This study evaluated the effects of Bacillus-based direct-fed microbial (DFM) supplementation on in situ nutrient disappearance and degradability of high-concentrate diets. Fifteen rumen-cannulated cows were used in a completely randomized design. Treatments consisted of a forage-based diet plus 500 g of protein-mineral (390 g dried distillers grain + 110 g vitamin-mineral mix; as-fed basis) supplement mixed with 1) 3 g (as-fed basis) of a Bacillus-based DFM containing Bacillus licheniformis 809 and B. subtilis 810 (BAC; n = 7; 2.2 × 10[9] CFU of the mixture/g; Bovacillus, Novonesis, Lyngby, Denmark), or 2) without BAC (CON; n = 8). Samples of 4 high-concentrate, byproduct-based diets (A, B, C, and D) were collected from a commercial feedlot in South Africa. These diets were composed of hominy chop, corn silage, Eragrostis hay, cottonseed cake, molasses syrup, urea, limestone, salt, and a mineral-vitamin premix, and subsequently dried for analysis. From days 28 to 32, each diet was placed in Dacron bags and introduced through the cannulas. Residuals were dried, and DM, NDF, and ADF content were determined. No treatment × hour interactions were detected (P ≥ 0.11) for nutrient disappearance in any of the diets. Dry matter and NDF disappearance were greater (P ≤ 0.05) in diets A, C, and D for cows fed BAC compared with CON, whereas no differences were observed in nutrient disappearance from diet B (P ≥ 0.33). Similarly, ADF disappearance was greater in diet A (P = 0.04) and tended to be greater in diet C (P = 0.07) in BAC vs. CON and was unaffected by treatment in diets B and D (P ≥ 0.13). Combining all four diets, degradation of fraction B (degradable fraction; total pool disappearing at a measurable rate) of DM was greater for BAC vs. CON (P = 0.02), whereas degradation of fraction C (undegradable fraction; total unavailable pool in the rumen) tended to be lower for BAC than CON (P = 0.08). Evaluating NDF degradation across diets, the degradation of fraction B tended to be greater for BAC vs. CON (P = 0.10), whereas effective degradability was greater for BAC compared with CON (P = 0.02). No treatment effects were observed on NDF fraction C (P = 0.18) or degradation rates (% per h; P = 0.92) between treatments. Collectively, feeding a Bacillus-based DFM improved in situ disappearance and the potentially degradable nutrient pool of a high-concentrate byproduct diet in forage-fed beef cows.

The adoption of cost-effective feeding regimes is essential to maintain the growth and physiological status of lambs after weaning. The economic benefits of an alternative concentrate diet for Omani lambs were also assessed. Twenty male Omani lambs (12.31 ± 3.22 kg; 5 months old) were allocated to a commercial concentrate (control) or formulated concentrate group (n = 10/treatment) for 63 days. The formulated concentrate consisted of date palm fronds (29%), barley (20%), fish meal (12%), date syrup (10%), Moringa oleifera (28%), and salt (1%), with all ingredient proportions expressed on a dry matter (DM) basis. Growth performance, feed efficiency, hematological indices, serum biochemical parameters, and cost analyses were evaluated using individual lambs as the experimental unit. Growth performance, feed efficiency, hematological and serum biochemical parameters, and cost analyses were performed. Final body weight (BW), total weight gain (TWG), average daily gain (ADG), and feed conversion ratio (FCR) were similar between the groups (p > 0.05). The average daily feed intake was similar between groups (p = 0.321). However, the feed cost per kg of gain and feeding cost were lower (p < 0.001) by 63 and 58%, respectively, for the formulated concentrate group. Time × group interactions were observed for red blood cell parameters, with higher platelet counts (p < 0.001) and reticulocyte hemoglobin content (p = 0.018) in the formulated concentrate group. The serum biochemical parameters changed over time, with similar responses between the groups. Appropriate substitution of ingredients in lamb concentrate diets with agricultural by-products positively influenced economic efficiency.

