical care nephrology perspective. [Journal Article]
- Kidney replacement therapy (KRT) is a life-saving intervention for critically ill children with severe acute kidney injury (AKI). However, the optimal timing of KRT liberation remains uncertain, and decisions regarding treatment discontinuation are among the least standardized aspects of AKI management. Whereas the timing of KRT initiation has been extensively investigated, evidence guiding KRT l…

- Interobserver agreement in prenatal ultrasound assessment of fetal urinary tract dilatation. [Journal Article]
- CONCLUSIONS: Significant interobserver variability exists in the interpretation of commonly used prenatal ultrasound features for evaluating the fetal urinary tract and predicting postnatal outcomes in UTD. The SFU and the ONEN grading systems improved agreement, supporting the need for standardized ultrasound assessment tools to enhance prenatal counseling and antenatal management in fetuses with UTD.

- Differential associations of early fluid accumulation with acute kidney injury and mortality in critically ill children: a prospective cohort study with mediation and propensity-score analyses. [Journal Article]
- CONCLUSIONS: Early FA shows differential associations: a potentially modifiable risk marker for AKI, associated with longer intensive care, but not independently associated with death once severity and AKI are considered. Limiting early FA may be a reasonable target for AKI prevention, although this requires interventional confirmation; PRISM III remains the mortality-prognostic tool.

- Early renal response and baseline serum albumin in childhood proliferative lupus nephritis managed with a mycophenolate mofetil-first, response-guided strategy. [Journal Article]
- CONCLUSIONS: Children selected for an MMF-first, response-guided strategy had favorable short-term renal response. Higher baseline serum albumin was associated with earlier CRR, particularly during the early treatment period, but should be interpreted as a marker of response kinetics rather than a validated independent predictor. These findings should not be generalized to children with severe proliferative LN requiring an alternative initial treatment strategy.

- Clinical and metabolic factors associated with surgical intervention in pediatric urolithiasis: a multicenter retrospective study. [Journal Article]
- CONCLUSIONS: Stone size was the dominant factor associated with surgical intervention, followed by stone location, infectious presentation, and hydronephrosis. Hematuria with pyuria was not independently associated once stone burden was accounted for. External validation is required before clinical use.

- When to start continuous kidney replacement therapy in critically ill children: a practical, PICU-focused, evidence-based review. [Review]
- Acute kidney injury (AKI) and fluid overload (FO) are common in critically ill children and have been shown to be associated with adverse outcomes. Continuous kidney replacement therapy (CKRT) is the preferred renal support modality in paediatric intensive care to treat children with AKI and FO and may be necessary for non-renal indications like life-threatening toxin or electrolyte removal. Howe…

- Therapy for kidney vasculitis in children during the acute phase and maintenance: updates and insights from new adult therapies. [Review]
- Kidney vasculitides in children encompass a heterogeneous group of immune-mediated disorders with distinct pathogenic mechanisms but a shared risk of irreversible kidney injury. Although pediatric vasculitides are rare, advances in understanding disease biology, coupled with significant therapeutic developments in adult vasculitides, have reshaped treatment paradigms. This review summarizes curre…

- The pharmacokinetics of a single intravenous dose of difelikefalin in adolescents aged 12 to 17 years on hemodialysis. [Journal Article]
- CONCLUSIONS: After a single difelikefalin 0.5 μg/kg dose, the overall exposure in adolescents on hemodialysis was comparable to prior adult data, with no new safety signals identified, supporting further evaluation of the 0.5 μg/kg dose in adolescents with moderate-to-severe chronic kidney disease-associated pruritus.
