- Incidence and characteristics of cytokine release syndrome in patients with systemic sclerosis undergoing autologous haematopoietic stem cell transplantation. [Journal Article]Semin Arthritis Rheum. 2026 Oct 11; 81:153087. [Online ahead of print]SA
- CONCLUSIONS: CRS is highly prevalent and clinically relevant in SSc patients undergoing autologous HSCT. The frequent respiratory manifestations may reflect SSc-related pre-existing cardiopulmonary involvement, vasculopathy, and fluid exposure during conditioning, rather than inflammation alone, suggesting a distinct CRS phenotype.

- Evolution of outcome assessment in clinical trials of rheumatoid arthritis: A scoping review. [Review]Semin Arthritis Rheum. 2026 Oct 09; 81:153088. [Online ahead of print]SA
- CONCLUSIONS: Outcome assessment in rheumatoid arthritis clinical trials has evolved substantially over the past four decades, transitioning from symptom-focused evaluation to multidimensional assessment encompassing clinical, structural, functional, biological, and patient-centered domains. This review synthesizes evidence from 611 randomized clinical trials and provides a comprehensive mapping of the historical evolution of outcome measurement, offering a valuable reference for future clinical research and the development of standardized core outcome sets.

- Association of antenatal glucocorticoids with risk of autoimmune diseases in children: a nationwide cohort study. [Journal Article]Semin Arthritis Rheum. 2026 Oct 04; 81:153085. [Online ahead of print]SA
- CONCLUSIONS: This large nationwide cohort study suggests that children exposed to antenatal glucocorticoids, particularly preterm children, may have an increased risk of JIA and KD. The compelling findings suggest that preterm children exposed to antenatal glucocorticoids should be closely monitored for potential immune-related health outcomes in early childhood.

- CONCLUSIONS: Gradual bDMARD dose reduction while maintaining background cDMARD as backbone therapy showed the most favourable comparative flare profile, whereas bDMARD discontinuation was associated with a substantially higher flare risk.

- Glucocorticoid discontinuation in patients with rheumatoid arthritis who initiated biologic or targeted synthetic agents: a real-world population-based analysis. [Journal Article]Semin Arthritis Rheum. 2026 Sep 23; 81:153083. [Online ahead of print]SA
- CONCLUSIONS: In the real-world setting, a high prevalence of RA patients was on GCs at the time of the initiation of b/tsDMARDs. However, only a limited proportion of these patients were able to discontinue GCs by months 3 and 6. Initiation of JAKi therapy was associated with a greater likelihood of glucocorticoid discontinuation compared with selected bDMARDs, while no statistically significant difference was observed relative to anti-IL6 therapy.

- All-cause emergency department visits and hospitalizations in persons with rheumatoid arthritis: A systematic review. [Review]Semin Arthritis Rheum. 2026 Sep 13; 81:153075. [Online ahead of print]SA
- CONCLUSIONS: Heterogeneity of estimates of all-cause acute care use in RA cohorts is considerable. Further understanding of the reasons for acute care use, impacts of health system structure, and geographic differences is needed.

- CONCLUSIONS: These recommendations on how and when to report the OMERACT-endorsed core domains in urate-lowering therapy trials address a gap in the current core outcome sets. Adoption of these recommendations should improve consistency and comparability across gout trials.

- Systemic sclerosis in Israel clinical characteristics, ethnic and spatial disparities: A nationwide study. [Journal Article]Semin Arthritis Rheum. 2026 Sep 13; 81:153077. [Online ahead of print]SA
- CONCLUSIONS: SSc prevalence in Israel is among the highest reported worldwide and shows marked spatial clustering. Ethnic differences vary after accounting for spatial dependence, with consistently high prevalence in Druze localities.

- CONCLUSIONS: Peripheral macrovascular thrombosis is a clinically relevant manifestation of CAPS, mainly presenting as deep vein thrombosis. Peripheral arterial thrombosis carried an elevated risk of major amputation, contributing substantially to long-term morbidity.

- Cyclophosphamide vs rituximab induction therapy for ANCA-associated vasculitis. A systematic review and meta-analysis of 12-month mortality. [Journal Article]Semin Arthritis Rheum. 2026 Sep 14; 81:153078. [Online ahead of print]SA
- CONCLUSIONS: Overall, there was a statistically significant difference favouring rituximab in 12-month mortality rates after cyclophosphamide or rituximab induction treatment for AAV. Subgroup sensitivity analysis considering study populations consistently supported this result although only treatment for GPA and baseline BVAS <17 reached statistical significance. Further direct studies between cyclophosphamide and rituximab induction are required.

- CONCLUSIONS: Renal and non-renal damage are highly prevalent in LN patients. Extra-renal damage is greater in the prevalent cohort, likely reflecting prior disease activity and higher cumulative exposure to immunosuppressive therapies, particularly glucocorticoids, before clinic entry.

- CONCLUSIONS: Both myositis cohorts exhibited significant impairments in physical function, maximal muscle power and muscle mass compared to an age-matched healthy cohort, reflecting substantial deterioration and muscle deficit. These findings underline the need for targeted, individualised and evidence-based interventions to support and improve the musculoskeletal health and functional status in individuals with myositis.

- CONCLUSIONS: In this large prospective study, people who smoke with RA were more likely to have ILA and BR than those without RA, but had similar prevalences of COPD and MP. Participants with RA, particularly those with COPD, had higher risk of acute respiratory exacerbation.

- CONCLUSIONS: Most studies evaluated instruments measuring "being physically and functionally able" and "participation beyond function". No studies tested instruments addressing all the themes of independence in the context of RA remission. A new measure of independence is recommended to be developed.