Low- and no-calorie sweeteners (LNCS) (also referred to as non-nutritive sweeteners) have been the subject of controversy, especially considering recent conditional recommendations by the World Health Organization (WHO) not to use LNCS as a means of controlling weight and disease risk reduction. We summarize the data presented and discussions held between a panel of international nutrition research experts at a high-level Canadian Nutrition Society Thematic conference on "Non-nutritive sweeteners and health: unravelling the evidence and controversy", which took place in Toronto, Canada on 27 January 2024. This paper summarizes the proceedings, including discussions on global sugar reduction policies, the toxicological safety and public perceptions of LNCS, divergence between public health and clinical practice guidance, and potential biological mechanisms underlying their effects. Randomized controlled trials show benefits of LNCS when substituting for sugars, while long-term cohort data have methodological limitations such as reverse causation and residual confounding. Mechanistic evidence does not support major adverse effects on energy intake regulation, glycemia, or gut microbiota. Together, these findings highlight the need for harmonized global policies, rigorous study designs that model real-world substitutions, and improved communication strategies to address misinformation.

Neither the excretion of guaifenesin in milk nor its effect on breastfed infants have been studied. It is unlikely that with usual maternal doses amounts in breastmilk would harm the nursing infant, especially in infants over 2 months of age. It is best to avoid the use of products with a high alcohol content while nursing.

Serotonin syndrome (SS) is a potentially life-threatening condition caused by excessive serotonergic activity in the central nervous system. It is commonly linked to combinations of serotonergic prescription drugs; however, over-the-counter or prescription cold medications containing dextromethorphan (DM) or first-generation antihistamines may also pose a risk. We report a case of a 52-year-old male on sertraline who developed SS after taking Bromfed DM, a cough syrup containing brompheniramine, pseudoephedrine, and DM. Three days after starting the medication, he experienced confusion, dilated pupils, myoclonic foot movements, and difficulty with speech. Symptoms improved following the discontinuation of both sertraline and Bromfed DM. This case underscores the importance of recognizing the serotonergic potential of common cold medications and the risk of SS even at therapeutic doses, particularly in patients on selective serotonin reuptake inhibitors (SSRIs). Prompt identification and cessation of the causative agents are critical to avoid complications.

This study aimed to explore current opioid availability and accessibility for palliative care (PC) practice in Nepal. A narrative review was conducted by performing literature searches in electronic databases, grey literature, policy documents, and pharmaceutical websites. This was supplemented by utilising the authors' expertise and experience in this field. Six different opioids are available in Nepal, including oral immediate and modified-release morphine formulations. Morphine is produced and distributed by only one manufacturer, which imports all raw materials from Europe. Access to morphine in rural areas is poor, with rural-level healthcare centres rarely stocking morphine, particularly in PC formulations. Fentanyl transdermal (TD) patch and methadone syrup are available in Nepal but are imported. The Fentanyl TD patch is extremely expensive, and methadone syrup is only available for the management of addiction. While opioid availability has increased in Nepal, there is a need to improve opioid accessibility for PC services, particularly in rural areas. Several approaches are explored in this review.

Diabetes Mellitus (DM), one of the oldest known metabolic disorders, dates back to 3000 BC and continues to have a profound impact on health and the economy. Nutrition plays a critical role in managing diabetes and enhancing overall quality of life. It is also vital for immune system function, as well as in the prevention and treatment of aging-related diseases. A key factor contributing to the global rise in obesity is the excessive consumption of fructose/glucose (corn) syrup, which leads to various metabolic complications. Uncontrolled intake of carbohydrates, particularly sugars like fructose, triggers the Maillard Reaction, a chemical process that occurs between sugars and proteins, resulting in advanced glycation end-products (AGEs). This process is accelerated in diabetic patients due to hyperglycemia, leading to increased glycation of plasma proteins such as immunoglobulins, which play an essential role in the immune system. Studies show that individuals with Diabetes Mellitus experience a higher susceptibility to infections due to increased viral entry, impaired immune responses, reduced viral clearance, and dysregulated inflammatory cytokine production. In this study, human IgG proteins were glycated in vitro using fructose, simulating the damaging effects seen in diabetic conditions. A mixture containing antioxidants like glutathione, oleuropein, and selenium was prepared and incubated with the glycated IgG to assess its protective properties. Lymphocyte cells from healthy volunteers were also treated with fructose and subjected to similar experiments. Results demonstrated that fructose significantly compromises immune function by damaging key proteins, but the antioxidant mixture effectively mitigates this damage, offering a protective mechanism against glycation in the immune system.

