- Incomplete Kawasaki Disease With Rapid Coronary Aneurysm Progression. [Case Reports]JACC Case Rep. 2026 Sep 28; :110344. [Online ahead of print]JC
- CONCLUSIONS: This case illustrates the challenges of diagnosing and managing incomplete Kawasaki disease, particularly when initial symptoms overlap with common viral infections and when early echocardiography is normal. Early risk stratification, guided by age and initial coronary z-scores, is critical for identifying high-risk patients who may benefit from adjunctive therapies. It demonstrates that severe coronary involvement can progress even after timely treatment and underscores the importance of close monitoring.Incomplete Kawasaki disease can rapidly cause giant coronary aneurysms despite timely treatment, making serial echocardiography essential.

- Crohn's disease unmasks pulmonary sarcoidosis on biologic screening: dual granulomatous pathology responsive to infliximab. [Case Reports]BMJ Case Rep. 2026 Sep 28; 19(9).BC
- A man in his 30s with longstanding ileal Crohn's disease was found to have pulmonary sarcoidosis during routine pre-biologic evaluation. Although he had no respiratory symptoms, lung function testing showed mildly reduced diffusion capacity. Radiological imaging demonstrated diffuse pulmonary nodules and mediastinal lymphadenopathy, and transbronchial biopsies confirmed non-necrotising granulomas…

- Integrated bioinformatics analysis of m6A/m7G/m5C/m1A RNA methylation-related genes in ulcerative colitis. [Journal Article]Biochem Biophys Res Commun. 2026 Sep 26; 838:154648. [Online ahead of print]BB
- Ulcerative colitis (UC), a primary subtype of inflammatory bowel disease, remains incompletely understood in terms of its molecular etiology. This study aimed to characterize the gene signatures associated with m6A, m7G, m5C, and m1A RNA modifications and explore their potential role in UC pathogenesis. We integrated three GEO datasets and, via LASSO regression, random forest, and SVM-RFE algorit…

- Early Prediction of Primary Non-Response to Infliximab in Crohn's Disease Using Routinely Available Clinical and Week 2 Laboratory Indicators: A Two-Center Real-World Cohort Study. [Journal Article]
- CONCLUSIONS: Model 2C provides an exploratory approach for early PNR risk stratification during IFX induction therapy, particularly for identifying patients at lower risk of PNR. However, it should not be interpreted as a tool for individual absolute-risk prediction or used as a standalone basis for treatment intensification or switching. Further multicenter prospective validation, recalibration assessment, and clinical-impact studies are required.

- Combined Infliximab and Tofacitinib Therapy for Severe Refractory Juvenile Idiopathic Arthritis Associated Panuveitis: A Challenge, Dechallenge, Rechallenge Case Report. [Journal Article]Ocul Immunol Inflamm. 2026 Sep 26; :1-7. [Online ahead of print]OI
- CONCLUSIONS: This challenge, dechallenge, and rechallenge sequence is consistent with a possible complementary effect of infliximab and tofacitinib in this patient. However, causality and pharmacologic synergy cannot be established from a single case. Combined tumor necrosis factor alpha and Janus kinase inhibition may represent an exceptional rescue approach for carefully selected patients with severe refractory JIAU, although its efficacy and long-term safety require further evaluation.

- Population Pharmacokinetics of Subcutaneous Infliximab: A Real-World Study of Naïve and Switch Patients. [Journal Article]
- Background: Subcutaneous infliximab (SC-IFX) represents a significant advance for patients with inflammatory bowel disease (IBD) due to the possibility of home self-administration. However, at this time, there are no clear recommendations for implementing model-informed precision dosing (MIPD) in accordance with real-world clinical knowledge. Objective: The aim of the present study is to develop …

- Connecting the Dots Between Pharmacokinetics, Inflammatory Biomarkers, and Mucosal Healing in Patients on Infliximab with Inflammatory Bowel Disease. [Journal Article]
- Background/Objectives: Mucosal healing (MH) is a key therapeutic target in inflammatory bowel disease (IBD) associated with sustained remission, reduced hospitalisation, and improved outcomes. Its assessment relies on endoscopic evaluation, which is invasive, costly, and not always feasible. Therapeutic drug monitoring (TDM) of infliximab (IFX) and evaluation of available biomarkers may provide n…

- Medical Therapies Beyond Infliximab for Perianal Fistulizing Crohn's Disease: Current Evidence and Future Perspectives. [Review]
- Perianal fistulizing Crohn's disease (pfCD) represents one of the most challenging manifestations. Optimal management requires a multidisciplinary approach combining surgical drainage, appropriate imaging, and advanced medical therapies. While infliximab remains the first-line medical therapy, primary non-response or secondary loss of response occurs in many patients, which highlights the need fo…

- Biological and Targeted Therapy in Non-Infectious Uveitis: Current Evidence and Future Perspectives. [Review]
- Non-infectious uveitis (NIU) comprises a group of immune-mediated inflammatory diseases of the uvea that frequently affect people of working age. If inflammation is not controlled promptly, it may lead to permanent structural damage and visual loss. Prolonged corticosteroid use increases the risk of adverse effects. Contemporary treatment aims not only to control inflammation, but also to minimis…

- Clinical Insights into RELA-Associated Disease: From Genotype to Phenotype and Exploring Treatment. [Journal Article]Genes (Basel). 2026 Aug 29; 17(9).G
- Background/Objectives: RELA encodes the p65 subunit of NF-κB and plays a critical role in immune regulation, epithelial protection, and anti-apoptotic signaling. Pathogenic RELA variants cause monogenic immune dysregulation with heterogeneous clinical manifestations. However, genotype-phenotype relationships and optimal treatment strategies remain incompletely defined. Methods: We conducted a com…

- Vedolizumab vs TNF Antagonists and Long-Term Outcomes in Ulcerative Colitis. [Journal Article]
- CONCLUSIONS: In this cohort study of biologic-naive patients with UC initiating advanced therapy, the burden of hospitalization did not differ significantly between patients receiving vedolizumab and those receiving a TNF agonist; however, a vedolizumab-first strategy was associated with a greater proportion of time spent in corticosteroid-free disease stability over 5 years than a TNF antagonist-first strategy. These findings may inform first-line treatment selection in UC.

- Predicting disease flares in axial spondyloarthritis using different machine learning models in the METEOR-SpA registry. [Journal Article]RMD Open. 2026 Sep 24; 12(3).RO
- CONCLUSIONS: A reduced logistic-regression model showed moderate internal and modest external discrimination for short-term flare risk in axSpA. While not intended to guide individual pre-emptive therapy modification, the model may support low-risk interventions such as group-level risk stratification or tighter follow-up after further validation.

- Pediatric-onset ulcerative colitis (UC) is characterized by an aggressive phenotype, with higher rates of pancolitis and more rapid progression to a medically refractory state. After failure of conventional therapies and multiple biologic agents, treatment options are limited and often require surgery. We report the case of a 15-year-old girl with a 5-year history of highly refractory pancolitis-…
