  • CONCLUSIONS: Anti-CD36 alloimmunisation should be considered in severe PTR with negative HLA antibody screening, particularly in heavily transfused patients from admixed populations. To our knowledge, this is the first Brazilian case of anti-CD36 alloimmunisation causing severe PTR; platelet CD36 deficiency has previously been reported in a Brazilian woman with anti-CD36 antibodies in the context of pregnancy, underscoring the importance of considering this antibody specificity in persistent PTR despite negative HLA and HPA antibody screening.

  • CONCLUSIONS: Long-term leukocyte-reduced PLT apheresis is associated with decreased circulating NK cells. Multiple-testing correction provided no evidence for progressive lymphocyte decline with cumulative donations. One-year donation frequency had no impact on lymphocyte subsets. With plasma rinseback activated for all Trima procedures, lymphocyte profiles were comparable between devices. Routine hematological parameters remained stable in long-term apheresis donors.

  • CONCLUSIONS: ROTEM-guided management was associated with lower plasma utilization, higher cryoprecipitate utilization, and shorter ICU stay. Given the retrospective, non-randomized design and low RBC exposure, these findings should not be interpreted as evidence that ROTEM reduced bleeding severity or RBC requirements, particularly in massive-transfusion populations. Prospective studies in trauma patients with massive hemorrhage are required to evaluate the utility of ROTEM in guiding specific hemostatic blood-product transfusions.

  • CONCLUSIONS: Surveyed institutions in the United States and Canada primarily use cryoprecipitate for fibrinogen replacement, with variable usage of FC and limited usage of PR cryoprecipitate. The perception of FC being more expensive than cryoprecipitate predominated. Further studies comparing available fibrinogen replacement options would be valuable, particularly with consideration of product acquisition cost, wastage, impacts on utilization of other blood products, and patient outcomes.