The preparation and application of the composite material "crosslinked polyvinyl alcohol-magnetite" as a sensitive matrix for use in digital colorimetry and optical micrometry methods are discussed. The material was synthesized in the form of spherical granules (for micrometry) and thin films (for digital colorimetry). The obtained composites were characterized by the registration of magnetization curves. It was shown that the amount of grown Fe3O4 particles in the polymer gel is in linear dependence with the iron salt concentrations in the impregnating solutions. The composite granules were applied to determining monosaccharides using optical micrometry. The optimal pH value for the total amount of monosaccharides' determination was 8.6. The study of the analytical response of composite granules and films performed with a low limit of detection (7.9 mmol/dm[3]) of both glucose and fructose and a possibility of the control of high alcohol contention in water media. The granules were used to determine the total carbohydrate content in samples of natural honey and syrups with high fructose contents, while the films were used to control the alcohol content in hand antiseptics. The results obtained are in good agreement with the data provided by the manufacturers.

The formation of mine-contaminated groundwater as a result of acidic mine drainage from the oxidation of sulfur-containing minerals entering the groundwater. Biological permeable reactive barrier (Bio-PRB) technology is excellent for the remediation of mine-contaminated groundwater. Usually, the organic substrates utilized in Bio-PRB are a combination of rapid initiators, which are readily bioavailable, and long-lasting nutrients, which are more difficult to degrade. Herein, we investigated the effectiveness of three rapid initiators and three long-lasting nutrients to remove sulfate from simulated mine-contaminated groundwater via simulated column experiments. The rapid initiators comprised crude glycerol, sodium acetate, and industrial syrup (IS), and the long-lasting nutrients included biodiesel emulsified oil, soybean oil emulsified oil, and high-carbon alcohol emulsified oil (HO). Microorganisms were stimulated using IS to create a sulfate reduction system owing to its high total organic carbon content (24.30 g L[-1]), achieving optimal sulfate removal rate (1.69 mmol dm[-3] d[-1]). The fastest (2.93 mmol dm[-3] d-[1]) and highest (88%) sulfate removal rates were achieved using HO, which is probably associated with the ability of HO to provide the most suitable C/N ratio (111.75) and induce the growth of sulfate-reducing bacteria (SRB) for substrate degradation. Conversely, a high concentration of sulfate reduction products inhibited SRB growth in the HO column. The addition of organic materials promoted SRB growth and various organic substrate-degrading bacteria. Furthermore, the competitive growth of methanogens (86.6%) may be responsible for the decrease in the relative abundance of SRB during the later stages of the experiment in the HO column.

The main purpose of this work is the development of a value-added product (candied chestnuts) from Portuguese chestnut (Castanea sativa) cultivars (Martainha, Longal and Judia), as a way to minimize product loss and wastes. To accomplish this goal, the effects of rehydration, cooking, and syrup conditions on composition, textural properties, and colour parameters of candied chestnuts were investigated. The obtained results revealed that the optimal conditions to prepare candied chestnuts with a sweet taste, dark brown colour, with a crispy texture on the outside and smooth texture in the inner flesh were rehydration at 45 °C for a period of 5 h, cooking in a pressure pan for 15 min, and an immersion process with sucrose syrup for two days (syrup with 25% of sucrose on the first day and syrup of 50% of sucrose on the second day). During the process, the drying loss, hydration ratio, and cooking gain of the different cultivars were about 90%, 79%, and 130%, respectively. The total colour difference of candied chestnuts ranged from 24.18 (Longal) to 29.95 (Judia), the stickiness was moderately intense, and the adhesiveness was high for the three varieties. Longal candied chestnuts were the softest and Martainha candied chestnuts were the hardest, the most elastic, and cohesive. Moreover, the candied chestnuts presented a moisture content ranging from 52.70% and 54.23%, amounts of carbohydrates in the range of 88.58 to 91.87 g/100 g d.m, values of protein (6.55-9.51 g/100 g d.m.), values of ash (0.78-1.98 g/100 g d.m.), and fat (0.87-1.58 g/100 g d.m.). In conclusion, the chestnuts of Portuguese cultivars Martainha, Longal and Judia reveal a good potential to produce candied products with high added value.

