- Asymmetric cement distribution in the coronal plane increases the risk of prosthetic loosening following total knee arthroplasty: an in-silico study. [Journal Article]BMC Surg. 2026 Jul 23; 26(1).BS
- CONCLUSIONS: Asymmetric cement distribution leads to an aggravated trend of compressive stress concentration in the cancellous bone on the side with a thinner cement mantle, where the cement thickness is less than 2 mm, the threshold for prosthesis loosening reported by published studies. This serves as a risk factor for increased aseptic loosening of the prosthesis.

- Workflow-oriented learning curve analysis of laparoscopic distal pancreatectomy: step-specific associations of operative maturation. [Journal Article]BMC Surg. 2026 Jul 22; 26(1).BS
- CONCLUSIONS: In this cohort, reductions in operative time during LDP were concentrated in anatomically complex dissection steps rather than uniform acceleration of the entire procedure. Step-specific analysis describes temporal patterns that characterize technical change during minimally invasive pancreatic surgery.

- C-Reactive protein and the CALLY index for predicting complicated appendicitis: a retrospective cohort study of 5,428 patients. [Journal Article]BMC Surg. 2026 Jul 22; 26(1).BS
- CONCLUSIONS: C-reactive protein remains the most robust laboratory marker for identifying complicated appendicitis. Composite biomarkers such as the CALLY index may provide additional value in risk stratification. These findings support the use of routinely available laboratory parameters in clinical risk assessment, although prospective validation is warranted.

- Comparative effectiveness of total thyroidectomy versus hemithyroidectomy for low-risk differentiated thyroid cancer: a multicenter retrospective cohort study with propensity score matching. [Multicenter Study]BMC Surg. 2026 Jul 23; 26(1).BS
- CONCLUSIONS: For appropriately selected patients with low-risk DTC, hemithyroidectomy provides oncologic outcomes comparable to total thyroidectomy while offering advantages in complications, thyroid-function preservation, quality of life, and healthcare costs. Treatment decisions should be individualized through shared decision-making. These findings support consideration of de-escalated surgery but require validation in prospective randomized trials.

- Same-day discharge sleeve gastrectomy: Is inpatient care worth it? [Journal Article]Surgery. 2026 Sep 28; :110721. [Online ahead of print]S
- CONCLUSIONS: In appropriately selected patients, same-day discharge after sleeve gastrectomy is associated with lower 90-day health care charges and comparable clinical outcomes as overnight stay sleeve gastrectomy.

- The operating room is a high-risk, high-entropy environment, prone to human error and preventable adverse events. Drawing on lessons from high-reliability organizations such as the aviation industry, Aimbient (formerly Operating Room Black Box by Surgical Safety Technologies, New York, NY) collects continuous, multimodal audiovisual and physiologic data capture to deepen our understanding of surg…

- Defining the learning curve of laparoscopic adrenalectomy: a single-center experience. [Journal Article]
- CONCLUSIONS: A learning curve was observed, with operative duration improving after approximately 23 cases. A secondary multivariable model identified a later model-based breakpoint at around 33 cases. These findings should be interpreted as cohort-specific estimates and suggest that learning-curve interpretation in laparoscopic adrenalectomy should consider both accumulated experience and operative factors that influence surgical performance.

- The implications of molecular alterations in cytologically benign thyroid nodules. [Journal Article]Surgery. 2026 Sep 25; 201:110709. [Online ahead of print]S
- CONCLUSIONS: Among thyroid nodules with benign cytology referred for molecular testing, 3%-11% demonstrated intermediate- or high-risk alterations. Considering this molecular heterogeneity, further investigation of clinicopathologic factors is warranted to inform nonoperative or preablation molecular risk stratification and ensure accurate exclusion of malignancy.

- A qualitative analysis of the impact of English proficiency on clinician-patient communication related to immediate breast reconstruction following mastectomy. [Journal Article]Surgery. 2026 Aug 26; 201:110561. [Online ahead of print]S
- CONCLUSIONS: We identified that modifiable, patient-reported communication barriers, along with social factors, influence immediate breast reconstruction decision-making. Targeted interventions to improve communication and interpreter services, foster trust, and advance equity are warranted to improve disparities in care.

- Surgery is not the end of the story: Challenges and strategies in the postoperative care of patients receiving long-term opioid therapy. [Journal Article]Surgery. 2026 Sep 25; 201:110701. [Online ahead of print]S
- CONCLUSIONS: Patients on long-term opioid therapy before surgery often have complex postoperative pain needs that challenge conventional perioperative workflows. Care pathways that support proactive patient-centered expectation setting, promote patient education, and strengthen cross-disciplinary communication may improve coordination and safety of postoperative opioid management.

- The impact of National Accreditation Program for Rectal Cancer accreditation on patient volume and geographical scope in a community hospital. [Journal Article]Surgery. 2026 Sep 25; 201:110699. [Online ahead of print]S
- CONCLUSIONS: National Accreditation Program for Rectal Cancer accreditation was associated with an increase in patient volume and a broader geographic scope of patients seeking care at a community hospital; however, this change cannot be disentangled from the hiring of 2 fellowship-trained colorectal surgeons. Accreditation may serve as a signal to patients and referring providers of high-quality, disease-specific care, potentially enhancing the institution's competitive standing and patient draw in the health care marketplace.

- Safety as a system: Event review, peer review, and professional practice evaluation. [Review]Surgery. 2026 Sep 28; 201:110716. [Online ahead of print]S
- CONCLUSIONS: Safety in surgery depends on the integration of robust reporting systems, structured event analyses, and continuous improvement, all within a just culture framework. By emphasizing a system of shared accountability over individual blame, health care organizations can transform adverse events into opportunities for learning and promote safer surgical care.

- Geriatric nutritional risk index and outcomes following a major surgical procedure: A national cohort study. [Journal Article]Surgery. 2026 Sep 25; 201:110707. [Online ahead of print]S
- CONCLUSIONS: Greater geriatric nutritional risk index-defined nutrition-related risk was independently associated with higher mortality, major complications, and readmission compared with no geriatric nutritional risk index. Geriatric nutritional risk index-based assessment may identify nutritionally vulnerable patients for preoperative optimization, early intervention, and tailored postoperative discharge planning.

- Comparative outcomes of central venous access via open jugular and ultrasound-guided subclavian approaches in low-weight pediatric patients: a study with secondary outcomes. [Randomized Controlled Trial]
- CONCLUSIONS: Both approaches were generally safe, with no major adverse events observed in either group. Ultrasound-guided subclavian access was associated with a shorter procedure time, whereas hematoma occurred only in the jugular group and catheterization failure and upper-limb edema were seen only in low-weight infants in the subclavian group. However, because access site and technique were evaluated together, their independent effects could not be determined. In addition, the findings in infants weighing < 2.5 kg were based on few events and should be considered hypothesis-generating rather than definitive.

- The Percorso Diagnostico-Terapeutico Assistenziale (Diagnostic, Therapeutic and Care Pathway, PDTA), has long provided a structured framework for cancer care, supporting continuity and adherence to clinical standards. However, as improvements in cancer outcomes are accompanied by increasing complexity in patient needs, particularly in breast cancer, greater attention is being directed towards mod…
