These preoperative parameters are helpful in identifying high-risk patients with poor postoperative symptom improvement and provide references for individualized clinical decision-making.

- CONCLUSIONS: Although ID-T2DM represents a relatively small subgroup, its rates increased over time. ID-T2DM was associated with higher rates of blood transfusions, genitourinary complications, and increased THC after NU. These findings suggest that ID-T2DM patients undergoing NU may represent a potential target population for perioperative optimization to reduce surgical morbidity and resource utilization.

It highlights that an aggressive multimodal strategy - integrating radical orchiectomy, RPLND for pathologic staging and potential therapeutic benefit, and adjuvant radiotherapy when chemotherapy is declined - can be curative. This long-term survival outcome provides a management framework for this orphan disease, though validation in larger cohorts is needed.
