- From Cholinergic Crisis to Anticholinergic Delirium: When the Antidote Mimics the Poison. [Case Reports]Cureus. 2026 Aug; 18(8):e115146.C
- Organophosphorus pesticide poisoning remains a major public health concern due to widespread agricultural use and easy availability of these compounds. Atropine is the cornerstone of treatment for life-threatening muscarinic manifestations. However, repeated or prolonged administration can cause central anticholinergic toxicity, which may closely resemble the neurological effects of organophospho…

- Potent inhibition and limited reactivation of human cholinesterases by cholesterol- and quinoline-based oximes. [Journal Article]Toxicology. 2026 Sep 01; 527:154579. [Online ahead of print]T
- Current oxime antidotes exhibit poor blood-brain barrier (BBB) penetration due to their permanent positive charge, limiting the reactivation of organophosphorus (OP)-inhibited cholinesterases in the central nervous system. To address this limitation, the present study investigates nine lipophilic cholesterol- and quinoline-based oximes as potential reactivators of human acetylcholinesterase (hACh…

- Organophosphate pesticide poisoning: benefits and limitations of oxime therapy. [Journal Article]Clin Toxicol (Phila). 2026 Aug 26; :1-7. [Online ahead of print]CT
- CONCLUSIONS: Laboratory parameter-guided management of organophosphate poisoning, based on acetylcholinesterase activity, acetylcholinesterase reactivatability, and plasma inhibitory activity, supports clinical decision-making regarding oxime therapy.

- Assessing oxime reactivation efficacy using principal component analysis: Insights from nerve agents inhibited human butyrylcholinesterase. [Journal Article]
- CONCLUSIONS: A large dataset was efficiently analysed by maximizing the preservation of variability and generating new, uncorrelated variables. To our knowledge, this study is the first to apply PCA to assess oxime's reactivation efficacy, thus providing insights into the relationships between oxime properties and their efficacy in restoring OP-inhibited cholinesterase activity.

- Intentional Large-Volume Diazinon Ingestion Complicated by Seizure and Acute Respiratory Failure: A Case Report. [Case Reports]Cureus. 2026 Jun; 18(6):e111774.C
- Organophosphate poisoning is a major cause of acute cholinergic toxicity and can progress rapidly to respiratory failure and death. In the emergency setting, diagnosis is often clinical, and treatment should not be delayed while awaiting confirmation of the specific compound. We report a case of a 30-year-old male patient who presented after intentional ingestion of a large reported volume of dia…

- Pain in the acephate: A case of protracted toxicity after an intentional ingestion of ant killer. [Case Reports]Toxicol Rep. 2026 Dec; 17:102311.TR
- Organophosphates (OP) have a variety of applications, including widespread use as pesticides. Although OP poisoning in the U.S. has been declining, toxicity from intentional ingestions of commercially available preparations is still a concern. We describe a case of a 17-year-old female who intentionally ingested an acephate-based fire ant insecticide and developed severe cholinergic toxicity, des…

- Conventional and nonconventional therapies for pesticide-induced cardiotoxicity: A systematic review. [Systematic Review]Food Chem Toxicol. 2026 Oct; 216:116299.FC
- Pesticides, as pervasive environmental pollutants, have been associated with cardiotoxic effects. However, evidence regarding cardioprotective strategies remains fragmented, particularly concerning the integration of conventional therapies and nonconventional approaches involving natural products. This systematic review (PROSPERO:CRD420251143191) aimed to synthesize experimental evidence on cardi…

- Successful treatment of acute organophosphorus pesticide poisoning complicated by intestinal necrosis with exploratory laparotomy: a case report. [Case Reports]Open Med (Wars). 2026 Jan; 21(1):20261458.OM
- CONCLUSIONS: This case highlights that in patients with severe organophosphorus poisoning, any unexplained alteration in abdominal signs should prompt high clinical suspicion of gastrointestinal complications, and prompt surgical intervention may be lifesaving. However, as a single case report, this study carries inherent limitations, and future large-scale investigations remain essential to further substantiate these findings and develop more effective therapeutic strategies to enhance the overall management of AOPP.

- A cohort study on poisons, clinical insights, management of poisoning, and predictors of mortality in Pakistan. [Journal Article]Sci Rep. 2026 Jun 27. [Online ahead of print]SR
- Acute poisoning is a severe global public health issue that involves injury or death caused by exposure to an exogenous substance through any route. This study evaluated the most common poisons, reasons for poisoning, clinical management of poisoning cases, and their associated outcomes. One-year prospective cohort study executed at an academic medical center in Sindh, Pakistan. The predictors of…

- Antibody-Assisted Protection from Paraoxon and Nerve Agent Model Compounds. [Journal Article]J Med Chem. 2026 Jul 09; 69(13):15581-15596.JM
- A key challenge in organophosphorus (OP) research is to develop efficient means to decrease OP levels after exposure. Scavenging, capturing, or degrading circulating OPs in vivo could minimize OP penetration into the brain and other organs, and decrease toxicity. OPs produce acute effects primarily through acetylcholinesterase (AChE) inhibition, leading to neurotoxic events. Current standard of c…

- Successful use of intravenous droperidol in hyperemesis following failure of sequential antiemetic therapy. [Case Reports]Am J Emerg Med. 2026 Aug; 106:131-133.AJ
- Nausea and vomiting in pregnancy is a common affliction, especially in the early trimesters. This is typically managed at home with doxylamine-pralidoxime or other home remedies. Many women, however, require additional adjuncts to control their symptoms. In this case we present a G2P0 female at 7 weeks gestation who arrived in the emergency department with persistent vomiting for several days. Th…

- N,N'-dibenzyl imidazolium oximes are potent reactivators of organophosphate-inhibited human butyrylcholinesterase. [Journal Article]Eur J Med Chem. 2026 Sep 05; 313:118895.EJ
- Organophosphorus compounds (OP), like pesticides and nerve agents, are potent inhibitors of acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) due to the phosphylation of their catalytic serine. Current treatment is limited, as approved oximes lack broad-spectrum efficacy and are poor reactivators of inhibited BChE. An alternative approach is pseudo-catalytic OP bioscavenging in which B…

- Serine hydrolase targets of a sarin surrogate anticholinesterase and oxime-mediated reactivation in rat brain. [Journal Article]Toxicol Sci. 2026 Mar 02; 209(3).TS
- Although acetylcholinesterase (AChE) is the primary inhibitory target for organophosphate (OP) insecticides and chemical warfare nerve agents, research supports the concept that the wide range of systems affected by OPs are not solely dependent on the anticholinesterase activity of OPs but are attributable to the inhibition of other serine hydrolases. Oxime reactivators play an integral role in t…
