- CONCLUSIONS: AACC following pharmacologic dilation was rare and occurred predominantly among patients with anatomical risk factors, supporting the safety of dilation in routine clinical care. Medication-associated risk signals were observed across commonly prescribed drugs at the population level, including previously unreported associations, highlighting the importance of proactive ocular risk assessment and medication reconciliation in anatomically predisposed patients. Novel medication associations with calcium channel blockers, CGRP monoclonal antibodies, and PCSK9 inhibitors warrant further investigation.

- CONCLUSIONS: Glaucoma is best conceptualised as a progressive optic neuropathy, with intraocular pressure a modifiable risk factor. Acute angle-closure glaucoma requires same-day referral, while most open-angle disease is asymptomatic and detected through opportunistic screening. Risk-based referral thresholds and medication considerations are outlined. Timely referral, reinforcement of treatment adherence and shared care with optometry and ophthalmology are critical to preventing avoidable vision loss in general practice.

- CONCLUSIONS: This case provides the first acute-phase histopathological evidence that characteristic scleral abnormalities in nanophthalmos persist during IOP elevation >60 mmHg, supporting their congenital etiology. Combined sclerostomy and cataract surgery successfully treated refractory nanophthalmos-associated glaucoma in the acute setting, and bilateral findings provide valuable comparative data supporting prophylactic surgical intervention in the fellow eye.
