- Endoscopic Treatment for Pancreatobiliary Diseases in Patients with Surgically Altered Anatomy: A Perspective from Double-Balloon Endoscopy. [Review]Digestion. 2026 Oct 05; :1. [Online ahead of print]D
- CONCLUSIONS: Double-balloon endoscopy (DBE), first reported in 2001, made it possible to insert the endoscope stably through the long limb and to reach the target perorally. The short-type DBE, and later a newly designed short-type DBE, then made therapeutic ERCP practical, because standard ERCP devices could be used. In a Japanese multicenter prospective study of 311 patients with SAA, the target site was reached in 97.7% (304/311) and the overall procedural success rate of 92.3% (287/311). Among the 283 patients requiring treatment, therapeutic success was achieved in 97.9% (277/283). Adverse events occurred in 10.6% (33/311), and 32 of the 33 events were managed conservatively. In this review we describe how DBE-assisted ERCP (DBE-ERCP) developed, summarize the current evidence, and discuss how it should be positioned relative to single-balloon endoscopy (SBE), interventional endoscopic ultrasound (EUS), and percutaneous drainage.DBE-ERCP and interventional EUS should be regarded as complementary approaches. DBE-ERCP is an important first-line option for many benign biliary disorders in non-Roux-en-Y gastric bypass SAA, particularly when preservation of the native papilla or surgical anastomosis and repeated intervention are important. Alternative approaches should be considered early in selected settings, including interventional EUS after failed balloon-endoscopy access or in malignant biliary obstruction, and EDGE or laparoscopy-assisted ERCP in Roux-en-Y gastric bypass anatomy when appropriate expertise is available. Treatment should be individualized according to the reconstruction anatomy, disease target, need for repeat intervention, and local expertise.

- CONCLUSIONS: The present RCT found no significant difference in the rate of DGE between SSGJ and ESGJ. Postoperative outcomes, nutritional status, and quality of life were comparable, indicating that both techniques yielded comparable outcomes.

- Iatrogenic portobiliary fistula treated with covered stent following percutaneous transhepatic biliary drainage. [Case Reports]Radiol Case Rep. 2026 Dec; 21(12):6277-6280.RC
- Percutaneous transhepatic biliary drainage (PTBD) is a common interventional radiology (IR) procedure typically used in the treatment of biliary obstruction when an endoscopic approach has failed or is not technically feasible. PTBD has a low overall complication rate with the Society of Interventional Radiology suggesting adverse event thresholds of 10% for major complications, 5.7% for hemorrha…

- Balloon Enteroscopy-Assisted ERCP and EUS-Guided Biliary Drainage for Malignant Biliary Obstruction in Patients With Surgically Altered Anatomy: A Systematic Review and Meta-Analysis. [Systematic Review]Dig Endosc. 2026 Oct; 38(10):e70309.DE
- CONCLUSIONS: In the three comparative studies, EUS-BD achieved higher technical and clinical success rates than BEA-ERCP for MBO in patients with SAA, with no significant difference in adverse events. These non-randomized findings are preliminary, and large-scale prospective comparative studies are warranted.

- CONCLUSIONS: Multidisciplinary discussion was essential for reaching the final diagnosis of biliary obstruction and restarting mavacamten, breaking the vicious circle of recurrent presyncope and pulmonary edema secondary to dynamic left ventricular outflow tract obstruction.Multidisciplinary decision-making is paramount when considering novel therapies in complex cardio-oncology patients.

- CONCLUSIONS: Endobiliary RFA was associated with improved survival in unresectable perihilar CCA, with the benefit concentrated in the first three months after biliary drainage, at the cost of more frequent non-tumor biliary obstruction.

- Cholecystogastric fistula masquerading as gastrointestinal stromal tumour: Unusual presentation of gallbladder adenomyomatosis - a case report. [Case Reports]Medicine (Baltimore). 2026 Oct 02; 105(40):e50819.M
- CONCLUSIONS: This case highlights a novel and clinically significant diagnostic pitfall: GA-induced CGF can convincingly simulate an invasive gastric neoplasm on multimodal imaging and endoscopy. Recognition of biliary red flags, including non-visualization of the gallbladder on CT, a mucosal opening adjacent to a subepithelial-appearing lesion on EGD, and intramural cystic foci on imaging, is essential for accurate preoperative diagnosis.

- Duodenal Obstruction and Grade III Acute Cholangitis Following Pancreaticoduodenal Artery Aneurysm Rupture During Conservative Management of Acute Type B Aortic Dissection: A Case Report. [Case Reports]
- Aneurysms of the pancreaticoduodenal arcade are uncommon visceral artery aneurysms that are strongly associated with stenosis or occlusion of the celiac axis and that may rupture irrespective of size. Rupture produces a peripancreatic or retroperitoneal hematoma, and duodenal stenosis is a recognized sequela; biliary complications have rarely been reported. An 84-year-old man admitted with an acu…

- Thyrotoxicosis-Associated Cholestatic Jaundice in a Patient With Incidental Gallstone Disease: A Case Report and Literature Review. [Case Reports]Am J Case Rep. 2026 Oct 02; 27:e953338.AJ
- BACKGROUND Thyrotoxicosis is associated with well-recognized and clinically significant complications, including congestive heart failure and cardiac arrhythmias. The relationship between hyperthyroidism and hepatic dysfunction is complex, with several proposed pathophysiological mechanisms; however, direct cholestatic injury is frequently overlooked. Although mild liver enzyme elevation is commo…

- CONCLUSIONS: An isolated, rapidly rising GGT can serve as an independent early alert for occult terminal bile duct lesions before full mechanical occlusion and hyperbilirubinemia manifest. Early advanced imaging with MRCP is critical in such atypical presentations. Comprehensive molecular profiling provides additional targetable and prognostic insights for personalized postoperative surveillance.

- CONCLUSIONS: DHPs did not prolong the TRBO or reduce its incidence compared with STs. However, migration-related RBO occurred only with STs. The potential advantage of DHPs may be a lower incidence of stent migration rather than longer overall stent patency.
