- Genotype-Guided Dosing Improves Attainment of Therapeutic Troughs in CYP3A5 Normal Metabolizers After Kidney Transplantation. [Journal Article]Am J Health Syst Pharm. 2026 Oct 12. [Online ahead of print]AJ
- CONCLUSIONS: Genotype-guided tacrolimus XR dosing improved early tacrolimus concentrations and significantly shortened time to therapeutic trough attainment among CYP3A5 normal metabolizers. These findings support the use of pre-transplant CYP3A5 genotyping to optimize tacrolimus XR dosing in normal metabolizers.

- Successful Use of Sutimlimab in two Pediatric Patients With Refractory Cold Antibody-Mediated Autoimmune Hemolytic Anemia. [Journal Article]Pediatr Blood Cancer. 2026 Oct 12; :e70751. [Online ahead of print]PB
- Cold antibody-mediated autoimmune hemolytic anemia, or cold agglutinin syndrome (CAS), in pediatrics is typically transient, although it can often be severe and require transfusion support. Recurrence in children is rare, but those with underlying immune dysfunction can have a refractory course with limited options available. We report a 4-year-old girl with Evans syndrome and life-threatening CA…

- Combined empagliflozin and liraglutide therapy protects against tacrolimus-induced pancreatic and renal injury associated with oxidative stress reduction. [Journal Article]Kidney Res Clin Pract. 2026 Oct 08. [Online ahead of print]KR
- CONCLUSIONS: These findings indicate that combined SGLT2i and GLP-1RA therapy confers additive metabolic protection associated with attenuation of oxidative stress in tacrolimus-induced pancreatic and renal damage in this preclinical model, warranting further investigation of its potential as an adjunct strategy for posttransplant diabetes mellitus and nephrotoxicity.

- Co-Administration of Mavacamten and Tacrolimus in a Renal Transplant Recipient With Obstructive Hypertrophic Cardiomyopathy. [Case Reports]JACC Case Rep. 2026 Sep 17; :110345. [Online ahead of print]JC
- CONCLUSIONS: This case uniquely details the longitudinal pharmacokinetic interaction between mavacamten and tacrolimus. This is highly relevant because pharmacokinetic models suggest CYP3A4 induction is dose-dependent; thus, safety demonstrated at 5 mg may not apply to higher regimens like 15 mg.Low-dose mavacamten and tacrolimus coadministration is feasible. However, rigorous tacrolimus therapeutic drug monitoring remains essential, especially during mavacamten dose escalation.

- Small-molecule NFAT signaling inhibitor with dual NFATc1/NFATc2 activity and limited calcineurin catalytic inhibition. [Journal Article]
- Calcineurin-nuclear factor of activated T cells (NFAT) signaling plays a central role in immune activation and represents a major therapeutic target for immunosuppression. Although calcineurin inhibitors such as tacrolimus are clinically effective, their long-term use is limited by severe systemic toxicities associated in part with broad inhibition of calcineurin catalytic activity. In this study…

- Population pharmacokinetic modeling of tacrolimus using real-world data for precision dosing in pediatric liver transplant recipients younger than 36 months during the immediate postoperative period. [Journal Article]
- CONCLUSIONS: Tacrolimus clearance in pediatric liver transplant recipients younger than 36 months is characterized by substantial occasion-specific variability during the immediate postoperative period, consistent with dynamic changes in graft function. Dose individualization should therefore incorporate occasion-specific therapeutic drug monitoring data. The developed model provides a quantitative framework for MIPD, implemented here through a publicly accessible web application (https://smjlab.shinyapps.io/fk506_frontiers/).

- Sex-based differences in clinical phenotype and damage accrual among patients with systemic lupus erythematosus in Saudi Arabia: a multicenter cross-sectional study. [Journal Article]
- CONCLUSIONS: Males showed a distinct phenotype characterized by greater renal involvement, antiphospholipid antibody positivity, renal damage, and more intensive treatment exposure, despite similar global disease activity and overall damage prevalence. These findings suggest that sex-based differences in SLE are more apparent in organ-specific phenotypes and damage patterns than in global disease activity.

- Calcineurin inhibitor monitoring in allogeneic hematopoietic cell transplantation: time to move beyond solid organ transplant paradigms. [Review]
- Calcineurin inhibitors, such as cyclosporine and tacrolimus, have been the backbone of graft-versus-host disease prophylaxis in allogeneic hematopoietic cell transplantation for more than four decades. Yet, the pharmacological framework governing their therapeutic drug monitoring has been significantly imported from solid organ transplantation. This critical narrative review argues that this extr…

- Beyond Crohn's disease: ileal Kaposi sarcoma in a transplant recipient presenting with subocclusive symptoms. A concomitant diagnosis of Crohn's disease and ileal Kaposi sarcoma. [Journal Article]Minerva Gastroenterol (Torino). 2026 Oct 07. [Online ahead of print]MG
- CONCLUSIONS: This case highlights the importance of considering intestinal Kaposi sarcoma in the differential diagnosis of refractory or atypical Crohn's disease in immunosuppressed patients. It also suggests that, in carefully selected cases with localized disease, surgical resection alone may represent an effective therapeutic strategy, allowing continuation of essential immunosuppressive therapy.

- Long-term cost-effectiveness of belatacept vs. tacrolimus in kidney transplantation: Trade-off between metabolic benefit and immunological risk. [Journal Article]Br J Clin Pharmacol. 2026 Oct 06. [Online ahead of print]BJ
- CONCLUSIONS: Belatacept offers metabolic advantages and modest quality-adjusted survival gains but is not cost-effective vs. tacrolimus owing to substantially higher costs. Whether longer lived or higher metabolic-risk recipients derive acceptable value is a hypothesis requiring dedicated age- and risk-stratified evaluation.

- Upadacitinib and upadacitinib-vedolizumab combination therapy for refractory pediatric-onset inflammatory bowel disease. [Journal Article]Intest Res. 2026 Oct 06. [Online ahead of print]IR
- CONCLUSIONS: Upadacitinib demonstrated acceptable efficacy and safety in patients with highly refractory pediatric-onset IBD. In this small subgroup, UVCT was associated with clinical remission in most patients (4/5), suggesting that it may warrant further investigation as a salvage strategy to avert surgery. Prospective, multicenter studies are needed to define the optimal positioning within pediatric-onset IBD treatment algorithms.
