- Microsurgical clipping for coil-extruded recurrent aneurysms: Case series and technical pearls. [Journal Article]Surg Neurol Int. 2026; 17:518.SN
- CONCLUSIONS: Coil extrusion is an uncommon but complex manifestation of aneurysm recurrence that may occur even decades after initial endovascular treatment and can affect a range of anterior circulation locations. This case series underscores the importance of lifelong imaging surveillance and demonstrates that, in carefully selected patients, microsurgical clipping remains a safe and definitive salvage strategy when guided by refined technique and contemporary intraoperative technologies.

- Single-stage hybrid management of severe carotid stenosis and unruptured intracranial aneurysms using direct transcarotid access: Illustrative case. [Case Reports]Surg Neurol Int. 2026; 17:504.SN
- CONCLUSIONS: This illustrative case demonstrates that individualized single-stage hybrid management using direct carotid access, endovascular aneurysm treatment, and CEA may represent a feasible option in highly selected patients when conventional staged strategies or transfemoral access are technically unfavorable. Careful multidisciplinary planning and tailored treatment sequencing are essential in these complex neurovascular scenarios.

- Closer matching between flow diverter diameter and parent artery diameter is associated with improved aneurysm occlusion. [Journal Article]Surg Neurol Int. 2026; 17:496.SN
- CONCLUSIONS: Greater FD elongation post-deployment may be linked to lower aneurysm occlusion rates at 6 months. Choosing an FD size that matches the parent vessel diameter is crucial for optimal occlusion.

- Predictors of complete occlusion in endovascular coiling of very small intracranial aneurysms. [Journal Article]Neuroradiology. 2026 Oct 03. [Online ahead of print]N
- CONCLUSIONS: Endovascular coiling of VSAs provides high occlusion and excellent clinical outcomes with minimal morbidity. Anatomical complexity (ultra-small size, posterior circulation) and incomplete initial packing remain targets for technical optimization and prospective evaluation.

- Aneurysmal subarachnoid hemorrhage: predictive score for angiographic vasospasm. [Journal Article]Arq Neuropsiquiatr. 2026 Oct; 84(10):1-10.AN
- CONCLUSIONS: We identified significant predictors of vasospasm in patients with aneurysmal subarachnoid hemorrhage, namely interpeduncular HU value, intracerebral hemorrhage, and serum chloride levels. A practical 5-point score based on these factors offers a potentially-robust, easy-to-use tool for early risk stratification, which may guide personalized follow-up and timely interventions. Further prospective multicenter studies are recommended to validate these findings.

- Development of a prediction model for aneurysmal events to guide imaging surveillance in autosomal dominant polycystic kidney disease. [Journal Article]J Neurosurg. 2026 Oct 02; :1-7. [Online ahead of print]JN
- CONCLUSIONS: The authors developed an internally validated, easily applicable risk score for predicting aneurysm-related events in ADPKD patients. Generalization requires caution because of potential selection bias, and external validation is warranted.

- HSF-Net: a hybrid-scale fusion network for intracranial aneurysm segmentation in CTA images. [Journal Article]
- Rupture of intracranial aneurysms is a major cause of subarachnoid hemorrhage, making accurate segmentation important for aneurysm assessment and treatment planning. However, automated segmentation in computed tomography angiography (CTA) remains challenging because of small lesion size, complex morphology, and ambiguous boundaries between aneurysms and adjacent vessels. To address these challeng…

- Occult Bartonella Quintana Infective Endocarditis Incidentally Diagnosed During Pre-Transcatheter Aortic Valve Implantation (TAVI) Workup in a Patient Without Classical Risk Factors. [Case Reports]
- Bartonella infective endocarditis (IE) is a rare cause of initially blood culture-negative IE, classically associated with cat flea exposure in Bartonella henselae infection or body lice infestation among individuals experiencing homelessness or alcoholism in Bartonella quintana infection. We report the case of an 84-year-old man with severe calcific aortic stenosis who was undergoing evaluation …

- Perioperative Antiplatelet Therapy in the New Flow Diversion Era. [Review]
- Flow diverter (FD) technology has provided a new concept for the treatment of intracranial aneurysms: reconstruction of the parent artery by covering the neck with a braided mesh stent. It has enabled us to treat not only saccular aneurysms but also complex aneurysms that were previously considered difficult or impossible to treat. Despite its clinical impact, FD remains associated with thromboem…

- Delayed intracranial lesions following cardiac myxoma resection in a patient with concurrent cerebral embolism: a diagnostic challenge in the absence of pathological evidence. [Case Reports]
- CONCLUSIONS: This case highlights the diagnostic challenges in differentiating intracranial lesions following cardiac myxoma resection and underscores the importance of long-term cranial imaging surveillance. Even after complete tumor resection, clinicians should remain vigilant for potential myxoma-related complications.

- Flow diversion versus conventional endovascular treatment for ruptured basilar artery dissecting aneurysms: a single-center cohort study. [Journal Article]
- CONCLUSIONS: Primary FD-based reconstruction was feasible in selected ruptured BADA, particularly in anatomically complex or fusiform lesions. Given the small sample, non-randomized treatment selection, and absence of statistically significant between-group differences, these findings support individualized treatment and do not establish superiority of either strategy.

- Inflammation, coagulation, and nutritional reserves refine 90-day prognostic stratification after ruptured intracranial aneurysm repair. [Journal Article]
- CONCLUSIONS: An integrated model combining inflammatory, coagulation, nutritional, and clinical-radiological factors improved risk stratification after ruptured intracranial aneurysm repair. Incorporation of DCI provided additional prognostic value, although external validation is needed.

- More than meets the eye: A meta-analysis of visual outcomes following clipping, coiling, and flow diversion for visually symptomatic paraclinoid aneurysms. [Review]J Clin Neurosci. 2026 Sep 30; 154:112311. [Online ahead of print]JC
- CONCLUSIONS: Clipping may offer the best chance of visual improvement in selected patients with symptomatic paraclinoid aneurysms, though this finding proved fragile across sensitivity analyses. Clipping also appears to carry an increased risk of new, permanent visual complications. In asymptomatic cases, and when visual deficits have been prolonged enough to make recovery unlikely, flow diversion appears to be an appealing strategy towards visual preservation, though the data is not yet sufficient to inform clinical decisions. Future study is necessary to inform patient counseling due to the retrospective evidence base and low certainty of evidence.

- Comparison of Pointwise Encoding Time Reduction with Radial Acquisition (PETRA) MR-Angiography with conventional MR-Angiography-Sequences for Follow-Up of intracranial Aneurysms in a real-world cohort. [Journal Article]AJNR Am J Neuroradiol. 2026 Sep 29. [Online ahead of print]AA
- CONCLUSIONS: PETRA shows potential as a non-contrast MRA technique for follow-up of cerebral aneurysms, providing better vessel visibility than TOF and diagnostic performance comparable to ceMRA. However, its prolonged acquisition time and associated lower image quality currently limits clinical feasibility and real-world applicability. Further studies focusing on scan time reduction and cross-device performance are required.
