- Therapeutic Treatment with a Clinically Relevant Dose of Dexamethasone Reduces Pulmonary Vascular Leak in a Mouse Model of ARDS. [Journal Article]Lung. 2026 Oct 05; 204(1).LUNG
- CONCLUSIONS: This study provides a clear link between timing of dexamethasone treatment and viral burden, vascular leak, and mortality. 0.5 mg/kg dexamethasone affects clinically relevant parameters in the influenza model and has comparable exposure to human dexamethasone dosing; thus it is a more relevant dose to use in future mouse studies.

- VEGF165b attenuates LPS-induced acute lung injury via antagonizing the VEGF165a/VEGFR2 signaling pathway. [Journal Article]
- Vascular endothelial growth factor (VEGF) plays a central role in the pathogenesis of acute lung injury (ALI) and its severe form, acute respiratory distress syndrome (ARDS), yet targeted therapies for these conditions remain elusive. This study identified the VEGF-A splice variant VEGF165b as a crucial endogenous protector against ALI/ARDS. In patients, the circulating ratio of VEGF165b to total…

- CONCLUSIONS: TISA-818 was generally well tolerated. Day-28 mortality was numerically imbalanced but not sustained, likely reflecting small-sample variability; cautious interpretation is warranted. Consistent improvements across all efficacy endpoints, particularly with 6 mg BID in non-intubated patients, support further investigation in adequately powered trials.

- Fat Emboli Entrapped in the Tricuspid Valve Chordae Following Autologous Fat Grafting: An Echocardiographic-Pathological Correlation. [Case Reports]Cureus. 2026 Sep; 18(9):e117082.C
- Fat embolism is a rare but feared complication of liposuction combined with autologous fat grafting (lipofilling). It carries a high mortality rate, predominantly affecting young patients with few or no comorbidities. To date, few guidelines exist, and management remains largely supportive. We report the case of a 40-year-old woman admitted to our center following cardiac arrest during breast lip…

- Endothelial Exostosin-1 Is Needed for Glycocalyx Integrity and Inflammatory Resolution in Acute Lung Injury. [Journal Article]
- The endothelial glycocalyx depends on Exostosin-1 (Ext1)-mediated heparan sulfate synthesis; however, its endothelial cell-autonomous role in acute inflammation remains unclear. Therefore, we examined the role of Ext1 during acute inflammation. In this study, endothelial-specific Ext1-deficient mice (Ext1[flox/flox] and VE-cadherin-Cre) were generated, and their response to endotoxin challenge wa…

- Characterization and 3-Year Survival of Clinical Subtypes and Host Response Phenotypes of Acute Respiratory Failure Requiring Mechanical Ventilation. [Journal Article]
- CONCLUSIONS: Our results show that patients who are mechanically ventilated with ARF demonstrate marked heterogeneity in physiologic features, biology, and long-term outcomes. Overall, fewer than one-half of patients survived to 3 years, but survival varied markedly across clinical subtypes beyond traditional ARDS categorization. Clinical subtype classification identifies distinct populations with divergent long-term prognoses, providing information complementary to biological subphenotyping. These findings provide quantitative outcome data to support prognostic counseling and to establish a reproducible framework for clinical research in ARF resulting from heterogeneous causes.

- CONCLUSIONS: Early LUS signs differ significantly between survivors and non-survivors among patients meeting the 2023 ARDS definition. Pleural line abnormalities showed an exploratory association with 28-day mortality, suggesting potential value for bedside risk stratification. This study was registered with the Chinese Clinical Trial Registry (Registration Number:ChiCTR2600117533).

- Alveolar epithelial glycocalyx degradation is associated with impaired lung mechanics, lung injury severity, and clinical outcomes in ARDS. [Journal Article]Am J Physiol Lung Cell Mol Physiol. 2026 Oct 01. [Online ahead of print]AJ
- Background: The alveolar epithelial glycocalyx is a layer of glycosaminoglycans (GAGs) and proteoglycans (e.g., syndecan-1) that is located between the apical alveolar epithelium and the surfactant layer. It is degraded in murine models of lung injury and in patients with ARDS. Targeted degradation of the epithelial glycocalyx impairs surfactant function in mice. We sought to determine the relati…

- Acute liver failure induced by hepatitis E virus complicated with pancreatitis and refractory acute respiratory distress syndrome after rapid ascent to high altitude: a case report and literature review. [Case Reports]
- CONCLUSIONS: Sustained high-altitude hypoxia may contribute to refractoriness and delayed recovery of ALF-related ARDS. Altitude descent may be a valuable adjuvant strategy for hemodynamically stable patients with refractory hypoxemia, pending further prospective validation.

- Plasma Biomarkers of Acute Respiratory Distress Syndrome Vary with Body Size in the Absence of Acute Illness. [Journal Article]medRxiv. 2026 Sep 21.M
- CONCLUSIONS: Plasma proteins used as biomarkers of inflammation and injury in ARDS vary with body size even in the absence of acute illness. Differences during ARDS may not be attributable to injury alone and may be misinterpreted in patients with obesity due to antecedent variation.

- Rapid progression of diffuse alveolar damage in ARDS: temporal alignment, composite phenotype, and physiological interpretation. [Letter]
- A recent study associated early proliferative or fibrotic diffuse alveolar damage with mortality in acute respiratory distress syndrome. We highlight four interpretive concerns: phenotype status was assigned at biopsy after follow-up began, creating potential immortal-time bias; a single biopsy cannot measure pathological velocity; sparse and imbalanced phase-by-timing cells limit component-speci…

- Preoperative Carriage of Respiratory Viruses and Acute Respiratory Distress Syndrome After Cardiac Surgery: Protocol for the VIRUS-ATTAC Prospective Cohort Study. [Journal Article]
- CONCLUSIONS: VIRUS-ATTAC is the first prospective multicenter study to directly evaluate the association between preoperative asymptomatic respiratory viral carriage and postoperative ARDS in cardiac surgery patients. If confirmed, this association would provide a rationale for evaluating preventive strategies, such as systematic preoperative vaccination or postponement of elective surgery in carriers, in future interventional studies aiming to reduce postoperative respiratory complications.

- Open versus closed suctioning on lung volume and oxygenation in ARDS patients with PEEP 5-10 cmH2O: A randomized controlled trial. [Randomized Controlled Trial]
- CONCLUSIONS: In ARDS patients receiving PEEP 5-10 cmH2O, CS reduces lung volume loss during suctioning and better preserves early oxygenation, but results in slower post-suctioning lung volume recovery compared with OS. These findings reveal a physiologically important trade-off between the two methods, though their clinical relevance warrants further investigation in larger, outcome-focused trials.

- Management of sepsis-associated acute lung injury (S-ALI) remains challenging due to complex inflammatory cascades, oxidative stress amplification, and limited therapeutic delivery to injured pulmonary tissues.
