- CONCLUSIONS: This study provides a detailed longitudinal assessment of evolving treatment patterns and outcomes, highlighting a clear shift in therapeutic strategies over time with the increasing adoption of systemic therapies. Despite evolving treatment strategies and increased use of systemic therapies, liver function remained the dominant prognostic factor across treatment settings. Observed survival differences between treatment modalities were strongly influenced by baseline patient characteristics and treatment selection.

- CONCLUSIONS: Our findings counter the "bigger is better" paradigm in medical AI: MLMs performed equivalently to LLMs in task mirroring on-scene triage while requiring a fraction of the computational resources. Nimble MLMs offer several advantages for military AI, including seamless deployment on edge computing devices and explainable decision support for combat medics. As AI technologies expand across warfighting domains, considerations of computational efficiency are critical to adopt and field new medical capabilities in the forward environment.

- CONCLUSIONS: Younger individuals face the highest risk of PrEP discontinuation despite being the demographic most vulnerable to HIV acquisition. Retention is driven more by structural healthcare integration and age than by individual risk behaviours. To improve persistence, healthcare systems must transition towards flexible, digital and decentralized care models tailored to younger, system-naïve users.

- CONCLUSIONS: Burnout is closely related to depressive symptoms and various dimensions of burnout. Burnout not only causes mental and physical exhaustion, loss of motivation and concentration difficulties, but can also be accompanied by sleep disorders, anxiety, somatic complaints (e.g. headache, stomach and muscle pain), and these are already a pre cursor to depression. Regular screening with burnout and depression questionnaires is important for prevention and intervention, especially in those who have problems expressing emotions, suppressing emotions, and increased psycho logical or somatic symptoms.
