- NaV1.8 Inhibition and the Future of Nonopioid Analgesia in Otolaryngology: A Narrative Review. [Journal Article]JAMA Otolaryngol Head Neck Surg. 2026 Oct 22. [Online ahead of print]JO
- CONCLUSIONS: Suzetrigine is a promising nonopioid analgesic with demonstrated efficacy compared to placebo in acute postoperative pain and a favorable safety profile. In otolaryngology, suzetrigine may serve as an adjunct to multimodal analgesia and an opioid-sparing option for patients with contraindications to traditional analgesics, opioid use disorder, or procedures that are expected to have high opioid utilization such as tonsillectomy, sleep surgery, and major head and neck surgery. Future studies in otolaryngology are warranted to evaluate whether suzetrigine can meaningfully reduce postoperative opioid exposure without compromising analgesic effectiveness.

- Suzetrigine (JOURNAVX) for Acute Postoperative Pain: Evidence, Perioperative Integration, and Clinical Guidance. [Review]Adv Anesth. 2026 Dec; 44(1):233-250.AA
- Suzetrigine (JOURNAVX) is an oral, first-in-class selective NaV1.8 inhibitor approved in the United States for adults with moderate-to-severe acute pain. In 4 randomized controlled trials (n = 2768) using bunionectomy and abdominoplasty models, suzetrigine improved SPID48 versus placebo and showed similar efficacy to hydrocodone/acetaminophen 5/325 mg. Its short-term safety profile differs from o…

- CYP2D6-Guided Opioid Therapy for Chronic Pain: A Multi-Center Pragmatic Randomized Controlled Trial. [Journal Article]
- The analgesic effectiveness of several opioids depends on CYP2D6-mediated metabolism to more active opioid receptor agonists. CYP2D6 activity is impacted by genetic variability and medications that inhibit CYP2D6. We conducted a pragmatic, multi-center, randomized, controlled trial to determine whether CYP2D6-guided opioid prescribing improves chronic pain. Participants were adults recruited from…

- Two decades of prescription opioid-related cardiovascular safety signals: a FAERS-based pharmacovigilance study using machine learning. [Journal Article]
- CONCLUSIONS: This study provides the first large-scale, cardiovascular adverse events-focused pharmacovigilance analysis across FDA-approved prescription opioids using over two decades of FAERS data. The findings highlight the need for targeted cardiovascular risk assessment and monitoring, and support more refined prescribing practices and opioid safety labeling. The results showed that the integration of traditional Empirical Bayes signal detection with unsupervised machine learning may enable scalable pharmacovigilance analyses and uncover latent cardiovascular risk patterns that may be masked in broader adverse event assessments.

- Clinical Responses to Cocaine-Positive Urine Drug Screens in Patients Receiving Chronic Opioid Therapy: A Four-Patient Case Series. [Journal Article]Orthop Rev (Pavia). 2026; 18:171215.OR
- CONCLUSIONS: Cocaine-positive UDS results may substantially alter chronic opioid prescribing. Management may include increased monitoring, gradual discontinuation of full agonist opioids, or consideration of buprenorphine-based therapy in appropriately selected patients. These cases emphasize the importance of individualized risk assessment rather than a uniform response to an unexpected UDS result.

- The role of pharmacogenomics in effective pain management with opioids. [Review]Pharmacogenomics. 2026 Aug; 27(8):443-458.P
- Opioids are commonly prescribed as standards of care for various types of pain. Multiple pathways can impact pain response, and understanding medication metabolism is imperative to achieving optimal analgesic response. Pharmacogenomics (PGx) can assist with understanding interindividual variability and aid in safe medication prescribing. Genetic variants in pharmacokinetic and pharmacodynamic gen…

- Chasing the Wrong Door for 30 Years: Is Suzetrigine the Key to Selective Pain Relief? [Review]
- For a long time, pain management has relied on relatively old opioids and broad-spectrum non-opioids, which display efficacy gaps and non-negligible safety risks. This review explores why voltage-gated sodium channels, particularly NaV1.7 and NaV1.8, emerged as prime targets for pain relief, and why early development efforts failed. NaV1.7 was initially considered a promising target due to its ro…

- Effects of MC-100093, GLT-1 enhancer, in mesolimbic brain regions of C57BL/6 mouse exposed to hydrocodone high-dose challenge using comprehensive laboratory monitoring system. [Journal Article]
- Chronic exposure and overdose of opioid cause neurotoxicity through alterations of several neurotransmitters, including glutamate. Upregulating glutamate transporter 1 (GLT-1) has been shown to attenuate the behavioral and neurochemical effects of chronic exposure to drugs of abuse. This study investigated the effects of GLT-1 enhancers-MC-100093 and ceftriaxone-on locomotor activity, respiratory…

- Several studies have supported Dr Gideon Koren’s 1993 landmark article, “Medications Which Can Kill a Toddler with One Tablet or Teaspoonful.” Although each study has its own merits, most focus solely on pediatric overdoses, and few provide an exhaustive reference list for real-time evaluation of ingestions in children and adults. The following xenobiotics warrant special attention because of the…
- Pharmacological actions of novel beta-lactam and non-beta lactam compounds on major astrocytic glutamate transporters in the brain and liver of male C57BL/6 mice exposed chronically to hydrocodone. [Journal Article]
- Chronic exposure to opioid reduced the expression of astrocytic glutamate transporter 1 (GLT-1) and cystine/glutamate antiporter (xCT) in the mesocorticolimbic brain region and ceftriaxone, a beta-lactam antibiotic, attenuated this effect. Importantly, a novel synthetic non-antibiotic beta-lactam, MC-100093, was found effective in attenuating fentanyl and morphine overdose-induced reduction in GL…

- Opioid Dispensing Volume and Per-Patient Intensity in Sickle Cell Disease. [Journal Article]
- CONCLUSIONS: Among individuals with SCD, total opioid dispensing increased or stabilized during 2021 to 2023 after prior declines, most prominently among Medicaid beneficiaries, without broad increases in per-recipient frequency, duration, or dose. These patterns are consistent with broader distribution of opioid dispensing rather than treatment intensification among recipients.

- Comparative Short-term Risk of Severe Gastrointestinal Events Associated With Opioid Type Among Patients Receiving Glucagon-Like Peptide 1 Receptor Agonists. [Journal Article]
- CONCLUSIONS: Among adults with T2D who were using a GLP-1RA, initiation of oxycodone was associated with higher short-term risks of severe constipation and bowel obstruction compared with hydrocodone or tramadol. We did not observe differences in the risk of gastroparesis. Risks were similar between patients initiating hydrocodone and tramadol, although variation across secondary analyses warrants cautious interpretation.

- Supercritical fluid extraction coupled on-line with supercritical fluid chromatography creates a powerful multidimensional analysis platform, but its adoption is limited by the resource-heavy process of method development. This research presents a surrogate optimization approach as a better alternative to traditional design of experiments and response surface methodology. Unlike fixed experimenta…