A 62-year-old female presented to the emergency department (ED) with fatigue and generalized body weakness for the last three days. Upon arrival, initial ECG showed wide complex tachycardia with sine waves and a heart rate (HR) ranging between 100-170 bpm. She was otherwise vitally stable. The patient had a past medical history of hyperaldosteronism, type 2 diabetes mellitus (DM), chronic kidney disease (CKD) with microalbuminuria, and hypertension. She also had a history of cerebrovascular accident (CVA) and residual left-sided weakness more pronounced in the upper limb. Initial venous blood gas (VBG) analysis showed a potassium level of more than 10 mmol/L, chloride 114 mmol/L, bicarbonate 9 mmol/L, sodium 135 mmol/L, and pH of 7.1. Treatment for hyperkalemia was started immediately with calcium gluconate 1 gm that effectively narrowed her QRS complex and normalized her ECG. Salbutamol nebulization, glucose/insulin infusion, and calcium polystyrene syrup were given. Later, she was started on 100 mg sodium bicarbonate infusion, and Foley's catheter was inserted to follow urine output (UOP) strictly. However, she did not show a decrease in serum potassium levels. Then the patient underwent hemodialysis for two hours. Her first potassium reading after hemodialysis was 5.2 mmol/L. The purpose of this case report is to emphasize the importance of hemodialysis in patients with persistent severe life-threatening hyperkalemia.

The incidence of diabetes mellitus (DM) is increasing rapidly and is associated with changes in dietary habits. Although restrictions in the use of sweeteners may prevent the development of DM, this might reduce the quality of life of patients with DM. Therefore, there has been a great deal of research into alternative sweeteners. In the search for such sweeteners, we analyzed the carbohydrate content of maple syrup and identified a novel oligosaccharide composed of fructose and glucose, linked at the C-4 of glucose and the C-6 of fructose. This oligosaccharide inhibited the release of fructose from sucrose by invertase (IC50: 1.17 mmol/L) and the decomposition of maltose by α-(1-4) glucosidase (IC50: 1.72 mmol/L). In addition, when orally administered together with sucrose to rats with DM, the subsequent plasma glucose concentrations were significantly lower than if the rats had been administered sucrose alone, without having any effect on the insulin concentration. These findings suggest that this novel oligosaccharide might represent a useful alternative sweetener for inclusion in the diet of patients with DM and may also have therapeutic benefits.

Hyperglycemia results in the overproduction of free oxygen radicals that impair the endogenous antioxidant defenses. A randomized controlled trial was conducted to compare the effect of 3 months of antioxidant supplementation in the form of foods rich in micronutrients with pharmacological supplement on total antioxidant status of Indian children with type 1 diabetes. Ninety children with diabetes (mean age 11.5 ± 3.6 yrs, 37 boys) were randomly allocated to three groups: Group 1 (n = 31) = DM controls; Group 2 (n = 30) = multimicronutrient syrup; and Group 3 (n = 29) = dietary supplements (nine snack recipes rich in micronutrients). They received intervention for 3 months. Healthy controls were enrolled from local schools. Fasting blood was tested for total antioxidant status (TAS) and glycosylated hemoglobin (HbA1C). Children with diabetes had lower TAS (0.70 ± 0.2 vs. 1 ± 0.24 mmol/l, p = .0001) compared to healthy controls. Anthropometric and biochemical parameters were similar at baseline for all groups of diabetic children. Group 1 showed significant deterioration in TAS at endline (0.72 ± 0.16 vs. 0.60 ± 0.17 mmol/l, p = .008). Increase in TAS recorded in Group 2 was from 0.66 ± 0.21 to 0.70 ± 0.16 mmol/l and in Group 3 was from 0.68 to 0.73 mmol/l. There was a significant difference between Group 1 and Group 3 for percentage change in TAS (-13% vs. 16%, p = .035). Postsupplementation there was an increase in TAS values in children with diabetes, but they were still lower than in healthy controls. Indian diabetic children have compromised antioxidant status, which may be improved by incorporation of multimicronutrient-rich recipes in their diets.
