  • CONCLUSIONS: This national cohort illustrates substantial progress in early pCD management over time. These shifts likely reflect growing clinical awareness of the importance of early and multidisciplinary interventions. However, surgical procedures aimed at fistula closure remain infrequently performed in the early disease course, which may be related to the inclusion of a predominantly therapy-refractory patient group.

  • CONCLUSIONS: FPIAP may represent an early marker of atopic predisposition rather than an isolated gastrointestinal disorder. Recognizing its potential role within the atopic march has important implications for diagnosis, dietary management, and timing of allergen introduction. Improved diagnostic precision, earlier reintroduction of offending allergen, and timely introduction of other food allergens may reduce unnecessary restriction and mitigate downstream atopic risk.

  • CONCLUSIONS: Mesalazine nonadherence in this Moroccan UC cohort was frequent and multifactorial. Behavioral factors, cultural beliefs, high pill burden, financial limitations, and health system barriers all contributed to poor adherence. Simplified dosing regimens, improved access to affordable formulations, and structured patient education may help optimize long-term adherence in real-life practice.
