There is no published experience with fluphenazine during breastfeeding. Very limited long-term follow-up data indicate no adverse developmental effects when other phenothiazines are used alone. Because of the lack of published experience with fluphenazine during breastfeeding, other antipsychotic agents may be preferred, especially while nursing a newborn or preterm infant. Monitor the infant for drowsiness and developmental milestones, especially if other antipsychotics are used concurrently.

Fluphenazine (CAS 69-23-8) concentrations in plasma of schizophrenic patients after oral (fluphenazine hydrochloride, Moditen) or intramuscular (fluphenazine enanthate, Moditen Retard, or fluphenazine decanoate, Modecate) administration of drug were measured by means of high-performance liquid chromatography. The extracted compound was identified by gas chromatography with mass selective detector. Pharmacokinetics of fluphenazine has been studied.

Comparison of extrapyramidal side effects of two fluphenazine hydrochloride injections. Lyogen and Moditen was carried out by the in vivo experiments in mice. The incidence, intensity and duration of the catalepsy in mice after i. m. application of 10 mg/kg fluphenazine hydrochloride in two forms of sterile water solution have been observed. Catalepsy of significant intensity occurred after a short period of 15 minutes regarding the both formulations and maximal values were observed 30 and 60 minutes after application of Lyogen, and Moditen respectively. Significant values of cataleptic intensity with respect to control group (the water for injection-treated group) were registered during 28 hours after the application of both formulations. This study of cataleptic effects could not confirm significant differences between these two formulations, between the rate of occurrence and duration of effects. On the other hand, the difference between intensities were observed and were more prominent in the period between 6-24 hours. An the most cases they were not significant (p greater than 0.05).

The efficacy of the use of moditen-depo and haloperidol decanoate as preventive anti-recurrence+ therapy was studied and compared in 2 groups of patients suffering from attack-like schizophrenia with frequent exacerbations. The cost-use analysis made it possible to establish their higher efficacy as compared to the routine maintenance therapy. Proceeding from the clinical, economic and socioeconomic indicators the use of haloperidol decanoate turned out more effective than the use of moditen-depo.

As many as 16 patients with multi-attack schizophrenia were examined for the dynamics of the efficacy of long-term therapy with moditen-depot , expressed in the clinico-economic parameters. It turned out that the degree of positive treatment effect was different, being dependent on the main disease tendencies during the treatment. In half patients who received the drug during retention of progredient tendencies, "depletion" of the positive treatment effect was observed which affected the dynamics of the clinico-economic parameters. Apparently, in the latter cases the replacement of the long-acting drug is desirable.

In 31 patients with paroxysm-like progressive and 19 with continuously progressive paranoid schizophrenia the authors examined the results of long-term (from 4 to 11 years) treatment with moditen depot. The mean dosage of the drug was 25-50 mg per 3-4 weeks. A good therapeutic response was observed in more than half the cases. In group I patients it was expressed in the absence of relapses, improvement of quality and increase in the duration of the remissions; in group II it was expressed in an increased tendency toward a regressing course with a reduced intensity of processional manifestations. A favourable effect was accompanied by improved social and occupational adaptation of patients.

Developed the present study in conjunction with a project that evaluated the psychiatric diagnosis of schizophrenia (Landmark, 1982; Landmark & Leslie, 1982). This design, which included 13 different interview systems for diagnosing schizophrenic disorders, allowed for an in-depth analysis of the Whitaker Index of Schizophrenic Thinking (WIST). Eighty-five outpatients who were receiving long-term chemotherapy for schizophrenia at the Moditen Clinic of London Psychiatric Hospital were reassessed individually using each of the 13 systems, following a clinical interview format, and the WIST. Major statistical analysis evaluated the concurrent validity and discriminatory efficiency of the WIST in comparison to the 13 systems. The WIST was found to show a consistently high agreement with each of the 13 systems for an average WIST discriminatory efficiency of 70%. The WIST is suggested as a practical, easily administered, diagnostic tool that performs well in comparison to the interview systems.

The results of treating 251 therapeutically resistant patients with paranoid schizophrenia by moditen-depo, its combination with other psychotic drugs or leponex was compared. The results obtained indicate that a comprehensive treatment (moditen-depo with other psychotropic drugs) and by leponex may be successfully applied to some of the paranoid schizophrenics resistant to moditen-depo and other neuroleptic drugs or their combinations. Comprehensive treatment appeared to be more effective in systematized delusions, the syndrome of psychic automatisms and in a crude emotional-volitional defect. Treatment by leponex was more efficacious in emotionally saturated nonsystematized delusions, pronounced disordered behaviour and mild manifestation of deterioration.

The authors studied the effectiveness of out-patient treatment by Moditen-Deppt of 128 schizophrenic patients. A comparison of the previous remissions appearing during usual pharmacotherapy and remissions seen during Moditen-Depot treatment depicted statistically significant differences, indicating a distinct prevention of exacerbations due to this preparation, an increase in the duration and improved quality of remissions. Such improved remissions were expressed in lesser severity, in a decrease of psychopathological disturbances, changes qualified as a "mollification of a defect" and an improvement of some socio-clinical indices.
