- CONCLUSIONS: We successfully developed two separate population PK models for tacrolimus in SPK and KT recipients together with a combined SPK and KT model, showing a significantly reduced tacrolimus oral absorption rate in SPK recipients. In addition, our data suggested that SPK recipients might need higher C0 target concentrations to obtain a similar tacrolimus exposure as KT recipients.

- CONCLUSIONS: This case suggests that sequential HER2-targeted ADC therapy utilizing different cytotoxic payloads may offer a clinically viable strategy to manage acquired resistance in mUC. Furthermore, it indicates a potentially manageable safety profile regarding renal allograft function. Further longitudinal follow-up and mature survival data are required to establish whether ADCs can serve as a novel standard of care for this specific patient cohort. Additional research is warranted to formulate optimal treatment strategies and safety guidelines for cancer therapy in transplant recipients.

- CONCLUSIONS: Early tacrolimus underexposure is a simple and clinically relevant predictor of 12-month AR, complementing C/D-ratio-based risk stratification at 3 months. These findings align with the evidence that high early tacrolimus clearance and low tacrolimus exposure increase AR risk and support the recommendations for optimizing early tacrolimus dosing.

- CONCLUSIONS: PTDM developed in one-fifth of this Indian cohort. Hyperglycaemia detected during the index hospitalisation, together with older age and an adverse pre-transplant metabolic profile, identified recipients at higher risk. Systematic inpatient glucose surveillance offers an actionable window for early intervention, consistent with randomised evidence that early basal-insulin therapy can attenuate progression to sustained PTDM.

- CONCLUSIONS: CMV infection should be considered a plausible trigger of de novo post-transplant thrombotic microangiopathy, particularly in patients with persistent disease despite withdrawal of calcineurin inhibitors. Although post-transplant TMA is frequently multifactorial, early recognition of CMV viremia and timely comprehensive management addressing all contributing factors may be associated with favorable hematologic recovery and improvement of graft function.

- CONCLUSIONS: This is the first report of early donor-derived HCM treated with mavacamten within 1 year of transplant. Targeted myosin inhibitor therapy appears promising, and CYP3A4-mediated pharmacokinetic interactions may be lower than previously believed.Mavacamten may be safe and effective for donor-derived HCM in the early posttransplant period. Utilization of carefully selected donors with HCM or left ventricular hypertrophy could increase the donor pool.
