- Calcitonin Gene-Related Peptide (CGRP) as a Potential Therapeutic Target for Discogenic Back Pain. [Journal Article]Spine J. 2026 Oct 28. [Online ahead of print]SJ
- CONCLUSIONS: CGRP induces inflammatory and neurotrophic signaling in human AF cells, attenuated by molecules that block CGRP.

- The Gut-Spine Axis in Lumbar Intervertebral Disc Degeneration: Microbial and Inflammatory Signatures. [Journal Article]Spine J. 2026 Oct 25. [Online ahead of print]SJ
- CONCLUSIONS: This study provides preliminary evidence for a potential functional gut-spine axis in symptomatic lumbar IDD. We show that IDD is associated with a pro-inflammatory gut dysbiosis that correlates with systemic inflammation. Furthermore, the identification of a unique, pathobiont-enriched microbiome within the degenerated disc challenges the view of IDD as a sterile condition. These findings suggest that IDD may have a systemic component, and point to microbiome-targeted diagnostics and therapeutics as potential avenues for future intervention.

- Outcomes Analysis of Low Back Pain Patients Managed Through a Spine Center Clinical Care Redesign Pathway: An Initial Retrospective Cohort Study. [Journal Article]Spine J. 2026 Oct 24. [Online ahead of print]SJ
- CONCLUSIONS: A clinical care redesign spine pathway may be associated with improved care efficiency and decreased use of selected services such as imaging and outside specialist visits. Additional studies with larger cohorts and multivariable analysis are warranted to further characterize the relationship between integrated spine center models and functional outcomes.

- Objective Functional Phenotyping in Spine Surgery: A Translational Framework for Quantifying Disability, Guiding Clinical Decision-Making, and Monitoring Recovery. [Review]Spine J. 2026 Oct 23. [Online ahead of print]SJ
- Imaging, physical examination, and patient-reported outcome measures (PROMs) remain central to spine care but incompletely characterize how spinal disease affects physical performance and how function changes following treatment. Objective assessment of gait, balance, functional transitions, lifting, and neuromuscular control provides complementary information regarding movement impairment, compe…

- Predictors of Age-Adjusted Pelvic Tilt Correction After Long-Segment Adult Spinal Deformity Correction. [Journal Article]Spine J. 2026 Oct 23. [Online ahead of print]SJ
- CONCLUSIONS: PT correction depends on both baseline alignment and extent of surgical correction. Greater correction of LL and T1PA proportional to PI, were independently associated with PT normalization.

- Long-term cervical sagittal balance after single-level anterior cervical discectomy with or without fusion or arthroplasty: sex-specific changes in cervical alignment over 5 years. [Journal Article]Spine J. 2026 Oct 23. [Online ahead of print]SJ
- CONCLUSIONS: Single-level ACD with or without fusion or arthroplasty is associated with long-term increases in cervical lordosis with sex-specific compensatory patterns. Alignment-outcome cross-sectional associations were limited to early follow-up, suggesting that restoring adequate segmental lordosis at surgery may reduce early postoperative arm pain.

- Rod Geometry and Material Characteristics as Risk Factors for Rod Breakage After Adult Spinal Deformity Surgery: A Multicenter Cohort Study with Five-Year Follow-up. [Journal Article]Spine J. 2026 Oct 23. [Online ahead of print]SJ
- Background Context Rod breakage is a frequent and clinically important late complication after adult spine deformity (ASD) surgery, yet most research has focused on patient and alignment factors rather than implant geometry. Purpose This study aimed to investigate the rod-related characteristics independently associated with rod breakage following ASD surgery, with particular focus on quantitativ…

- Clinical Outcomes and Factors Associated with Radiological Resorption in Symptomatic Extruded or Sequestrated Lumbar Disc Herniation: A Retrospective Cohort Study. [Journal Article]Spine J. 2026 Oct 22. [Online ahead of print]SJ
- CONCLUSIONS: Resorption was associated with a more favorable clinical course, but improvement also occurred without resorption. Komori type III and Modic type I were associated with earlier documented resorption; resorption magnitude showed no consistent outcome association.

- Outcomes after Posterior Cervical Spine Surgery: A Matched Cohort Analysis and Comparison of GLP-1 Receptor Agonist Use and Bariatric Surgery in Patients with Type 2 Diabetes. [Journal Article]Spine J. 2026 Oct 21. [Online ahead of print]SJ
- CONCLUSIONS: Preoperative GLP-1 RA use was associated with reduced perioperative, short-term, and long-term complications following posterior cervical spine surgery in patients with type 2 diabetes. When compared with a matched bariatric surgery cohort, no significant differences in postoperative outcomes were observed. These findings should be interpreted in the context of potential residual confounding and support further prospective investigation of GLP-1 therapy as a strategy for preoperative metabolic optimization in posterior cervical spine surgery.

- Association Between the Bone-cutting Width-to-facet Distance Ratio and Neurological Recovery After Cervical Laminoplasty. [Journal Article]Spine J. 2026 Oct 21. [Online ahead of print]SJ
- CONCLUSIONS: Within the range of decompression achieved in this cohort, relative bone-cutting width was not associated with neurological recovery. Wider cutting enlarged the canal but did not improve JOA recovery, and dural contact was not associated with worse recovery. Although these findings do not establish equivalence between narrower and wider decompression, they suggest that routinely maximizing bone-cutting width may offer limited additional neurological benefit.

- The rapid expansion of minimally invasive, endoscopic, percutaneous, and implant-based spine interventions has outpaced standardized training requirements. This statement provides recommendations based on current training pathways, contemporary practice, and expert consensus, with patient safety as the guiding principle. These recommendations are not intended to define or restrict scope of practi…

- Association of Spinal Epidural Lipomatosis With Systemic Lipid Metabolism in Patients With Lumbar Spinal Stenosis: A Propensity Score-Matched Study. [Journal Article]Spine J. 2026 Oct 20. [Online ahead of print]SJ
- CONCLUSIONS: In patients undergoing surgery for LSS, SEL was associated with a more atherogenic lipid profile after matching for BMI and other major baseline characteristics, as well as greater preoperative disability and leg pain. The association of ApoB with SEL, together with the graded relationships of ApoB and triglycerides with epidural fat burden, suggests that SEL may reflect systemic lipid metabolic dysfunction rather than local epidural fat deposition alone.

- Postembolization CT Digital Subtraction Angiography for Assessing Spinal Tumor Devascularization: Concordance With Catheter Angiography and Association With Surgical Blood Loss. [Journal Article]Spine J. 2026 Oct 19. [Online ahead of print]SJ
- CONCLUSIONS: Postembolization CT-DSA showed moderate concordance with catheter angiography and may complement selective angiographic assessment by depicting residual enhancement throughout the tumor. Partial devascularization showed a directionally higher but imprecise association with surgical blood loss in metastatic tumors and requires further validation.
