- Balanced crystalloids versus 0.9% saline for fluid resuscitation in children with septic shock: an updated systematic review and meta-analysis of randomized controlled trials. [Review]
- CONCLUSIONS: Balanced crystalloids reliably reduce hyperchloraemia, chloride loading, and hypernatraemia and are associated with less renal replacement therapy but confer no advantage in mortality or length of stay; the evidence now excludes a clinically important mortality benefit. They remain a physiologically preferable and equally safe default where available, but fluid choice should not displace early recognition, timely antimicrobials and haemodynamic support.

- CONCLUSIONS: Hyperchloremia is common during pediatric DKA treatment and is associated with CDAKI and longer hospitalization. Saline boluses were associated with chloride burden. Because initial BHB correlated with initial Scr, CDAKI at presentation should be interpreted cautiously. These findings highlight the need for prospective studies on chloride-sparing strategies in pediatric DKA.

- Balanced crystalloid versus saline for resuscitation in adult sepsis: a systematic review and meta-analysis. [Systematic Review]
- CONCLUSIONS: In adult patients with sepsis, treatment with balanced crystalloid solutions for fluid resuscitation significantly reduces mortality (moderate certainty), decreases kidney-related adverse events, and better maintains electrolyte balance, thereby improving patient outcomes. These findings support the preferential use of balanced crystalloids over saline in this population.

- CONCLUSIONS: These findings suggest that topiramate-induced metabolic acidosis is physiologically heterogeneous and often concealed within the Henderson-Hasselbalch framework. Strong ion analysis provides a more complete description of the underlying mechanism and reveals unmeasured anions as a major and previously unrecognized contributor to this drug's metabolic effects.

- CONCLUSIONS: Pneumonia and AKI were associated with sepsis. Identified risk factors may guide veterinarians in the rapid recognition of sepsis. The APPLE FAST score was associated with sepsis and mortality and may support diagnostic and therapeutic decision-making for critically ill dogs with sepsis.

- CONCLUSIONS: Modest benefits in biochemical outcomes were associated with balanced crystalloids. However, due to the small evidence base and low certainty of evidence, it remains unclear if these advantages translate into improvements in clinically important outcomes, particularly rare complications such as cerebral edema and need for mechanical ventilation or inotropic support. Current evidence remains insufficient to support a transition from NS to balanced crystalloids as the initial fluid in pediatric DKA. Further large, multicenter RCTs are needed to better determine the clinical role of balanced crystalloids in this setting.
