- Dissolution Rate and Stability Improvement of Hydrochlorothiazide and Valsartan by Co-Amorphous Formulation. [Journal Article]
- The current study aimed to improve the dissolution rate and physical stability of two antihypertensive drugs, hydrochlorothiazide, and valsartan, through the development of a co-amorphous system. A co-amorphous system was successfully prepared at molar ratios of 1:1, 1:2, and 1:6 employing rotary evaporation technique. Characterization of the formulations was performed using powder X-ray diffract…

- Oral and Topical Spironolactone in Adult Female Acne: A Narrative Review Comparing Systemic and Local Approaches. [Review]
- CONCLUSIONS: Oral spironolactone is supported by robust evidence as an effective non-antibiotic option for AFA and is suitable for moderate-to-severe disease, whereas topical spironolactone appears promising but remains supported by limited and partly indirect evidence, derived largely from small and heterogeneous studies, and may be appropriate for mild-to-moderate acne or when systemic therapy is contraindicated. Because no head-to-head trial has compared the two routes, the comparison presented here is necessarily indirect. Future research should prioritize head-to-head comparisons of both routes and standardization of topical formulations.

- 'My HS is significantly impacted by my cycle': a cross-sectional study exploring the impact of menstruation and pregnancy on hidradenitis suppurativa. [Journal Article]Skin Health Dis. 2026 Oct; 6(5):749-753.SH
- CONCLUSIONS: This study demonstrates a strong association between hormonal fluctuations and HS symptom deterioration. Clinicians should proactively assess menstrual patterns in HS management and provide support during the pregnancy and postpartum periods. Coordinated multidisciplinary care is essential. Further research is needed to evaluate antiandrogenic therapies for hormonally triggered HS exacerbations.

- Batch-to-Flow Translation of β-Cyclodextrin Polymer Adsorption for Emerging Contaminant Removal: Hydrodynamic and Operational Validation. [Journal Article]
- Translating adsorption performance from batch experiments to continuous-flow operation is a key step toward practical water-treatment applications. In this study, a laboratory-scale continuous adsorption system based on a water-insoluble β-cyclodextrin-epichlorohydrin (β-CD-EPI) polymer was validated from hydrodynamic, adsorptive, and operational perspectives. Downflow operation caused progressiv…

- Late diagnosis of Gitelman syndrome in an elderly patient: identification of a novel SLC12A3 pathogenic variant. [Case Reports]
- Gitelman syndrome (GS) is an inherited salt-wasting, hypokalemic tubulopathy caused mainly by biallelic loss-of-function variants in the SLC12A3 gene. Despite being classically diagnosed in adolescence, delayed recognition may occur. We present the case of a 74-year-old man with a history of hypokalemia, in whom the diagnostic evaluation revealed hypokalemia, hypomagnesemia, metabolic alkalosis, …

- Seasonal variation in the office and ambulatory blood pressure control in patients treated with two dual antihypertensive therapies. [Journal Article]
- We investigated seasonal variation in the office and ambulatory blood pressure control in hypertensive patients treated with two single-pill dual-combination antihypertensive therapies. The study participants (n = 560) were hypertensive patients enrolled in a 24-week therapeutic study. Antihypertensive treatment was initiated with amlodipine/benazepril 5/10 mg/day or benazepril/hydrochlorothiazid…

- Thiazide Diuretics Are Associated with a Higher Triglyceride-Glucose Index Than Calcium Channel Blockers in Hypertensive Patients. [Journal Article]
- CONCLUSIONS: RAAS blocker plus thiazide/thiazide-like diuretic regimens were associated with a significantly higher TyG index and a less favorable metabolic profile than RAAS blocker plus CCB regimens, independent of multiple clinical confounders. Metabolic risk profiles should be considered when selecting antihypertensive combination therapy; prospective studies are needed to establish causality.

- A SHAP-Informed Formal Feature Attribution Framework for Drug-Drug Interaction Risk in Large-Scale Claims Data. [Journal Article]
- Pairwise drug-drug interaction databases flag co-prescribed pairs, but they under-weight multi-drug combinations that drive adverse drug events in older adults. We studied that gap in Virginia All-Payer Claims Database records (2016-2019) for adults aged 65-114 years with non-opioid emergency department visits (n = 1182 cases; 16,105 matched controls across three geriatric age bands). - Comparative Effectiveness of Chlorthalidone Versus Hydrochlorothiazide and Dose-Response of Amiloride on Blood Pressure and Biochemical Parameters. [Journal Article]J Am Heart Assoc. 2026 Aug 30; :e047583. [Online ahead of print]JA
- CONCLUSIONS: Chlorthalidone and hydrochlorothiazide, administered at a 1:2 dosing ratio, show similar effects on BP and laboratory parameters. Amiloride 5 and 10 mg show similar effects on BP and most metabolic parameters, with no clear dose-response relationship, suggesting that any independent BP-lowering effect of amiloride is, at best, weak.

- [Primary aldosteronism in resistant hypertension unmasked by a thiazide-induced severe hypokalaemia]. [Case Reports]
- Primary aldosteronism is the most common cause of secondary hypertension but remains frequently underdiagnosed in primary care. We report a 71-year-old man with resistant hypertension and low-normal serum potassium in whom a minimal increase in hydrochlorothiazide dosage (12.5 to 25 mg) precipitated severe hypokalaemia (1.6 mmol/L), metabolic alkalosis, and rhabdomyolysis. An office electrocardio…

- Non-alcoholic Wernicke Encephalopathy With Atypical Cortical Involvement on MRI: A Case Report. [Case Reports]
- While Wernicke encephalopathy (WE) is well-recognized as a condition most frequently associated with chronic excessive alcohol consumption, non-alcoholic WE (NAWE) remain underrecognized despite their clinical importance. An 85-year-old nursing home resident was brought to the Emergency Department (ED) with impaired consciousness. She had been taking furosemide 40 mg and spironolactone 25 mg dail…

- Polypill for heart failure with reduced ejection fraction: the POLY-HF randomized trial. [Randomized Controlled Trial]
- Heart failure with reduced ejection fraction carries a poor prognosis. Although guideline-directed medical therapy reduces morbidity and mortality, its real-world utilization is low. Accordingly, we conducted an open-label randomized trial (POLY-HF) at two centers enrolling a predominantly underserved population to test whether a polypill strategy improves cardiac function in heart failure. - Looking beyond Sorption in GAC Filters: How Extended Contact Times and Functionally Distinct Microbial Biomass Enable Enhanced Micropollutant Biodegradation. [Journal Article]Environ Sci Technol. 2026 Jul 14; 60(27):19680-19692.ES
- Micropollutant abatement in granular activated carbon (GAC) filters is governed by both sorption and biodegradation processes. Yet a comprehensive understanding of which micropollutants are biodegraded, how much biodegradation contributes to GAC's performance, and how GAC's properties affect biodegradation, remains limited. This study addresses these knowledge gaps by combining reactive transport…
