- Septic Pylephlebitis and Cavernous Transformation Presenting as Pyrexia of Unknown Origin: A Diagnostic Challenge. [Journal Article]
- Acute cholangitis classically presents with fever, jaundice, and right upper quadrant pain, but atypical or subacute disease may present as pyrexia of unknown origin (PUO) and remain unrecognized until vascular complications develop. We report a 45-year-old man referred for weight loss, malaise, and suspected non-cirrhotic portal hypertension after upper gastrointestinal endoscopy showed esophage…

- Bilateral Upper Extremity Superficial Venous Thrombosis and Altered Mental Status as Initial Manifestations of Aggressive B-Cell Lymphoma: A Case Report. [Journal Article]
- A 72-year-old woman with fever, confusion, and upper extremity superficial venous thrombosis was admitted. Despite screening for infectious, neurologic, and autoimmune causes, no diagnosis was established. Generalized lymphadenopathy was found afterwards. An excised lymph node biopsy with immunohistochemical analysis revealed aggressive B-cell lymphoma.

- Performance of a host-protein score (MeMed BV score) for detecting bacterial infections in young children. [Journal Article]
- CONCLUSIONS: In infants under 3 years old with fever without source, the MeMed BV showed limited sensitivity and warrants further evaluation before routine clinical implementation.

- mNGS-Assisted Diagnosis of Visceral Leishmaniasis Presenting as Hemophagocytic Lymphohistiocytosis: Two Cases Confirmed by rK39. [Case Reports]
- CONCLUSIONS: This study demonstrates that mNGS can serve as a valuable adjunct for the rapid etiological diagnosis of VL, particularly when conventional tests are negative. Nevertheless, its current use is largely restricted to endemic areas where advanced laboratory infrastructure is available; therefore, mNGS should be regarded as a complementary diagnostic tool rather than a substitute for routine assays. Broader implementation in clinical practice will require further studies on cost‑effectiveness and operational feasibility.

- Clinical application value of metagenomic next-generation sequencing in children with fever of unknown origin. [Journal Article]
- CONCLUSIONS: We analyzed a targeted pediatric FUO subgroup, and the overall pathogen detection positivity rate showed no statistical difference between mNGS and routine microbial testing. Accordingly, mNGS cannot currently replace standard workflows or routinely screen all FUO children. The two testing methods exhibited complementary pathogen detection spectra. mNGS may act as an auxiliary tool for complicated infectious cases with negative conventional test results. This study generates descriptive observational data on pathogen identification and temporally associated anti-infective regimen adjustments in a selected cohort of FUO children. Further prospective studies with larger sample sizes are required to validate these findings.

- Adult-Onset Still's Disease as an Uncommon Cause of Fever of Unknown Origin. [Case Reports]
- Adult-onset Still's disease (AOSD) is a rare systemic autoinflammatory disorder characterized by fever, rash, arthritis, and hyperferritinemia, often presenting as a diagnostic challenge due to its nonspecific manifestations. We report the case of a 35-year-old previously healthy male who presented with a 20-day history of pruritic erythematous skin lesions, high-grade fever, severe odynophagia, …

- Neuroendocrine Tumor of Unknown Origin Leading to the Diagnosis of Advanced Celiac Disease: A Case Report Exploring a Possible Association. [Case Reports]
- CONCLUSIONS: This case highlights the possible rare coexistence of CD with an atypical neoplastic process, particularly in patients presenting with unexplained iron-deficiency anemia. While CD is associated with an increased risk of certain rare gastrointestinal malignancies, its relationship with NETs remains poorly defined. Larger studies are needed to further explore any potential association between CD and NETs.

- Shifts in liver biopsy indications and histopathological diagnoses: A report from a tertiary care center in Türkiye. [Journal Article]
- CONCLUSIONS: Over the past 22 years, the annual number of liver biopsies remained stable, with abnormal liver function tests as the leading indication, a decline in fibrosis/cirrhosis evaluation, chronic hepatitis B as the most common diagnosis, and a gradual rise in autoimmune liver diseases and MASH.

- Fever of Unknown Origin and Oncocytic Thyroid Cancer: An Unforeseen Association. [Case Reports]
- Fever of unknown origin (FUO) poses significant challenges for clinicians due to the myriad of possible etiologies, exhaustive evaluation requirements, diagnostic delays, patient and family frustration, and prolonged hospital stays. Defined as a fever lasting more than three weeks without a diagnosis despite one week of inpatient evaluation, this clinical phenomenon could result from infectious, …

- Routine stool microscopy unmasking strongyloidiasis: a case report from Eastern Uttar Pradesh. [Case Reports]Front Parasitol. 2026; 5:1892905.FP
- Strongyloidiasis is an intestinal parasitic disease caused by Strongyloides stercoralis, a soil-transmitted nematode. It is a common infection in Southeast Asia, including India, and is related to poor sanitation, rural residence, barefoot walking, and occupational exposure to soil. We report a case of a 71-year-old male farmer from rural Gorakhpur, Eastern Uttar Pradesh, who presented with inter…

- Clinical Utility of Metagenomic Next-Generation Sequencing in Adult Patients with Fever of Unknown Origin: A Retrospective Real-World Study. [Journal Article]
- Background: Fever of unknown origin (FUO) remains a major diagnostic challenge due to its heterogeneous etiologies and nonspecific clinical manifestations. Although metagenomic next-generation sequencing (mNGS) represents a promising diagnostic tool, its clinical utility in adult patients with FUO remains incompletely characterized. Methods: In this study, we retrospectively analyzed adult FUO pa…

- Diverse Gatekeeper Role of a Hospital General Medicine Physician: A Case of a Pathologist with Recurrent Fever. [Journal Article]Keio J Med. 2026 Aug 11. [Online ahead of print]KJ
- In Japan, general medicine (GM) has been recognized as a unique specialty addressing the needs of an aging society with complex comorbidities. GM physicians act as essential "gatekeepers," managing initial evaluations for diverse conditions and facilitating collaboration among various medical departments. This case study presents the critical nature of this role when clinical suspicion encounters…

- Molecular and microscopy-based diagnostic capacity is often insufficient or unavailable in places where infectious disease burdens are highest, such as in West Africa. Rapid diagnostic testing (RDT) can provide quick and affordable diagnoses of common infections but is an imperfect solution due to limitations around detecting and dealing with false-negative and false-positive results. An alternat…

This report describes an unusual systemic toxoplasmosis, presenting as parasitic arthritis in an adult 14-year-old neutered male indoor domestic shorthair cat, with no identifiable cause of immunosuppression, and referred for lethargy, anorexia, and fever.
