- CONCLUSIONS: This case highlights the diagnostic challenge of distinguishing de novo Ph-positive T-ALL from T-lymphoid BP-CML at initial presentation, particularly in pediatric patients. Accurate classification is essential because it has important implications for therapeutic decision-making and long-term management in the TKI era. The subsequent development of CML following TKI discontinuation strongly supports that the initial presentation represented lymphoid BP-CML rather than de novo Ph-positive T-ALL. This observation further underscores the importance of sustained TKI therapy and adherence in preventing disease recurrence and optimizing long-term clinical outcomes.

- CONCLUSIONS: CAN is effective in achieving rapid and sustained clinical and laboratory control in FMF, including stringent endpoints of complete response. Early effectiveness may favour better long-term outcomes, but delayed achievement of complete response and full laboratory control is not uncommon. - CONCLUSIONS: This case highlights an unusual severe presentation of clinically diagnosed Lyme disease with probable neuroborreliosis, intracranial hypertension, septic shock, and bone marrow suppression. Geographic origin should not preclude diagnostic consideration. In critically ill patients with unexplained fever, cytopenias, and dynamic cutaneous lesions, careful tick exposure history and bedside dermatological assessment may prove decisive where laboratory testing is inconclusive.
