- Evaluating and Comparing Approaches to Measuring Quality of Postdischarge Mental Health Outpatient Follow-Up Care. [Journal Article]Psychiatr Serv. 2026 Oct 29; :appips20250472. [Online ahead of print]PS
- CONCLUSIONS: Greater engagement in 30-day postdischarge behavioral health follow-up is associated with greater outpatient behavioral health engagement, higher likelihood of receiving primary care, and lower 1-year all-cause mortality. These findings support resource allocation toward postdischarge care coordination and broadly adopting more stringent quality standards.

- Involuntary Treatment in First-Episode Psychosis and Mania: A Retrospective Population-Based Cohort Study. [Journal Article]Psychiatr Serv. 2026 Oct 29; :appips20260138. [Online ahead of print]PS
- CONCLUSIONS: The authors found that involuntary treatment at FEP/FEM is common and is characterized by greater baseline illness severity. Involuntary status can serve as an entry point into care without excess long-term service utilization, and both groups showed similar rates of functional improvement. Persistently lower functioning and higher odds of vocational disengagement in the involuntary group support the need for targeted vocational interventions.

- The Asynchronous Frame: Patient Portal Messaging Between Psychiatric Visits. [Journal Article]Psychiatr Serv. 2026 Oct 29; :appips20260317. [Online ahead of print]PS
- Asynchronous patient portal messaging has become a routine part of psychiatric practice, yet its impact on the therapeutic frame remains poorly defined. Unlike telephone calls or e-mail, portal-based communication is embedded into the electronic health record, extending care beyond scheduled visits. Although patient portals may improve access, transparency, and satisfaction, concerns have been ra…

- Evaluating Presidential Fitness: The 25th Amendment and Presidential Disability. [Journal Article]Psychiatr Serv. 2026 Oct 29; :appips20260571. [Online ahead of print]PS
- The 25th Amendment to the U.S. Constitution offers procedures to address presidential disability. However, it omits a standard and process by which inability to discharge the duties of the office should be judged. Formulation of a standard and an approach to evaluation should begin by recognizing the similarities with workplace fitness-for-duty evaluations. Thus, operationalizing the 25th Amendme…

- The Need for a Validity-Based Diagnostic Hierarchy in DSM-6. [Review]Psychiatr Serv. 2026 Oct 22; :appips20260333. [Online ahead of print]PS
- The DSM's loss of a diagnostic hierarchy has contributed to widespread diagnostic proliferation and a misunderstanding of comorbidity. The original concept of comorbidity referred to the co-occurrence of two distinct disease entities, each with its own etiology and pathology. In psychiatry, by contrast, comorbidity has come to mean the accumulation of multiple DSM diagnoses, many of which have ov…

- CONCLUSIONS: Ketamine provided additional QALYs while saving costs relative to ECT for treatment-resistant depression. Health insurance plans that cover all costs of ECT should also cover all costs of ketamine treatment for treatment-resistant depression.

- In light of the dramatic rise in Medicare Advantage (MA) enrollment over the past 20 years and the negative impacts of certain MA plans on accessibility, affordability, and equity in mental health care, the authors analyzed literature and publicly available data to explore the impacts of the expansion of MA on psychiatric care. A comprehensive search was conducted in PubMed between December 2025 …

- Discontinuation of the U.S. Food and Drug Administration's clozapine risk evaluation and mitigation strategy program led to a package insert (PI) revision in June 2025. However, that PI update retained many elements that are inconsistent with modern clozapine prescribing, including rapid titration, twice-daily dosing at the start of therapy, and titration without regard to patient factors (e.g., …

- CONCLUSIONS: The model performed well in predicting self-harm events in the 90 days after an ED visit for individuals with behavioral health diagnoses. Such machine learning models could be useful in identifying ED patients who may need safety planning and close follow-up after discharge.
