- The BioSynIRS trial: biologic versus synthetic mesh for multicompartment pelvic floor reconstruction. [Randomized Controlled Trial]
- CONCLUSIONS: No significant differences in safety were detected between biologic and synthetic mesh; functional and anatomical outcomes improved similarly. Given the pilot design and limited sample size, these findings support the feasibility, but not equivalence or non-inferiority, of the biologic mesh. Adequately powered trials are required to confirm these findings and to evaluate the long-term durability of the biologic mesh.

- Delayed secondary repair of third- and fourth-degree obstetric anal sphincter injuries: a randomized controlled trial comparing overlapping versus end-to-end sphincter repair. [Randomized Controlled Trial]
- CONCLUSIONS: In delayed secondary OASI repair, overlapping sphincteroplasty provided superior continence outcomes and lower complication rates than end-to-end repair, when combined with meticulous muscle mobilization and a standardized postoperative regimen. These findings support overlapping repair as the preferred technique in this setting.

- Clinical outcomes of transvaginal fistulectomy and internal orifice closure in rectovaginal fistulas secondary to stapled prolapsectomy: a monocentric case series. [Journal Article]Tech Coloproctol. 2026 Jul 20. [Online ahead of print]TC
- CONCLUSIONS: Transvaginal fistulectomy with internal orifice closure without fecal diversion could be a safe and effective surgical option for RVF secondary to SP. Given the limitations of this small retrospective single-cohort series, these findings should be considered preliminary.

- Predictors of ileal pouch-anal anastomosis failure in ulcerative colitis: a European tertiary referral center experience. [Journal Article]Tech Coloproctol. 2026 Jul 17. [Online ahead of print]TC
- CONCLUSIONS: While overall pouch failure rates remain low in specialized centers, exposure to preoperative biologic therapy significantly elevates this risk. The newly developed nomogram provides a valuable, evidence-based tool for tailored surgical counseling and personalized patient management.

- Endoanal ultrasound-measured internal anal sphincter changes after nonablative 1470-nm diode laser therapy for anal incontinence: a prospective single-arm pilot study. [Journal Article]
- CONCLUSIONS: In this uncontrolled pilot study, nonablative 1470-nm diode laser therapy was associated with EAUS-detected IAS changes and symptomatic improvement in selected patients with FI. Because of the absence of a control group, small absolute sonographic changes, and limited follow-up, these findings should be interpreted as preliminary and require confirmation in controlled studies with standardized imaging and functional assessment.

- Exercise frequency and physician burnout among colorectal surgeons in the USA: should we be worried? [Journal Article]Tech Coloproctol. 2026 Jul 14. [Online ahead of print]TC
- CONCLUSIONS: Burnout symptoms were commonly reported among colorectal surgeons in the USA. Higher self-reported exercise frequency was associated with lower reported rates of certain burnout symptoms, particularly feelings of being overwhelmed. However, given the low response rate and study design, these findings should be interpreted cautiously and do not establish a causal relationship. Further prospective studies are needed to better define the relationship between physical activity and physician burnout.

- Negative-pressure wound therapy for perineal wound complications after abdominoperineal resection/extralevator APR: Systematic review and meta-analysis. [Review]Tech Coloproctol. 2026 Jul 11. [Online ahead of print]TC
- CONCLUSIONS: This meta-analysis demonstrated no significant clinical benefit for NPWT in managing postoperative perineal wound after oncologic APR/ELAPE. Further randomized controlled trials with standardized protocols are needed to clarify the role of NPWT in this area.

- Digital rectal examination in rectal cancer response assessment: clinical performance and the need for standardized reporting. [Journal Article]Tech Coloproctol. 2026 Jul 12. [Online ahead of print]TC
- CONCLUSIONS: Within a multimodal watch-and-wait pathway, DRE at first reassessment appears to function primarily as a clinical warning-sign examination rather than as an isolated diagnostic test. A normal DRE does not exclude future residual disease or regrowth, whereas an abnormal DRE should be considered a relevant warning sign within multimodal reassessment. These findings are hypothesis-generating. The marked variability in reporting underscores the need for standardized terminology and structured documentation in rectal cancer response assessment.

- The role of pelvic floor muscle training in the management of idiopathic fecal incontinence: a single-center prospective study. [Journal Article]Tech Coloproctol. 2026 Jul 10. [Online ahead of print]TC
- CONCLUSIONS: Pelvic floor muscle exercises are an effective conservative treatment for fecal incontinence; however, physiotherapist-supervised training yields superior clinical outcomes compared with unsupervised exercise. Professional supervision, appropriate patient qualification, and patient motivation are key factors influencing the success of pelvic floor rehabilitation.

- Genital bar electrode for dorsal genital nerve stimulation: feasibility and safety of a novel approach for managing bowel dysfunction and overactive bladder. [Journal Article]Tech Coloproctol. 2026 Jul 09. [Online ahead of print]TC
- CONCLUSIONS: Inserting a genital bar electrode is feasible and safe. Preliminary results indicate a symptom response comparable to patch electrode stimulation in the majority.

- Robotic-assisted right colectomy with the DEXTER[®] Robotic Surgery System: first prospective multicenter study. [Multicenter Study]
- CONCLUSIONS: Right colectomy with DEXTER is feasible and the results of this study support the short-term safety and clinical performance of the device, even early in the learning curve. Studies with long-term follow-up and functional outcomes are needed to assess long-term safety and performance.

- A novel retro-muscular Sugarbaker technique for parastomal hernia prevention in end colostomy: combination of laparoscopic posterior component separation and extraperitoneal bowel pull-through-a prospective, single-arm study using the IDEAL framework. [Journal Article]Tech Coloproctol. 2026 Jul 01. [Online ahead of print]TC
- CONCLUSIONS: Prophylactic retro-muscular mesh placement in a Sugarbaker configuration, achieved by combining single-port laparoscopic separation and an extraperitoneal route for colostomy, appears technically safe and feasible. The approach holds theoretical promise and likely results in a reduction in the incidence of PSH.

- Single-stapled versus non-single-stapled colorectal anastomosis in minimally invasive anterior resections: a systematic review and meta-analysis. [Review]Tech Coloproctol. 2026 Jun 29. [Online ahead of print]TC
- CONCLUSIONS: Single-stapled anastomosis in minimally invasive anterior resection is associated with a lower pooled risk of anastomotic leak compared with non-single-stapled techniques. Despite heterogeneity in secondary outcomes, SST appears to be a promising technical refinement; however, randomised trials are required before routine adoption.
