- Effect of improved uterotonic availability on postpartum hemorrhage and maternal morbidity after vaginal delivery, review of a single-center quality-improvement initiative. [Journal Article]
- CONCLUSIONS: Improving second-line uterotonic availability in the delivery room did not improve VD PPH rates or SMM-H but did change second-line uterotonic choice.

- Development and Validation of Artificial Intelligence Prediction of Epicardial Coronary Artery Spasm in Patients Without Obstructive Coronary Artery Disease. [Journal Article]
- Background: Epicardial coronary artery spasm (CAS) is a frequent and important cause of myocardial ischemia. We aimed to develop and validate a noninvasive, artificial intelligence (AI)-driven risk score using routine clinical data to predict CAS in patients without obstructive coronary artery disease (CAD). Methods: This retrospective study analyzed a derivation cohort of 1050 patients and an ex…

- Mild Thrombocytopenia at Admission to Labor and Delivery: Implications for Postpartum Hemorrhage. [Journal Article]
- CONCLUSIONS: Mild thrombocytopenia prior to delivery is associated with a small increased chance of red blood cell transfusion, postpartum hemorrhage, uterotonic use, and the need for additional procedures postpartum.

- Failure of Sequential JADA® Devices in Major Postpartum Hemorrhage Requiring Emergent Supracervical Hysterectomy: A Case Report. [Case Reports]Cureus. 2026 Mar; 18(3):e104498.C
- Major postpartum hemorrhage (PPH) continues to be a significant contributor to maternal health complications, and rapid control of bleeding is essential to prevent hemodynamic instability and the need for surgical intervention. The JADA[®] uterine vacuum system has been increasingly adopted as a primary tool to manage PPH caused by uterine atony. Although reported success rates are high, particul…

- Methylergometrine-induced acute coronary vasospasm in a postpartum patient following caesarean delivery of DCDA twins. [Case Reports]BMJ Case Rep. 2026 Feb 24; 19(2).BC
- A woman in her early 30s with a twin pregnancy conceived through ovulation induction underwent elective caesarean delivery at term. Soon after administration of intramuscular methylergometrine (0.2 mg) for mild postpartum bleeding, she developed sudden, severe chest pain with breathlessness. Her ECG showed diffuse ST-segment depression with reciprocal elevation in aVR, and cardiac enzymes were ra…

- Management of immediate and delayed postpartum hemorrhage with cesarean delivery. [Review]
- The management of immediate and delayed postpartum hemorrhage with cesarean delivery presents specific challenges in diagnosis and treatment. Real-time monitoring and quantitation of intraoperative and postoperative blood loss facilitate rapid recognition of immediate or delayed abnormal bleeding, enabling timely intervention. Effective response to ongoing postpartum hemorrhage depends on clear c…

- Prediction of Coronary Artery Spasm in Patients Without Obstructive Coronary Artery Disease Using a Comprehensive Clinical, Laboratory and Echocardiographic Risk Score. [Journal Article]
- Background: The lack of an accurate coronary artery spasm (CAS) risk prediction model highlights the failure to consider dynamic coronary health and reveals a gap in understanding CAS. Methods: A total of 913 Taiwanese patients (460 women and 453 men) with suspected ischemic heart disease but without angiographic obstructive coronary artery disease were subjected to intracoronary methylergonovine…

- Treatment patterns and costs among hospital births with abnormal postpartum uterine bleeding and postpartum hemorrhage in the United States (2016-2022). [Journal Article]
- CONCLUSIONS: Using second-line uterotonic/TXA use and PPH diagnosis codes, the observed rate of abnormal postpartum bleeding was higher than previously reported and associated with significant clinical and economic burden. Total costs increased with greater use of uterotonics/TXA, especially in cases involving UBT or SMM such as hysterectomy. Higher treatment intensity likely reflects escalation in response to more severe clinical presentations. These findings highlight the need for appropriate intervention, better treatment options, and preventive strategies to manage abnormal postpartum uterine bleeding and PPH.

- Drugs and Lactation Database (LactMed): Methylergonovine [BOOK]Drugs and Lactation Database (LactMed). National Library of Medicine (US): Bethesda (MD).BOOK
- Limited information indicates that maternal doses of methylergonovine up to 0.75 mg daily produce low levels in milk. Product labeling in the U.S. currently recommends avoiding breastfeeding for 12 hours following the last dose of methylergonovine. This warning appears to be based on unpublished adverse reactions in breastfed infants after several days of maternal methylergonovine therapy. The us…
- Coronary Artery Spasm: From Physiopathology to Diagnosis. [Review]
- Coronary artery spasm (CAS) is a reversible vasoconstriction of normal or atherosclerotic epicardial coronary arteries with a subsequent reduction in myocardial blood flow, leading to myocardial ischemia, myocardial infarction, severe arrhythmias, or even sudden death. It is an entity that should be recognized based on a particular clinical presentation. Numerous differences exist between CAS and…

- Time to Hemorrhage Response after Implementation of Obstetric Hemorrhage Bundle. [Journal Article]Am J Perinatol. 2025 Nov; 42(15):2049-2054.AJ
- This study aimed to evaluate the time to uterotonic administration before and after the implementation of an obstetric hemorrhage (OBH) bundle. This is a retrospective cohort study of deliveries at an urban tertiary care hospital before and after the implementation of an OBH bundle. Two groups were compared on time from delivery to the first dose of postpartum uterotonic intervention. Secondary ou…

- A Snapshot Survey of Uterotonic Administration Practice During Cesarean Section: Is There a Difference Between the Attitudes of Obstetricians and Anesthesiologists? [Journal Article]
- Background and Objectives: We aimed to evaluate the current uterotonic administration practices among anesthesiologists and obstetricians and gynecologists (OBGYNs) during cesarean section (CS), focusing on variations in approaches for low- and high-risk postpartum hemorrhage (PPH) cases. The objective was to identify key differences and provide evidence that could contribute to the development o…

- Implementation of an oxytocin bolus protocol for cesarean delivery at a Texas level IV maternal center: a single-center retrospective study. [Journal Article]
- CONCLUSIONS: Patients in the oxytocin bolus cohort were approximately 75% less likely to receive a secondary uterotonic agent compared to patients in the oxytocin infusion cohort. A limitation of this study was that we defined intramyometrial administration of oxytocin as a secondary uterotonic, and our results may not be generalizable to hospitals that do not use intramyometrial oxytocin.

- Intramuscular methylergonovine to decrease blood loss during cesarean delivery for twins: A triple-blinded placebo-controlled randomized trial. [Journal Article]
- CONCLUSIONS: Prophylactic IM methylergonovine administered after umbilical cord clamping during twin CD significantly reduced intraoperative blood loss and hemoglobin drop on POD 1. These data support further study of methylergonovine as a preventative treatment strategy at the time of twin CD.

- Methylergometrine has widely been used pharmacologically to treat conditions such as pain, addiction, vasoconstriction, migraines, and Parkinson's disease. Despite its side effects, it is used as a therapeutic agent and research material for various diseases based on its natural potential; however, the regulatory effect of its interaction with the nicotinic acetylcholine receptor (nAChR) has not …
