A first section describes how methodological developments have positively influenced the transition of PET from a research tool towards a clinical diagnostic test. In a second section, evidence in support of its superior diagnostic accuracy, its value to guide decision making and to predict outcome, and its cost-effectiveness is summarized. A third section finally outlines new PET-based approaches and concepts which will likely influence clinical cardiovascular medicine in the future.
