- Making space for cures: Conditioning for autologous hematopoietic stem cell gene therapies. [Journal Article]Semin Hematol. 2026 Sep 09. [Online ahead of print]SH
- Although allogeneic hematopoietic stem cell transplantation has long been the only curative therapy for nonmalignant blood diseases, recent clinical successes have demonstrated the transformative potential of autologous gene therapies as definitive treatments for these disorders. By providing durable life-changing clinical benefit while reducing the need for suitable allogeneic donors and avoidin…

- Allogeneic stem cell transplantation in aggressive Adult T cell leukemia/lymphoma: a prospective multicenter study. [Journal Article]Blood Adv. 2026 Oct 07. [Online ahead of print]BA
- Adult T-cell leukemia/lymphoma (ATL) is associated with a dismal prognosis in aggressive subtypes, with a median overall survival (OS) of less than one year. In Japan, first line chemotherapy followed by allogeneic stem cell transplantation (allo-SCT) is recommended for eligible patients, yet this strategy is scarcely reported elsewhere, especially among ethnic minorities. We prospectively evalua…

- The Role of Aldehyde Dehydrogenase 2 in Hematopoiesis is Stress-Severity Dependent. [Journal Article]Exp Hematol. 2026 Oct 02; :105940. [Online ahead of print]EH
- Aldehyde dehydrogenase 2 (ALDH2) is a key mitochondrial enzyme that detoxifies reactive aldehydes including formaldehyde, thereby mitigating their genotoxic effects. While ALDH2 deficiency exacerbates bone marrow (BM) failure in Fanconi anemia models, its role under exogenous genotoxic stress is less clear. In this study, we used Aldh2[-/-] mice to investigate how ALDH2 deficiency impacts hematop…

- The complications of irradiation or busulfan for CAYA before hematopoietic stem cell transplantation: a systematic review and meta-analysis. [Systematic Review]
- Hematopoietic stem cell transplantation (HSCT) represents a curative therapeutic modality for children, adolescents, and young-adult (CAYA) patients with various life-threatening hematological malignancies and non- malignant haematological disorders. Conditioning regimens, a cornerstone of HSCT, typically involve total body irradiation (TBI) or busulfan (Bu)-based myeloablation. However, the opti…

- Quercetin and bradykinin potentiating peptide protect against Busulfan induced liver and kidney damage in Sprague Dawley rats: an ultrastructural study. [Journal Article]Ultrastruct Pathol. 2026 Sep 29; :1-12. [Online ahead of print]UP
- Busulfan (BUS), a chemotherapeutic agent used to treat chronic myeloid leukemia, is associated with hepatotoxicity and nephrotoxicity. This study used transmission electron microscope (TEM) to characterize BUS-induced ultrastructural alterations in the liver and kidney of rats, and to evaluate the protective effects of quercetin (QCT), bradykinin potentiating peptide (BPP) and their combination. …

- Assessment of minimum discard volume for central venous catheter sampling: Validation via busulfan therapeutic drug monitoring in pediatric transplant patients. [Journal Article]
- CONCLUSIONS: A minimal discard volume of 1 mL demonstrated non-inferior accuracy compared with the standard 4 mL discard volume for determining plasma Busulfan levels and overall drug exposure during therapeutic drug monitoring. Implementing this approach may reduce cumulative discarded blood volume by up to 21 mL per patient during the seven sampling time points; however, the direct effect of reduced discard volume on anemia or transfusion requirements was not evaluated. This study adhered to the STROBE guidelines for observational research, and no patient or public contribution was involved.

- Cost analysis of allogeneic hematopoietic stem cell transplantation before and after the implementation of therapeutic drug monitoring. [Journal Article]
- CONCLUSIONS: The total hospitalization cost for allogeneic hematopoietic stem cell transplantation did not differ significantly between groups, but the TDM Group showed lower exam and medication expenditures, suggesting economic benefits. Despite similar overall costs, therapeutic drug monitoring may provide advantages in other clinical outcomes not assessed in this study. Further research is warranted to explore these endpoints.

- Clinical and biochemical correction of a patient with neurodegenerative Mucopolysaccharidosis IIIA using hematopoietic stem cell gene therapy. [Journal Article]Mol Ther. 2026 Sep 25. [Online ahead of print]MT
- Autologous haematopoietic stem cell gene therapy (HSC-GT) is an emerging therapy for treatment-refractory neurodegenerative lysosomal storage disorders (LSD). Mucopolysaccharidosis IIIA (MPSIIIA) is a devastating LSD where pathogenic variants in the SGSH gene lead to toxic heparan sulfate accumulation, inflammation and cell death. Children present with developmental delay, neurodegeneration, sign…

- From Therapeutic Drug Monitoring to Model-Informed Precision Dosing: A Review of Busulfan Dosing Optimization in Pediatric Hematopoietic Stem Cell Transplantation. [Review]
- Background/Objectives: Busulfan is a cornerstone conditioning agent for pediatric hematopoietic stem cell transplantation (HSCT), yet its narrow therapeutic index and substantial pharmacokinetic (PK) variability complicate dosing. This review synthesizes current evidence on pediatric busulfan PK, exposure-guided dosing, and the transition from conventional therapeutic drug monitoring (TDM) to mod…

- Incidence of Veno-Occlusive Disease in Patients with Neuroblastoma Treated with Autologous Stem Cell Transplantation After Chemoimmunotherapy with Temozolomide, Irinotecan, and Dinutuximab Beta. [Journal Article]
- BACKGROUND: Chemoimmunotherapy (ChIt) has been proven to be an effective treatment for patients with refractory high-risk neuroblastoma (HR-NBL). The objective was to assess the appearance of veno-occlusive disease/sinusoidal obstruction syndrome (VOD/SOS) after high-dose chemotherapy (HDC) and autologous stem cell transplant (ASCT) in patients previously treated with ChIt for refractory HR-NBL. …

- Allogeneic hematopoietic stem cell transplantation (HCT) is a curative treatment for acquired aplastic anemia (AA). More than 1000 cases of transplants for AA are performed annually in China, and transplant protocols have been continuously refined over recent years. To characterize contemporary transplant practices in China and provide useful insights for global professionals, this study analyzed…

- Comparing the Predictive Performance of Machine Learning Models with a Newly Developed Population Pharmacokinetic Model for Busulfan in Paediatric Patients. [Journal Article]
- CONCLUSIONS: A new popPK model was developed to support individualised dose adjustments in children using fewer blood samples. Both Random Forest and XGBoost could predict busulfan's plasma concentration with performance like that of the popPK model (population prediction). However, a larger dataset is needed for ML methods to describe a complete pharmacokinetic profile.

- Impact of busulfan exposure stratification on transplant outcomes and immune reconstitution in children with severe aplastic anemia. [Journal Article]
- CONCLUSIONS: First-dose busulfan AUC0-t < 900 μmol min/L (3.7 mg h/L) was not associated with impaired engraftment or increased early toxicity in pediatric patients with SAA undergoing allo-HSCT. The predominant longitudinal immune finding was time-dependent immune reconstitution, without confirmatory evidence that the two exposure groups followed different trajectories. These findings are limited by the modest sample size and retrospective design; therefore, prospective validation is needed to inform dosing strategies.
