- CD40-CD40L interactions in systemic autoimmune diseases. [Journal Article]Rheumatology (Oxford). 2026 Oct 16. [Online ahead of print]R
- Systemic autoimmune diseases arise from a heterogeneous interplay of genetic, epigenetic, and environmental factors that culminates in loss of tolerance, chronic inflammation, and tissue injury. Among the pathways linking susceptibility to effector disease, CD40-CD40L is a central regulator of antigen presentation, T-B cell collaboration, and germinal centre responses, while also contributing to …

- Depressive symptoms are associated with achieving remission in PsA patients - an analysis from the RABBIT-SpA register. [Journal Article]BMC Rheumatol. 2026 Oct 14; 10(1).BR
- CONCLUSIONS: Depressive symptoms were associated with higher disease activity and less frequent remission in PsA, with no evidence that this effect differed by sex. Routine screening for depressive symptoms should be considered in PsA management to identify patients at risk of suboptimal treatment response.

- Clinical characteristics and severity classification in Mixed Connective Tissue Disease: a nationwide retrospective study. [Journal Article]Rheumatology (Oxford). 2026 Oct 15. [Online ahead of print]R
- CONCLUSIONS: Mixed connective tissue disease exhibits marked clinical heterogeneity. Severity classification was not associated with off-scale-high anti-U1-RNP antibody status or broad phenotypic categories. These findings support rigorous cardiopulmonary screening and management tailored to the dominant clinical phenotype.

- Responsiveness of patient-reported outcomes by disease trajectory in systemic lupus erythematosus. [Journal Article]Rheumatology (Oxford). 2026 Oct 15. [Online ahead of print]R
- CONCLUSIONS: FACIT-F and SF-36 PCS demonstrated the greatest responsiveness and alignment with clinician-assessed disease activity. EQ-5D-3L showed less consistent responsiveness, suggesting domain-specific PROs may better capture treatment benefit in SLE trials and health technology assessments.

- Organ damage profiles and associated factors in juvenile versus adult-onset SLE: a matched analysis from the Japanese ANSWER-SLE cohort. [Journal Article]Rheumatology (Oxford). 2026 Oct 15. [Online ahead of print]R
- CONCLUSIONS: Juvenile-onset SLE is associated with organ damage compared to adult-onset SLE, with prominent ocular, neurological, renal, cardiovascular, and musculoskeletal systems.

- Emerging therapies for knee osteoarthritis. [Review]
- Knee osteoarthritis (OA) is the most prevalent form of OA and a leading cause of chronic pain and disability worldwide. Its growing burden, driven mainly by ageing and obesity, is associated with major societal costs and increased cardiovascular mortality. Knee OA is now recognized as a heterogeneous whole-joint disease involving biomechanical, metabolic, inflammatory, osteochondral and neurovasc…

- Spatial frequency domain imaging for assessment of skin fibrosis in systemic sclerosis: a prospective cohort study. [Journal Article]Lancet Rheumatol. 2026 Oct 14. [Online ahead of print]LR
- CONCLUSIONS: SFDI provides an objective, non-invasive measure of skin involvement in systemic sclerosis that aligns with established clinical, imaging, and histological assessments. These findings support its potential as a quantitative outcome measure for evaluating skin disease in systemic sclerosis, consistent with Outcome Measures in Rheumatology principles.

- A clinical model for identifying proliferative lupus nephritis: a multicenter study. [Journal Article]Rheumatology (Oxford). 2026 Oct 14. [Online ahead of print]R
- CONCLUSIONS: We developed and externally validated a noninvasive clinical model based on routinely available clinical variables for identifying PLN. This model may provide a practical tool for risk stratification and support clinical evaluation in patients with lupus nephritis.

- Long-term comparative outcomes of rituximab and belimumab in lupus patients. [Journal Article]Rheumatology (Oxford). 2026 Oct 14. [Online ahead of print]R
- CONCLUSIONS: In this observational cohort, belimumab was associated with similar rates of low disease activity as rituximab. Drug survival differed and warrants further study.

- The 2026 British Society for Rheumatology guideline for the treatment of polymyalgia rheumatica. [Practice Guideline]
- Polymyalgia rheumatica (PMR) is a common inflammatory rheumatic condition in older adults. Glucocorticoids (GCs) remain the cornerstone of treatment, but many people with PMR relapse during tapering and are exposed to prolonged GC therapy with associated toxicity. There is therefore a need for evidence-based strategies that optimize disease control while minimizing GC-related harm. This guideline…

- Genomic ascertainment of systemic autoinflammatory diseases: a retrospective cohort study in two population-based biobanks. [Journal Article]Lancet Rheumatol. 2026 Oct 13. [Online ahead of print]LR
- CONCLUSIONS: These findings suggest that monogenic autoinflammatory diseases are more common than currently recognised. Despite carrying disease-associated variants, almost no affected individuals had a documented systemic autoinflammatory disease diagnoses, pointing to a substantial gap in disease recognition and a missed opportunity to identify individuals with systemic inflammation who could benefit from genetic evaluation to guide therapy.

- Trained immunity in chronic, relapsing and low-grade joint inflammation. [Review]
- Trained immunity is a form of innate immune memory mediated by metabolic and epigenetic reprogramming that promotes sustained inflammatory responses. Accumulating evidence implicates a role for maladaptive trained immunity in sustaining inflammation and promoting disease progression across distinct forms of inflammatory joint disease, including rheumatoid arthritis, gout, calcium pyrophosphate de…
