- Case Report: Efficacy and safety of SGLT2 inhibitors in patients with Alström syndrome: a follow-up report of two siblings from the same family. [Case Reports]
- CONCLUSIONS: Consistent with current clinical management guidelines for Alström syndrome, ertugliflozin demonstrated clinically meaningful reductions in fasting and postprandial glucose concentrations in two patients with AS, as documented in this longitudinal case report. Modest improvements in total cholesterol levels were also observed; however, the isolated effect of ertugliflozin on triglycerides could not be ascertained in one patient due to concurrent fibrate therapy. Early enhancements in insulin sensitivity-assessed via HOMA-IR and dynamic glucose tolerance testing-were observed during the initial treatment phase; however, this effect waned over time in Patient 2, the younger patient. The agent was generally well tolerated, with no serious adverse events reported. Owing to the inherent limitations of this analysis-including a small sample size (n = 2), absence of a control group, and lack of histopathological or mechanistic biomarker data-these observations remain preliminary and warrant validation in adequately powered, prospective, controlled clinical trials.

- Simplified definition and imaging-based stratification of at-risk MASH using magnetic resonance elastography. [Multicenter Study]
- CONCLUSIONS: A simplified definition of at-risk MASH enables broader identification of treatment candidates. LS-MRE demonstrates excellent diagnostic performance and improves noninvasive risk stratification when paired with optimized cutoffs. These findings support the use of MRE to guide resmetirom and semaglutide eligibility and improve access to emerging therapies.

- Relationship between participant-reported outcomes, residual beta cell function and metabolic parameters in youth with newly diagnosed type 1 diabetes. [Randomized Controlled Trial]
- CONCLUSIONS: PedsQL and HFS scores changed little during the first 48 months after diagnosis of type 1 diabetes. These scores showed modest but statistically significant associations with measures of glucose management (HbA1c and CGM time in range), whereas the relationships with residual beta cell function (C-peptide) were weaker and did not reach significance. The modest size of these effects suggests current PROMs capture only limited aspects of the clinical benefit associated with beta cell preservation. Future research should incorporate psychometric instruments that are specifically adapted for young people using modern diabetes technologies and undergoing disease-modifying therapy, to ensure outcomes are meaningfully represented in early-stage type 1 diabetes trials.
